and Environmental matrices by the National Voluntary Laboratory Accreditation Program (NVLAP), Lab
Code # 101896 for Transmission Electron Microscopy (TEM) and Polarized Light Microscopy (PLM)
analysis and the American Industrial Hygiene Association (AIHA), Lab ID 101533 - Accreditation Certificate
#480 for Phase Contrast Microscopy (PCM) analysis. This laboratory is currently proficient in both
Proficiency Testing and PAT programs respectively.
Reservoirs Environmental, Inc. has analyzed the following samples for asbestos content as per your
request. The analysis has been completed in general accordance with the appropriate methodology as
stated in the attached analysis table. The results have been submitted to your office.
RES 148759-1 is the job number assigned to this study. This report is considered highly confidential
and the sole property of the customer. Reservoirs Environmental, Inc. will not discuss any part of this study
with personnel other than those of the client. The results described in this report only apply to the samples
analyzed. This report must not be used to claim endorsement of products or analytical results by NVLAP or
any agency of the U.S. Government. This report shall not be reproduced except in full, without written
approval from Reservoirs Environmental, Inc. Samples will be disposed of after sixty days unless longer

As requested on 1/3/08 we performed a asbestos survey building inspection, includes a
tactile and visual survey at the above referenced location of accessible materials in the residence
(request by the client).
The purpose of asbestos sampling was to document the specific building materials that
may contain asbestos prior to any renovation, reconstruction or demolition and remediation.
All samples are then taken and delivered to Reservoirs laboratory, a certified and
accredited laboratory. The laboratory analytical results, if any of the samples are attached to this
report. We do not accept any responsibility for these analyses.
Sampling [Type/Method]: Bulk sampling — sampies were collected using wet methods to
minimize the possibility of a fiber release. Bulk sampling; is done by knife or other similar
coring devices, suitable to cut through the entire thickness of the material being sampled. Bulk
samples are for the purpose of a cross-section of the sample building material to be collected and
sent to the lab for identification.
Site Visit/Work performed:
To save the building owner additional expenses, we tested randomly throughout the residence as
required by Colorado Reg 8 guidelines for suspect materials that may have contained asbestos.
In some areas; work performed was a non-destructive sampling inspection, in other areas was a
minimal destructive sampling. These areas will need to be abated by a professional abatement
contractar based on the lab sample results. These areas are listed on the assessment form, 19

Lab Results / Asbestos Containing Building Materials identified by sample:
All samples show no asbestos (Non-Detect) as listed in Reservoirs Laboratory report.
IV.E.3.- a homogeneous area is considered not to contain ACM only if the results of all
samples required to be collected from the area show asbestos in a amounts of one percent or less.
N.E.3.b — a homogeneous area shall be determined to contain ACM based on a finding
that the results of at least one sample collected from the area show the asbestos is present in an
amount greater than one percent.
All samples meet Colorado Regulation 8/ section IV.E.3.a., which does not require
abatement in these area(s). This renort is limited to these 19 samples areas taken or other areas
outline in the report as written "suspected ACM "
Observations (Visible Inspection):
Two floor structure with crawl space, built in early 1980's, stucco/wood exterior and flat
roof. Wooded floors on first level, carpet on: stairs and second floor. Attached garage: walls
and ceiling are homogenous with first floor & second floor bathroom (bedroom w/ HVAC
room). The common wall (between units) and exterior wall is wooded design for upstairs
bedroom (w/ HVAC room) and first floor bedroom. There wall paper covering common wall in
living room (fireplace).
All wall and ceiling are homogenous in texture and color of the following first floor:
entry way, kitchen, dining room and main living room and stair well. On the second floor:
hallway and loft area, master bedroom (jacuzzi bathtub and walk in closet), laundry closet (walls
only). New addition / master bedroom is all wall are wood, even the bathroom, ceiling is smooth
in texture like other two bathrooms. Laundry closet ceiling is popcorn texture, a cavity between
roof and ceiling.
In the crawl space: copper plumbing and insulated foundation walls. No barrier covering
damp soil. Heating is radiant heat (two tanks and radiant unit located on 2"d floor bedroom (all
white in color), the electrical is romex cables.
Recommendations:
Follow all EPA, OSHA and Colorado Regulations.

inspected, and is limited to areas inspected as listed in this report. This inspection is not intended to be
technically exhaustive and is not to be used as a guarantee or warranty, expressed or implied, regarding
the adequacy, performance or condition of the inspected structure. This report is subject to the limitation
as agreed in writing.

Proposed Mud Room Addition

As requested, a representative from our office observed the exposed material in the base of the foundation excavation for the
proposed mud room addition on May 4, 2010. The purpose of our observation was to verify that the exposed material is suitable
for supporting the foundation for the proposed mud room addirion.
The foundation excavation was located in the northeast corner of the proposed mud room adjacent to the existing duplex. The
foundation excavation was approximately 4 feet deep below the existing ground surface. Materials exposed in the base of the
excavations for the proposed foundation consisted of a brown, slightly moist to moist, dense silty sand with scattered cobbles.
Neither ground water or deleterious material were observed within the base of the excavation.
Based on our observations, it is our opinion that the subgrade soil shouid support a foundation system designed for a maximum
allowable soiI bearing pressure of 1,500 psf with a low risk of movement. Our opinion is based on the soils within the base of the
foundation excavation remaining dense, unsaturated, and frost free.
The opinions presented in this letter are based on our observations of the material exposed in the base of the excavation and our
experience with similar subsurface condirions and projects in the area. It is possble that soil conditions could change below the
exposed foundation excavation. We appreciate the opportunity to provide this service. If we can be of further assistance, please
