Asbesros Containing Materizls LVI will perFomi cemoval of all selected A.C.M. seheduled for removal in aceordance with the OSHA, L'PA, and S[ate of Colorado Regula[ion 8. All abatement requirements for asUestos are defined in 29CFR1926 . 1101 . These requirements will include the use of regulated work area isolation systems, HEPA Filtration Devices, respiratory protection devices, protective clothing, a�nd worker decontanunation facilities . Specific details regarduig the use of tllese work pt•ocedures a�id methods are provided in the asbestos worlc procedures section of this Work Plan. The iippermost consideration for this project is to prevent health and safety hazards for the public, building occupants, and asbestos abatemeut team. With this im m�ld, befoi�e any abatement work proceeds, the Owner / General Cont�actor shall shiit down and/or isolate all heating, air conditioning nitake aud exhaust openings in the abateinent a1•eas . Procedures to perform tl�e above t•efet•enced items are contained within LVI ' s Corpor•ate Safety Prograin. III. LVI will utilize half-face negative pressure respirators equipped with HEPA filters for pre-cleaning, preparation, final cleail, secondary containmeilt and non-friable removal activities . LVI will ntilize full face negative pressure and powered air purifiers for gross removal and gross debris clean-up within the fiill enclosure areas. At no time will LVI allow employees ' exposure inside the provided respirator to exceed 0 .01 f/cc. All employees utilizing respirators will be given training in the proper use, cleaning and maintenance of the provided respirator. All employees will also receive qualitative fit testing prior to use of negative pressure respirators to ensure adequate protection is being provided to the einployee. f Details concei7iing the proper use, cat�e and maintenance of the respirators are provided in LVI ' s Respiratory Protection Program in the Cot•porate Safety Manual . s` i � € � ! . � ' �� i . � _ _ �' _ _ __ _ _ _ _ _ _ IVo SAFE'TI' �IZ�CEDiJ12�S Safetv LVI has instituted strict safety and healYh policies for performance of asbestos abatement projects to ensure that all employees are properly trained to perform their speciiic job taslc. All abateinent work will Ue carefizlly plamied and analyzed in advance to determine the safest, most efficient methods to successfizlly complete the specific tasks without incident. LVI ' s safety and health policies strictly adhere to all OSHA regulations as well as any state and/or local requirements . Prior to initiation of work on the project, all employees will receive the LVI site-specific training, a fiill orientation of this Work P1ai1, site condition and site-specific training and a review of the emergency procedures for the project. Safety meetings will be held at the job site daily to inform employees of job specific hazards , emergency procedures and required work practices. �. EMERGENCY PROCED�JI2ES The following emergency procedures will be implemented upon initiation of worlc on the abatement project: ' Personnel Exposure ' 1 . Remove worker to fresh air and provide basic first aid procedures as required until ' medical personnel ai�rive. ', 2 . Decoutaminate the worker and any affected areas after removal from the contaminated ' area. ' 3 . Sununon medical assistance and/or transpor•t to the designated medical facility for treatment. 4 . Notify the Site Safety Officer, Project Manager, and LVI Emergency Persoru7el . Persomiel fi1 'Lv On-Site 1 . Provide basic fi1•st aid procedures as required by injury. 2 . Decontaminate the worker and any affected areas after removal from the work area. 3 . Sununon inedical assistance and/or transport as required by the injury. Notify Site Safety Officer, Pi•oject Manager, and LVI Emergency Personnel. 4. LVI' s roreman or otller designated representative shall accompany the injut•ed employee to the location of inedical exanunation and t�eatment. The designated representative shall � discuss with the physician the feasibility of placing the injured employee in a limited duty assignrrient. The physician shall complete the Medical Status Report �ection of the "LVI Pet•sonal Injury/Illness Report" ft•om 541 �0129F after treatment of the employee. S . The LVI foreman shall complete the remaining sections of the "Personnel Injuty/Illness Report", except for the section on severity. The report shall be provided to LVI ' s Contract Administrator and Field Representative on the day of the injury. i 6. Within 24 houis of the injury, LVI will provide copies of the "Employer' s First Reports � of Injury or Illness", to LVI' s Contractor Administrator and Field Representative 3 7 . LVI will proinptly investigate any incident involving an accident, injury, serious "near ! miss", or the recognition of a potential hazard to health and envir•onment. LVI will not attempt Yo figl�t the fire beyond reasonable means accomplished with fire extinguishers. 