Gentlemen:
As requested, a representative of Hepworth-Pawlak Geotechnical, Inc. observed the
reinforcement steel of the pool at the subject site on June 16, 2010. The services were
performed in accordance with our agreement for professional engineering services to Vail
Plaza Hotel, dated May 5, 2010.
The reinforcement steel had been placed at the time of our site visit. Additionally,
reinforcement had been placed in the stairs area according to Monroe&Newell
Engineers letter dated June 16, 2010. The results of our observations are attached.
Based on our observations, the reinforcement steel placement met the project
requirements.

Gentlemen:
Monroe &Newell Engineers, Inc. is the Engineer of Record for the renovation project at
the Vail Plaza Hotel. We have reviewed the reinforcement of the new pool that is in
place. The shape of the excavation required additional reinforcement and concrete to be
installed under the entry stairs into the pool. Section 1/S2 shows the reinforcement that is
required for the pool walls. As the excavation of the pool does not allow for a sloping
bottom of the stair slab, the contractor shall install additional reinforcement in addition to
the bars shown in section 1/S2 under the steps:
1. Vertical reinforcement shall be#5 spaced at 18"on center in each direction.
2. Horizontal reinforcement shall be #5 spaced at 8" on center each way and in
layers that are spaced at 8"on center.
If this additional reinforcement is installed the pool construction on site will be in
compliance with the intent of the construction documents. The additional concrete weight
will only result in an incremental increase in weight on the raft slab below and will not
result in overloading the soils below the raft slab. If you have any questions or

Pool Remodel.
Building Department Comments:
1. Please revise plan submittal to use consistent plan sheet size.All plans submitted shall
be drawn at scale and plotted on the same sheets size.
I have attached reprinted architectural sheets in 24'x36"format. See drawings A2.1,
A2.2&A2.3.
Architectural Comments:
2.All sheets of the architectural plans are required to be stamped and signed by a
Colorado State Licensed Architect. IBC 107.1
I have attached new stamped and signed sheets of the required drawings.
3.Include Floor Plan, or Site Plan showing proposed location of the new firepit.
I have attached sheet A2.1-Architectural Site Plan to locate the fire pit and to
address other additional comments.
4. Revise architectural plans to provide an accessible service counter area at the new pool
bar per ANSI 117.12003.
We have removed the pool bar and will provide food and beverage service by other
means. The bar has been removed from the architectural site details.

Accessible parking spaces and accessible route to the pool deck are existing.Access
around pool deck is accessible including to the pool and 25%of the provided spas.
Access to the pool and accessible spa will be by our existing ADA compliant Hoyer
Pool Lift(Model SS-HSP-1). See Pool Drawing PL1.o for sleeve locations for lift
mounting.
6.Revise plans to show 4' minimum height for pool enclosure per IBC 3109.3.All gates
shall have self closing and self latching gates.
We will maintain a 4'high pool enclosure per IBC 3109.3 as indicated on sheet A2.1.
The existing gates are self-closing, self latching&4'-o"high per IBC 3109.3•
Structural Comments:
7. Please include complete structural plans stamped and signed by a Colorado State
Licensed Engineer for the proposed gunite pool and spa construction. IBC 107
Stamped and Signed structural plans by Hannes Spaeh of Monroe&Newell have
been provided for the proposed gunite pool and spa construction. See attached
drawings Si, S2&S3.
8. Please include analysis from a structural engineer for the proposed additional dead
loads in the pool deck planting areas and patio areas.
Structural analysis by Hannes Spaeh of Monroe&Newell has been provided. See
attached documents.
9.Revise plans to include construction plans for the relocation and support of the stainless
steel hot tubs. IBC 107

