If yes was answered to any of the questions then a "Public Way" permit is required. You can pick up an application at either Community Development, located at 75 South Frontage Road or Public Works, located at 1309 Elkhorn Drive. Notice sign -offs for utility companies. ALL utilities must field verify (locate) respective utilities prior to signing application. Some utility companies require up to 48 hours notice to schedule a locate. y A construction traffic control /staging plan must be prepared on a separate sheet of paper. An approved site plan may also be used. This plan will show locations of all traffic control devices (signs, cones, etc.) and the work zone, (area of construction, staging, etc.). This plan will expire on November 1st and will need to be resubmitted for consideration for approval through the winter. Be aware that your resubmission for winter may be denied depending on the location of construction. i= Sketch of work being performed must be submitted indicating dimensions (length, width and depth of work). This may be drawn on the traffic control plan or a site plan for the job. Submit completed application to the Public Work's office for review. If required, locates will be scheduled for the Town of Vail electricians and irrigation crew. The locates take place in the morning, but may require up to 48 hours to perform. y The Public Work's Construction Inspector will review the application and approve or deny the permit. You will be contacted as to the status and any requirements that may be needed. Most permits are released within 48 hours of being received, but please allow up to one (1) week to process. As soon as the permit is approved, the Building Department will be notified, allowing the "Building Permit" to be released. Please do not confuse the "Public Way Permit" with a "Building Permit ". Box 1230 Clifton, CO 81520 -1230 Cell Phone: 970 - 270 -3689 Home Phone: 970 - 464 -5265 E -mail: sfink80654@aol.com P. 10 On April 3, 2002, an inspection/survey was conducted and 10 bulk samples were collected from the: RESIDENCE Manor Vail Lodge Units 240, Survey; 239 & 24I Assume The purpose of the inspection/survey was to locate and sample suspected asbestos containing materials that might be present in the units that are planned for renovation. The inspection was made and the samples were collected by John R. Peterman, and A.H.E.R -A. and State of Colorado Certified Building Inspector. Great care was taken during the inspection and sampling to be as accurate as possible. It should be noted that minimal damage was done to the existing building structures during the inspection so there is no documentation for unseen conditions or stored items. All samples were analyzed by DCM Science Lab in Lakewood, Colorado. This laboratory is deemed "Proficient" in the E.P.A. Quality Assurance (QA) program for the determination of asbestos in bulk materials, and is accredited by the American Hygiene Association (AIHA). A random sampling scheme was used to sample the suspect materials that were discovered. If during any future demolition or renovation work suspect work suspect material is discovered that hasn't been sampled and would be disturbed, work should be halted until the material has been tested. Apr 09 02 11:17a p.11 The inspection/survey on Manor Vail Lodge, Units 240, 239 and 241 was limited to the interior of the units. The interior walls are wood frame with lightly textured sheetrock on them. The ceilings are sheetrock with a heavy spray on texture. The floors are covered with carpet and 2 types of sheet vinyl. The heating is provided by hot water base board heat and a fireplace. No visible suspected asbestos containing materials were present on the heating system. The three units are joined by a common hallway. Laboratory analysis of the bulk samples indicate that the ceiling texture contains asbestos in excess of the regulatory limit (greater than 1 %). The composite sheetrock samples contain asbestos that is less than the regulatory limit. The asbestos content of the composite sheetrock samples was verified by the required point count analysis. As long as the sheetrock on the walls is removed by facility component methods. No asbestos abatement work practices would be required. Work that would impact directly on the muddied areas of the sheetrock only should be done under asbestos abatement work practices. � be properly abated prior to disturbance of the material. Only Unit 241 and the common hallway were accessible at the time of the survey.