3 . LVI will direct all evacuated employees away from the fire ancl/or explosion to a safe location upwnld. A designated locatiou to meet will be assigned by the onsite supeivisor and serve as a gathering place where all employees can be accounted for. All emergencies will immediately be repot�ted to the I,VI Emergency Pet-sonne9, Weecycle and the Owner. ASI3ESTOS AIR MOI�I'I'ORI1�1G Personal Air Monitorint LVI shall secure the seivices of an approved asbestos third pat•ty air-monitoring laboratory to perfonn all OSHA employee asbestos exposure sample analysis . All sample results will be , analyzed within the time constraints allowed by OSHA and be posted on site for review by ' the affected employees. Any einployee exposures above the PEL of 0 . 1 f/cc or 1 .0 f/cc will be immediately reported to the LVI Safety Departinent. Followuig completion of the project, a copy of the personnel air monitoring results will be submitted to LVI for the permanent ' records. ! . � ��. -� � �1IIIo MO�ILIZ�-1TION/SITE I�OGIS'TICS Mobilization Prior to the mobilization process, LVI will complete �nd submit all pre-work submittals, and notifications required Uy the project speci�cations . Utilit,�Requirements LVI will require a source of teniporaly power and water for this project. LVI will require potable water for flie decontamination facilities and to wet the ACM prior to and during removal tasks . The water source shall be capable of providing approximately 35 psi of water pressure at each location. LVI will provide water hoses for coimection to btiiilding and or owner provided water tank systems. Pre-Abateinent Worlc After establislunent of the necessary job site facilities, LVI will mobilize work crews to the ' site to begin abatement operations for the project. Conunencement of abatement work on the ' project shall not occur until : 1 ) all pre-work submissions, notifications, postings and permits ' have been obtauled and are satisfactory to Weecycle and the Owner, 2) all pre job safety ' plamling and worker training have been completed; 3) all site security arrangements have i been established; 4) all equipment for abatement, cleanup and disposal are on site ; and 5) LVI has received written "Authorization to Proceed" fi�om Weecycle and the Owner IX. ASBESTOS A13ATEMEI�IT WORK AREA ISOLATION SYSTEIVIS Wark Scope j Please see Section I, General Stateinent. Full and or Secondary Containment Procedures Ove�-view To segregate the abatement work areas fiom all other areas during abatement activities, LVI will install war�iing barrier tape and signs adjacent to the work area. The wanling tape shall serve as the �rst line of demarcation for all abatement work areas . X. ASBESTOS A]BAT]EIVIE�IT PItOCEDUI2ES, GEIVEI2AI� Facilit. Component Procedtues : LVI will initiate removal of the identified asbestos containing materials . Removal of fliese ; materials will typically begin at the area farthest froin the decon unit location and proceed toward the decon units. Prior to removal of any asbestos materials non-toxic, non- hazardous cheinical will be applied with low-pressure sprayers to thoroughly saturate the asbestos materials. Once the asbestos has been inade adequately wet, it will be wrapped in two layers of six mill poly and removed inavageable sections . ACM will not be allowed to accumulate t on surfaces and dry out. Before the end of each work shift, all abated materials will be � � i - �;�. .�' ; _ ��r� � -�� � - � ; ;- ' ��= � - � ; i a � � ; properly wrapped and will be moved to a waste storage area inside coiitainment for staging prior to reinoval from the contauunent Fine Cleani�zg Procedures Upon completion of all removal activities the regulated area' s will be �ne cleaned utilizing wet inethods and/or HEPA vacuums to remove any remainulg visible ACM. All wastewater generated during the �ne cleaning process will be collected and �ltered and disposed of ulto the sanitary sewer system. Work Area Clearance Procedures After completion of the fine cleailiug activities and removal of all waste materials, LVI will request a visual 'ulspection of the as•ea by Owners Air Monitorin� Specialist or .Building Inspector. Waste Removal Procedures All asbestos containing/contaminated material regulated by CDPHE, other than that to be disposed of through direct waste load out procedures, will be disposed of in 6-mil poly bags . Prior to removal of the waste materials through waste load out areas , all waste bags will be cleaned and placed in a second six-mil bag for transport to the waste containers. The outside of the individual waste bags will be hand wiped with wet towels to remove any asbestos coutamination on the outside of the bag and iirunediately placed ui the second disposal bag. The waste bags will then be passed through an airlock constructed for equipment and waste materials to the outside of the containment isolation system. Once the waste bags are removed from the contaimnent, they will be placed in a poly lined waste bui for transport to the designated waste container on site . All ACM bags for this project will be printed or tagged with the required OSHA warning labels, DOT laUels and asUestos generator information require by regulations .