INSTALLATION PREPARATION
Please carefully follow the steps below when: 1)Selecting the Location.2)Construction of the
Enclosure. 3)Installation of the HWI Fire Pit.The steps listed as WARRANTY REQUIREMENT must
be strictly followed to qualify for product warranty.Warranty will be void if not followed.
SELECTING THE LOCATION
-HWI FIRE PITS ARE DESIGNED FOR OUTDOOR USE ONLY.HEARTH
PRODUCTS CONTROLS CO.MUST BE NOTIFIED OF ANY OTHER USE.
-FIRE PITS CREATE VERY HIGH TEMPERATURES-IT IS VERY IMPORTANT
THAT COMBUSTIBLES BE KEPT AT SAFE DISTANCES.
• WARRANTY REOUIREMENT:For installation of I l0vac or 24vac powered control systems,
it is required to install a wall switch or breaker for the fire pit electrical outlet away from the
enclosure to prevent unwanted ignition of the fire pit.
• WARRANTY REQUIREMENT:The fire pit location must accommodate a gas shut off outside
of fire pit enclosure.The gas line should be a minimum of 3/4"or larger based on fire pit size.
• To enjoy your fire pit,select a well drained location that allows for sufficient clearance from
combustible materials.
• Choose a location that allows easy access for installation and maintenance of the fire pit.Make
sure that trees and shrubbery are well clear around and above the fire pit.
• Pick a location that allows sufficient horizontal room to enjoy the fire pit while allowing a safe
distance from the heat and flame.
• Select a location where the fire pit can be attended during operation. Never leave an operating fire
pit unattended or by someone not familiar with its operation or emergency shut off locations.
Wooden or solid surfaces such as granite or marble must be located far enough away that
they do not reach a temperature of more than 100 degrees F plus ambient air temperature.
Example:If surrounding air temperature is 70,the wood surface temperature must stay at or
below 170 degrees F.

CONSTRUCTION OF THE ENCLOSURE
- THERE MUST BE AN ELECTRICAL(WALL SWITCH OR BREAKER)AND GAS
SHUTOFF ON THE EXTERIOR OF THE FIRE PIT TO ALLOW FOR EASY ACCESS FOR SHUTDOWN
OR IN THE CASE OF AN EMERGENCY.
-ALWAYS USE PROPER MATERIALS AND CONSTRUCTION FOR GAS SUPPLY,
POWER,AND ENCLOSURE.
• WARRANTY REOUIREMENT:The enclosure must be constructed on a stable surface. The
weight of the fire pit must be supported by the pan-not by the control box.Make sure that the fire pit

