As requested, sftrcaral steel special inspection was conducted at the above referenced project per
the IBC 2003 Edition - Chapter 17: Structural Tesb and Special Inspection and project requirements. I
arrived on site at 10:30 AM. I met witb you to review the project reguire,ments and specifications. The steel
erector fo this project is Trinity Welding Inc. in kadville, Colorado. The welder qualffication test reports
for welders on site were reviewed and observed to be acceptable wift project specifications. Copies of
certificdions are on file with Trinity Welding. Wbile on site I received copies of thee engineering
clarifications from the project structural engineer, Ivft. Kinberly R McGhee with KRM Consultails, Lrc. in
VaU CO. dAtrrJ,823,9112, andgllgl0T.Please refer to the attached reports for documentation.
Visual welding inspection was conducted on Unit #3, #5,  'tilizing fte following drawings
provided by KRM Consultants, Inc.: ) Unit #3 - Sl & 52: Job ti0606-16,2) Unit #5 - Sl, 52, & 53: Job
#0606-17, and 3) Unit #8 - Sl, 52, &S3: Iob #0607-28. Inspection was conducted fiom the Garage Level to
the Roof Level on all unit liste4 where applicable, All fillet welds inspected were observed to be as detailed
and in complirnce with pmject specifications and submitted engineering modifications.
This concludes all rcquired structural steel special inspection per Chapter l7 and project
requirements by Thunder Mountain Testing for the Tyrolean Residence Remodel projects referenced All
areas of inspection werr observed to be in complimce with project specifications.W

r All new @nstruction within the Town of Vail is considercd to be of unusually tight construction,
thus all combustion air is rcquircd to be drawn from outside the structure for mechankal
equipment.
Town of Vail Fircolace Ordinance
In September of 1991, the Vail Town Gouncil adopted an oldinance which restdcb te construction and
use of open heath fireplaes within municipal boundaries. Since that time the ordinane has
undergone numerous <franges and revisions, striving for compromise, yet effiectiveness in addrcssing
the aar quality issue. Thercfiore the following criteria has been adopted:
o Construction of open hearth wood burning fireplaces is no tonger permitEd within Town of Vail
municipal bounparies.
r Dwellino Units - Each nerr dwelling unit may contain:
One (1) EPA Phase II certified solid ftrel burning devioe and no more than two (2) gas applhnes
(B vent)
OR
Two (2) gas log fireplaces and no more than two (2) gas appliane ftreplaces (B bent).
r Restricted Dwellino Units- Each new rcstricted dwelling unit may contain:
One (1) gas log fireplace and not more than one (1) gas appliance fireplace.
o Accommodatiod Units - Each new aooommodation unit may contain:
One (1) gas log fireplace or one (1) gas appliance fireplace.
r If two or mone separate dwelling units or acommodation units are combined to form one lalger
unit, the combined unit may retain one woodburning fireplace (if one alrcady exists) and no morc
than 2 gas appliance fireplaces, or may @nvert up to two existing fireplaces b gas.
o If during the ource of a remodel an existing woodburning ftrcplace is altercd or movedr the unit
must then comply with all provisions of the ordinane. That is, the fireplace must be onvel&d to
natural gas or rFplaced by an EPA Phase II certified unit.

1. Design heating and cooling loads for the building must be determined using procedures in the ASHRAE Handbook of Fundamenlals
or an approved equivalent calculation proceduro.
2. All equipment and syslems must bs sized to bo no greater than needed to mest calculatsd loads. A single piece of equipment
providing both heating and cooling must satisty this provision lor one funclion wilh the capacity for the other funclion as srmll as
possible, within available equipment options.

If this permit requires a Town of Vail Fire Department Approval, Engineer's (Public Work) review and approval,
a Planning Department review or Health Department review, and a reviary by the Building Department the
estimated time for a total review will take as long as three (3) weeks.
All commercial (large or small) and all multl-family permits will have to follow the above mentioned maximum
requirements. Residential and small projects should take a lesser amount of time. Howwer, if residential or
smaller projects impact the various above mentioned departments with regard to necessary review, these
projects may also take three (3) weeks to review and approve.
Every attempt will be made by this department to o<pedite this permit as soon as possible.

