General Information: This application is for all changes to approved plans prior to Certificate of Occupancy. An ap-
plication for Design Review cannot be accepted until all required information is received by the Community Development
Department. Design review approval expires one year from the date of approval, unless a building permit is issued and
construction commences.
Submittal Requirements:
1. Three (3) Copies of all pertinent approved plans will illustrated, labeled changes
2. Joint Property Owner Written Approval Letter, if applicable

The following is a narrative of changes made to the permit set for the Gingrich residence remodel originally
dated 7-8-09 for Permit and resubmitted on 10-6-09 with minor adjustments. The following modilkit;om
are clouded on page Sl and described below.
3/S1: 20 at 16'o.c. existing bearing wall field verified.
3/S1: Existing 2x 12 rafter with the addition of (3)11.25 LVL new rafters sisWed to and connected each one
to another w;' (2 )rows of 16d at 12"o.c. as well as glued.
2/S1: Field verified existing 2x 10 rafters ok at new (2)2x 12 subfascia.
2/S1: Ne~'k (3)16" LVL beam w associated detail A.'SI replacing existing 2x4 at 16"o.c. bearing wall.
1/S1: Removal of a portion of existing foundation stem wall for access to mechanical room w' associated
details kADD and B ADD for existing foundation repair and underpinning.