• is high enough that the control valve box is above the grade to prevent water damage to the controls
inside the box. NEVER install a HWI fire pit below ground level.Drainage must be provided for the
enclosure to prevent water accumulation leading to damage to components in the valve box.
• WARRANTY REQUIREMENT:The enclosure must incorporate 1 vent on at least two
sides at a minimum size of 18 sq.inches each(Example:3"x 6"or larger)to allow heat within the
enclosure to escape.Failure to do so will result in the fire pit automatically shutting down when
internal valve box temperature reaches 175°F.This could lead to heat damage to internal
components.Some enclosures may require more ventilation based on material,size,and
extended use.This vent may work as a drain as well when installed at bottom sidewall to prevent
water build up.
• WARRANTY REQUIREMENT:The interior void space of the enclosure surrounding the
valve box cannot be filled with any material(gravel,crushed rock,concrete,etc.)-It is a
requirement to have a minimum of 6"under the valve box for proper ventilation.
• Select materials that are non-combustible in both initial installations as well as over time.
• Make sure that the structure is level. We recommend the use of the installation collar(optional)
that may be mortared into the surround.
INSTALLATION OF THE HWI FIRE PIT ASSEMBLY
-CONFIRM THIS APPLIANCE IS BUILT FOR GAS USED-NATURAL GAS OR LP.
DO NOT USE NATURAL GAS APPLIANCE WITH LP OR LP APPLIANCE WITH NATURAL
GAS.REFER TO LABEL ON THIS APPLIANCE.
-TO PREVENT DAMAGE,UNHOOK FIRE PIT FROM GAS SUPPLY FOR
PRESSURE LEAK TESTS GREATER THAN 1/2LBS PRESSURE.
• WARRANTY REQUIREMENT:The HWI fire pit should be supported by the pan when placed
into enclosure.The control box should not support weight of assembly.
• WARRANTY REQUIREMENT:The fire pit assembly should be recessed a minimum of 4"
from the top of the enclosure to protect flame from being blown out.Some areas may require
more-8"is not uncommon.
• WARRANTY REOUIREMENT:All fire pits come with a 13"x 13"sheet of insulation between
pan and valve box to protect internal components from heat damage.This may need to be
trimmed on smaller enclosures for proper fit.PLEASE USE THIS AT ALL TIMES.
• WARRANTY REQUIREMENT:When filling the pan with lava rock and/or decorative glass,
the instructions on Pg.5 must be followed.
• The main gas should already be plumbed to the location of the fire pit area. The 24"flex line
coming from the fire pit should be connected to the main gas line. Tighten the flex line fittings to
the gas supply stub and to the fire pit.Avoid sharp bends with flex line to prevent whistling.
• Turn on main gas supply and check all fittings in and around fire pit for leaks using a leak
reactant,leak detector or soapy water. If leaks are found,shut off gas supply repair leaks and
retest.
• The 3'power cord can be either tied into the main power supply for use with wall switch or
or plugged into remote receiver to use remotely.POWER MUST BE TURNED OFF TO
ELECTRICAL OUTLET WHEN NOT IN USE.
HWI FIRE PIT OPERATION
WARRANTY REQUIREMENT:WHEN NOT IN USE,THE FIRE PIT MUST BE COVERED AT ALL
TIMES TO PREVENT DAMAGE TO ELECTRONIC COMPONENTS.
-BEFORE USE,BE SURE TO TEST ALL GAS CONNECTIONS FOR LEAKS.DO NOT USE
FIRE PIT IF THERE IS ANY EVIDENCE OF LEAKING GAS. IF LEAKING GAS IS SUSPECTED,TURN
OFF THE MAIN GAS SUPPLY AND REPAIR IMMEDIATELY.
-WHEN FIRE PIT IS NOT IN OPERATION,POWER TO ELECTRIC OUTLET MUST
BE TURNED OFF VIA WALL SWITCH OR BREAKER
-NEVER USE ANY MATERIAL THAT IS NON-POROUS AND HOLDS MOISTURE LIKE
GRAVEL,PEBBLES,RIVER ROCK,ETC. THIS MATERIAL,WHEN HEATED WILL CAUSE THE
TRAPPED MOISTURE TO BOIL,AND FRACTURE UNEXPECTEDLY.THIS MATERIAL IS NOT
SUFFICIENTLY POROUS TO ALLOW HEATED STEAM TO READILY ESCAPE WHICH CAN BREAK
AND CAUSE PERSONAL INJURY OR DAMAGE.
-LEAVES,STICKS,WOOD,PAPER,CLOTHING,FOOD MATERIAL,SHOULD ALWAYS
BE KEPT AWAY FROM THE FIREPIT. MAKE SURE THAT THERE IS NO VEGETATION OR OTHER
OBJECTS OVER THE TOP OR SIDES OF THE FIRE PIT THAT COULD INTERFERE WITH SAFE&
PROPER OPERATION.