How it relatE to Building Permits:
Fill out the attached check list with the Buildinq Permit Application.
If yes was answered to any of the questions then a "Public Way" permit is required. You can pick up an
application at either Community Developmen! located at 75 South Frontage Road or Public Works' located at
1309 Elkhorn Drive.
Notice sign-oft for utility companies. ALL utilities must field verifo (locate) respective utilities prior to signing
application. Some utility companies require up to 48 hours notice to schedule a locate.
A construction traffic control/staging plan must be prepared on a separate sheet of paper. An approved site plan
may also be used. This plan will show locations of all traffic conFol devices (signs, cones, etc.) and the work
zone, (area of consfuction, staging, etc.). This plan will expire on November 1$ and will need to be resubmitted
for consideration for approval through the winter. Be aware that your resubmission for winter may be denied
depending on the location of constuction.
Sketch of work being pefformed must be submitted indicating dimensions (length, width and depth of work).
This may be drawn on the faffic control plan or a site plan for the job.
Submit completed application to the Public Works offce for review. If required, locates will be scheduled for the
Town of Vail electricians and irrigation crew. The locates take place in the morning, but may require up to 48
hours to perform.
The Public Works Construction Inspector will review Ere application and approve or deny the permit. You will be
contacted as to the status and any requirements that may be needed. Most permits are released 48 hours of
being received, but please allow up to one (1) week to process.
As soon as the permit is approved, the Building Departnent will be notified, allowing the "Building Permif'to be
released. Please do not confuse the'tPublic Way PermiY'with the "Building Permit".
NOTE: The above prooess is for work in a public way ONLY. Public Way Permits are valid only until
November 15th. A new Public Way Permit is rcquired each year if work is not complete. Re-
application each November 15s does not mean an automatic tenewal.

be littered, tncked or deposited, sand, gravel, rocks, mud, dirt, snow, ice, or any other debris or material upon any
street, sidewalk, alley or public place, or any portion thereof.
Notice; Abatement: The Director of Public Wo*s may notiff and require any person who violates or @uses another
to violate the provision of subsection A hereof, or who has in the Director's employment a person who violates or
causes another to violate the same, top remove such sand, gnvel, rocks, mud, dirt, snow, ic€ or any other debris or
material within twenty four (24) hours after receipt of said notice by the Director of Public Works. In the event the
person so notified does not comply with the notice within the period of time herein specified, the Director of Public
Works, or other authorized agent, may cause any such sand, gravel, rocks, mud, dirt, snow, ice, debris or any other
material to be removed from any street or alley at the expense of the notified.
Summons and Penalty: As an altemauve to the notice for removal provided in subsection B above, any person

Problem Statement
The Vail Village and LionsHead Mllage are invaluable assels to the community. Thousands of
people come to Vail each year to sightsee, recreate, shop and to enjoy everything the Vail Valley
has to ofier. The community and merchants rely upon our guests to generate revenue. Our
summer/winler guests expect a pleasant experience while in Vail.
To insure that all of the proposed construction has as litfle negative impact on the community and
on our guest as possible, the Town of Vail finds it imperative to create and implement the Vail
Mllage and LionsHead Village Construction Hours Handout.
It would be irresponsible for the Town of Vail, the construction contractors and the merchanls, to
not actively participate to minimize the impact of construclion in the Vail and LionsHead Village
areas.
The Vail Village Streetscape Project will continue through 2007. lmprovements
proposed/scheduled for 2007 will bo concentrated in the Siebert Circle lmprovement
Project, Willor Road/lntemational Bridge and on E. lleadow Drive between Village Center
Chute and Sliier Plaza. Additional streetscape improvemenls are scheduled for East
Meadow Drive and Village Genter Ghute pendlng the Crossroad/Solaris Construction
project.
All construction projects and/or remodels within the areas mentioned above should
contact Scott Bluhm, Town of Vail Streetscape Coordinator, at 970{77-3418 for further
information and coordination.
Givens
The following givens are intended to provide the basic foundation by which construction will be
completed in the villages:
o The public's health, safety and welfare shall be honored at all times.
e Adequate pedestrian, loading/delivery, vehicle, and emergency vehicle access and circulation
shall be maintained.
. Roadways and pedestrian walkways shall be kept clean and free of dirt and debris.
e All construction deliveries, equipment, tools and materials entering the Vail Mllage and
pedestrian areas must go lhrough CheckPoint Charlie. All LionsHead mnstruction activity
must go to the Vail Police Department for a parking permit. Do not use delivery zones for
construction parking; refer to approved staging plan or make other arrangements for parking.
o The hours of outside construction activity shall be as follows:
- April 1 5 - June 22 & September 4 - November 16, 7:00 a.m. until 7:00 p.m., seven days a
week.
June 29 thru September 1, work must end by 4:00 p.m. on Fridays.
November 12 - April 15, 2008 work allowed within daylight hours only.
- Deliveries shall be restricted to the hours of 7:00 a.m. to 11:30 a.m. on Gore Creek Drive
and 7:00 a.m. to 8:30 a.m. on Bridge Street.
- Special conshuction delivery permits must be requested in advance from Public Works in
conjunction with Code Enforcement at Checkpoint Charlie.
- No construction activity shall occur on Saturday or Sunday between June 25 and
September 3.