—IF LAVA ROCK IS WET,ALLOW FIRE PIT TO BURN FOR 45 MINUTES PRIOR TO
COMING 15 FEET WITHIN FIRE PIT.
Fire Pit Start Up:
Initial Start-up: Several"on/off'cycles may be necessary to purge air in gas lines after system
installation.Fire pit will lockout after 15 attempts to light-power down,then restart.
Sequence of Operation:
1.The hot wire igniter will be powered(glow red)for 5 seconds before pilot valve opens.
2.The hot wire igniter will only be powered the initial 15 seconds of the 60 second pilot cycle.This
sequence will repeat up to 15 times(-15 minutes)before going into lockout.To reset,turn"OFF"
power then back"ON"again.
3.Pilot flame will ignite and warm thermocouple-it may take 30 seconds at times for thermocouple to get
hot.If thermocouple is not hot in 60 seconds,system will shutdown then go back to Step 1.
4.Once thermocouple is hot,main valve will open allowing main burner to ignite.
5.If pilot flame is blown out at anytime,system will shutdown,then automatically restart(Step 1).
NOTE: IF POWER TO FIRE PIT IS TURNED"OFF"THEN IMMEDIATELY TURNED BACK
"ON"WHILE MAIN BURNER IS"ON",THE SYSTEM WILL GO INTO LOCKOUT MODE IF
THERMOCOUPLE DOES NOT COOL PRIOR TO 30 SECONDS AS A SAFETY FEATURE.
TO RESET,TURN"OFF"POWER THEN BACK"ON"AGAIN.
FIRE PIT START UP
1. STOP! Read the safety information on"What to Do If Smell Gas"(Pg.
2. Confirm there is no debris in fire pit(as mentioned in warnings)including water.
3.Turn"ON"electrical power and gas to fire pit.
r�4.Using wall switch or remote,turn"ON"fire pit-this may take several cycles to
purge any air.
5. To reset after lockout,power unit down then restart.
6.Once fire pit has ignited,DO NOT leave unattended.
OVERHEATING: The fire pit will automatically close gas valve if temperature exceeds 175°F inside valve
box to prevent component damage.When unit cools below 175°F,the unit will automatically restart.To
correct overheating,ensure enclosure has adequate ventilation-see"Construction of Enclosure".
FIRE PIT SHUTDOWN
1.Turn"OFF"fire pit using remote control or wall switch.
IMPORTANT: REMOTE CONTROL USE: YOU MUST ALSO TURN OFF POWER TO
ELECTRICAL OUTLET OR GAS TO FIRE PIT TO PREVENT ACCIDENTAL START.
2.Once fire pit is cooled,use appropriate cover to protect fire pit.
Fire Pit Maintenance:
1) Keep fire pit covered at all times when not in use.
2) Keep any debris out of fire pit-clean as needed.
3) Ring Cleaning:If flames exhibit any abnormal shapes or behavior,or if burner fails to ignite properly,
then the burner holes may require cleaning.The appliance can be cleaned by carefully removing the
logs and media to allow access to burner.Use a brush to carefully remove dust,spider webs,and loose
particles from base,logs,and fire ring itself.
4) Thermocouple Cleaning of Soot:Remove lava rock&glass around pilot,then blow out box.Clean
thermocouple of any soot using soft brush.Be careful not to damage hot wire element.Place lava rock
or glass back as explained on Pg.5.
5) Visually inspect the pilot-The pilot flame should cover 3/8"to 1/2"of the thermocouple as shown
below.Cleaning of orifice may be required by removing pilot
head. —

Lava Rock & Glass Application
Please follow the instructions below to add the finishing touch to your fire pit.
Remember the deeper your lava rock or glass the more risk of reducing if not smothering
the flame. Particular attention needs to be on the pilot assembly area- if thermocouple is
covered,this will cause the pit to shutdown due to smothering pilot flame.

HWI Fire Pit Trouble Shooting
Below are some potential causes and countermeasures to the symptoms indicated in
bold.If still unable to resolve issue, please contact your retailer or certified technician.
Hot Wire Will Not Glow
1.No power to unit. Confirm breaker,wall switch, and remote are on(120Vac)
2. Blown fuse in module. Change 5A fuse located on module in valve box.

manufacturing defects that prevent safe and correct function as follows:
*Electronics, Gas Valve,& Pilot Assembly: Commercial-limos; Residential- 1 yr.
*Stainless Steel Pan,Fire Ring, & Valve Box: Commercial-lyr.; Residential 3yrs.
This commences from the date of original sale/shipment from HPC FOB Dayton, Ohio.
This warranty is for parts and in-house (HPC) labor. The defective product must be
sent back to HPC with a Return Merchandise Authorization(RMA) issued by HPC for
that specific product and any other additional information for the nature of the defect
or warranty claim.
The warranty does not cover items that have been damaged by overheating,
modification,abuse,or improper storage. Also any labor involving installation or
maintenance with the unit is not covered.
This warranty excludes claims for consequential, indirect-collateral expenses
arising from product defects or warranty recovery.

The following are our responses to the structural comments:
1. Item 7: attached are sheets S1 —S4, the structural construction documents for the
pool and spa area renovation, including the gunite pool construction and the
support of the hot tubs.
2. Item 8: attached is an analysis of the loading of the soils in the spa area resulting
from the new pool and spa features. The soils engineer's report was completed
during the excavation of the pool area. The new loading is similar or equal to the
loads that existed after the original pool construction was completed.
3. Item 9: attached is sheet S1 showing the location of the new spas and section
4/S2 specifies the required raft slab support and the concrete walls surrounding
the spas.

Subject: Observation of Partial Excavation, Proposed Swimming Pool and Hot
Tubs, Vail Plaza Hotel, 16 Vail Road, Vail, Colorado
Gentlemen:
As requested, a representative of Hepworth-Pawlak Geotechnical, Inc. observed the
excavation at the subject site on May 14, 2010. The findings of our observations and
recommendations for the swimming pool and hot tub support are presented in this report.
The services were performed in accordance with our agreement for professional
engineering services to Vail Plaza Hotel, dated May 5, 2010.
The original pool and plaza area in the southeast corner of the property was being
demolished at the time of our site visit. The excavation was being taken down to an
existing structural mat about 8 feet below plaza level. The foundation walls of the
building were exposed on the north and west sides and will partly underlie the hot tub
area. It is proposed to backfill the excavation to design bearing elevations of the pool and
hot tubs with crushed rock. Additionally, a structural slab is proposed under the two hot
tubs in the southwest part to control differential settlement since they will span the
existing foundation and new backfill.
At the time of our visit to the site, the excavation was in progress and had been partly cut
down to the existing structural mat. More demolition of the original facilities was
needed. The material being removed consisted of crushed rock that we understand will
be replaced to support the new facilities. The existing structural mat overlies more
foundation wall backfill but the east side could be on natural ground. We are not aware
of the existing backfill or natural ground conditions and the potential for future
settlement. To our knowledge, the original facilities were structurally sound and did not
show signs of settlement and distress. Considering the conditions exposed in the
excavation and the nature of the proposed construction, it appears feasible to place the
proposed swimming pool and hot tubs on structural fill consisting of crushed rock. The
rock should be placed in one foot thick lifts and compacted with four passes of a walk-
behind double drum roller vibratory plate compactor or equivalent to compact fully the
rock. We should observe the placement and compaction on a regular basis. We concur
that a structural slab should be used to control differential settlements.

The recommendations submitted in this letter are based on our observation of the exposed
excavation conditions and do not include subsurface exploration to evaluate the
subsurface conditions within the loaded influence depth and that the soils beneath the
structural mat will provide adequate support of the new facilities. The risk of movement
may be greater than indicated in this report because of possible variations in the
subsurface conditions. We should be informed of any variations in the subsurface
conditions which could change the recommendations contained in this letter.

This manual contains important safety and maintenance instructions. Please read it
carefully before installing and using your patient lifter.
A DANGER!
The National Electrical Code requires that all metallic objects in the
Proximity of a swimming pool must be connected by no less than a#8
AWG solid copper wire to a common bonding grid such as the structural
reinforcing steel or the walls of a welded metal pool structure. Attach wire
to ground lug on cement-in sleeve.
A WARNING!
This lifter is not interchangeable with any other Hoyer Litter models. Use
only genuine Hoyer parts and Hoyer slings on all Hoyer lifters. Hoyer lifter
parts are not interchangeable with other manufacturers'parts. Replace any
worn parts immediately.
A WARNING!
Do not exceed maximum weight capacity of 400 lbs.
Installation of Cement-in Sleeve
Notice- All sleeves are now being supplied for flush installation with flush cover
Position the sleeve location per the layout in
Figure 1. Proper position for the mast is 8 to
10 inches from the edge of the pool or tub.The
cement-in sleeve itself is a cylinder 3'/4 inches in
diameter and 8 inches long. Mark a 6 inch circle
if existing concrete is to be removed. In a new
installation,position the sleeve on center and
flush with grade.
When the cement-in sleeve is to be installed in
an existing concrete floor or deck,a hole at
least 6 inches in diameter must be cut(Figure
2). The opening must be at least 8 inches deep.
Fill the bottom of the hole with non-shrinking
cement, such as Por-Rok. Make sure the sleeve
is perfectly vertical. Fill in around the sleeve
with more non-shrinking cement. Allow the
cement to set-up and finish the top surface.