INTRODUCTION:
On March lziln,2oT an inspectior/survey was conducted and 3 bulk
samples were collected from the:

The purpose of the inspection/survey was to locate and sample
suspected Asbestos containing materials that might be present in the area of
the Residence Unit that is planned for renovation.
The inspection was made, and the samples were collected by John R.
Peterman, an A.H.E.R.A. and State of Colorado Certified Asbestos
Inspector. Great care was taken during the inspection and sampling to be as
accurate as possible. It should be noted that minimal damage was done to
the existing building structures during the inspection so there is no
documentation for unseen conditions or stored items.
All samples were analyzed by DCM Science Lab in Wheatridge, CO.
This laboratory is deemed "Proficient" in the E.P.A.Quality Assurance (QA)
program for the determination of asbestos in bulk materials, and is
accredited by the American Hygiene Association (AHA).
SAMPLING PROTOCOL:
A random sampling scheme was used to sample the suspect materials that
were discovered. If during any future demolition or renovation work,
suspect material is discovered that hasn't been sampled and would be
disturbed. work should be halted until the material has been tested.

RENOVATION AREA DESCRIPTION:
The Residence at The Tyrolean, Unit #3, is planned for selective renovation.
The interior walls are covered in sheetrock with a light texture, and the
ceilings are covered in sheetrock like the walls. The floor is covered with
finished wood, and ceramic tiles. The heating is provided by baseboard
heating, and a fireplace.
CONCLUSIONS & RECOMMENDATIONS:
Laboratory analysis, of the bulk samples, indicates that no Asbestos was
detected in the bulk samples.

Bulk Sample Analysis
Paee A otABULK SAMPLE ANALYSIS PROCEDURES:
DCM Science Laboratory, Inc. analyzes bulk asbestos samples following procedures developed by the
McCrone Research Institute and in compliance with guidelines established by the Environmental Protection
Agency (EPA€00/R-93/1 16, July, 1993).
Bulk samples are prepared for analysis using a 10X-80X stereo microscope in a hepa filter hood which provides
a contamination-free environment. The sample is then analyzed by polarized light microscopy (PLM) at 100X.
When the sample consists of more than one layer, each layer is prepared and analyzed separately. Fiber and
matrix materials are identified by the characterization of optical properties including color and pleochrorism,
form, cleavage, relief, birefringence, e)dinction, orientation, twinning, interference figure and other distinguishing
features. Dispersion staining is also used to further aid in mineral identification. All percentages of asbestos,
otherfibers and non-fibrous constituents are calculated from the values obtained from the stereo and PLM
microscopes analysis. ln-house and NIST standards as well as a chart prepared by R.D. Terry and G.V.
Chif inger for "The Journal of Sedimentary Petrology'', (Volume 24, pp.229-234, 1955) provide a guide for
estimating percentages. All samples are archived for six months unless other arrangements are made by the
client.