aintenance and Use
The diagram (Figure 3)shows a cross section of
the finished installation for the cement-in sleeve.
Notice that the sleeve is completely imbedded in
cement and that a good bond is secured between
the new and old concrete. When installing the
sleeve in a new job,position the sleeve and line it
up when the forms are being installed. Pour the
concrete around the sleeve as the deck or floor is
poured. No special type of concrete is required.
NOTE A wad of newspaper stuffed into the top of the
sleeve will keep the interior free of concrete while the floor is
being poured.
Installation of Lifter
When the sleeve has been installed as described
above and the concrete has had several days to
cure,the lifter may be installed. The nylon bearing
socket supplied with the lifter must be installed in
the sleeve(Figure 4). Grease the outside of the
socket lightly before it is inserted into the sleeve.
This will prevent corrosion which may cause the
socket to stick in the sleeve.Vaseline is an excel-
lent lubricant if no commercial grease is on hand.
NOTE. The socket should be removed occasionally to prevent
corrosion.
With the socket in place,unpack the lifter and
insert the base of the mast into the socket and
sleeve(Figure 4). To attach the end of the boom
in the yoke hinge at the top of the mast,remove
the wing nut and bolt from the yoke hinge and
insert the boom end. Reinsert the bolt and
secure with the wing nut. Attach the swivel bar
to the boom end and to keep the chains apart.
The lifter is now ready for use.

the swivel bar hook every other month.
While oiling the lifter,check all the cap nuts and bolts to be sure they are tight.
Put a drop of oil on the pump handle hinge when the Hoyer lifter is put into service
and oil again every other month. This is very important,as the holes in the hinge
socket will wear when dry.
NOTE. In high humidity or salt areas, wax exposed lifter parts periodically to help prevent corrosion.

Operating Instructions
To use the Hoyer Swimming Pool Lifter, remove the cover from the floor sleeve and
insert the nylon bearing socket. Insert the mast of the lifter into the socket. Attach
the long chains to the swivel bar. To use the Hoyer Sling#112-D,place the sling under
the person being lifted so that the end of the seat comes to the person's knees. Attach
the ends of the chains to the swivel bar and the S-hooks to the sling. When the
person is lifted so that he/she just clears the chair,the lifter boom should be almost at
maximum height. The person being lifted can then be lowered the full 62". Follow
lifting instructions in "How to Use a Patient Lifter"manual.
Warranty
Your Hoyer lifter is warranted to be free of defects in materials and workmanship for
one (1)year after purchase by the original consumer/owner.Components included in
this warranty are as follows:
Lifter structure:base,mast,boom,and swivel bar;and Hydraulic pump
All other non-durable components,such as rubber parts,plastic parts and painted
surfaces are warranted for 3 months from first consumer purchase. These are subject
to normal wear and replacement.
Limitations
1. We do not warrant damage due to:
a. Neglect,misuse or improper installation or repair.
b. Use of parts or changes not authorized by Sunrise Medical
c. Exceeding the specified weight capacity of the lifter.
2. This warranty is void if the original lifter serial number tag/label is removed or
altered.
Our sole liability is to repair or replace covered parts. This is your only remedy for
consequential damages.
What you must do
1. Obtain from us,while this warranty is in effect,prior approval for return or repair
of covered parts.
2. Contact Customer Service at 1-800-333-4000 for shipping information.
3. Pay the cost of labor to install or repair parts.
