RE: Revisions to Tenant Improvement Plans for L2 Lobby Restaurant; Project Address:
Retail Suite 207, 141 E. Meadow Drive, Vail, CO 81657; Building Permit B10-0095
Dear Mr. Haeberle,
We are pleased to transmit revised Electrical Plans L2 Lobby—Matsuhisa Restaurant
Tenant Improvement Plans for review and approval. Sheets E4.1 and E7.1 have been
revised to resolve Electrical coordination issues for the Dishwasher Equipment.
Included with this letter are:
1. One Transmittal Form.
2. Three full size stamped and signed drawings E4.1 and E7.1, with an issue date of
31 May 1 l.
The revisions are summarized as follows:
a. The main electrical service from the West Retail Electrical Room 246 has been
revised to a 600 amp service. We are using a spare 600 amp service that was
constructed with Solaris Core and Shell.
b. One electrical panel has been added in the wall on Grid H to near Grid 12.3 on
Sheet E4.1.
c. The one-line diagram has been updated and a new panel schedule has been added
to Sheet E7.1.

Dear Mr. Haeberle,
We are pleased to transmit revised L2 Lobby—Matsuhisa Restaurant Tenant
Improvement Plans for review and approval. The Kitchen and Private Dining Room for
Matsuhisa will be located in Retail Suite 207,just west of the Lobby on Leve12. The TI
Plans have been revised to resolve MEP coordination issues.
Included with this letter are:
✓1. One Transmittal Form.
✓ 2. Three full size stamped and signed Plans, with a cover date of 25 March 2011.
The revisions are summarized as follows:
a. The location of the water heater has been switched with the Pizza Oven due to
coordination issues near Grids H.5 and 12.
b. The electrical panels have been relocated from a wall on Grid 11 to near Grid 12.5
on Sheet E4.1, due to Item a. above.
c. One hand sink has been added near Grids H.3 and 12.2 on Sheet P2.2.
d. A Sequence of Operation has been added on Sheet M2.2 to describe FCU-3 and 4
maintenance activities.

Dear Mr. Ogg:
This letter will serve to confirm the plan review of the above captioned food service
establishment. The plans have been approved with the following additions and/or modifications:
1. The hot water generation unit must provide a minimum energy input of 366,066 BTUs
per hour. It is recommended that the hot water generation unit be dedicated to the
kitchen fixtures. Provide engineering design data for the building use, if the kitchen hot
water is to be part of the building system.
2. Separate toilet facilities shall be required for each sex with seating capacity in excess of
15 patrons or more than 15 employees. Employees and patrons may use the same toilet
facility provided that patrons have access to them without entering the food preparation,
food storage, dishwashing area or utensil storage areas of the restaurant.
3. If grease trap is required it must be installed either outdoors or with the top flush to or
below the finished floor. It must also be located outside of any food preparation area and
adequate access must be provided to allow for proper cleaning.
4. The wells of the three compartment ware washing sink must be large enough to
accommodate the largest utensil used in the facility. If the three compartment sink is
used for food preparation,the prep sink shall discharge atmospherically by means of an
air gap over a waste sink.
5. Food shall not be prepared in a pot and pan sink. If a sink is necessary for defrosting,
evisceration, or preparation of food, a separate one (1)compartment, stainless steel sink
equipped with a drain board is required.
6. Condensate drain lines from all refrigeration units and ice machine(if installed)must
have an air gap of at least two(2)pipe diameters at the drain.
7. Provide vacuum breakers on all hose bibs.
8. Hand washing sinks shall be conveniently located to employees involved in food and
utensil handling. Sinks used for food preparation or for washing equipment shall not be
used for hand washing.
9. A hand washing and glove use restaurant policy must be developed and followed by all
food handling employees. All ready to eat foods(e.g., sushi products)must be not be
assembled with bare hands.
10. Provide a minimum of fiberglass-reinforced plastic(FRP), or stainless steel on the wall
surface behind the three-compartment pot cleaning sink and dishwashing areas.
11. The minimum acceptable floor finish in the walk in cooler is quarry tile installed with a
grout having an absorbency rate of 4%or less, 20 gauge stainless steel or 18 gauge
aluminum floor.
12. The minimum acceptable ceiling finish in food preparation and storage area is gloss
enamel or epoxy-based paint on dry wall; or non perforated vinyl-coated dry wall, lay-in
ceiling tiles.
13. The junctures between walls and floors shall be coved and sealed.
14. Under counter surfaces must be finished and sealed to provide a washable surface.
15. All light fixtures must be adequately enclosed or shielded to protect foods in the storage,
serving, or preparation areas from accidental bulb or tube breakage.
16. All ice bins and exposed food placed out for serving shall be adequately covered or
protected by approved sneeze guards.
17. All grease-producing equipment shall be vented through a properly designed ventilation
hood and grease collection system that meets the Town of Vail Building Department
requirements. Filters or other grease extracting equipment shall be easily removable for
cleaning and replacement when not designed for in place cleaning. Verify with your
hood supplier and installer that the design, construction and installation will meet these
criteria.
18. Fire prevention, extinguishing equipment and lighting system shall be installed in a
kitchen ventilation system exhaust hood so as to not create a cleaning problem.
19. Provide stainless steel on the wall surface behind the cooking equipment from the bottom
of the canopy down to the base molding.
20. Properly seal sinks and drain boards to their adjacent wall. Exposed screw heads and
bolts are not approved.
21. All plumbing, electrical and other utility lines must be concealed in the walls, floor or
ceiling. Water service lines extended to sinks may be surface mounted provided they are
properly mounted on approved spacers at least '/z inch from the walls and 6 inches above
the floor.
22. A minimum of two(2) separate shelves will be required for storage of cleaning and toxic
materials. They must be separated and labeled by the following requirements:
a. Detergents and sanitizers and other toxic chemicals(upper shel fl.
b. Insecticides and rodenticides(lower shel fl.
23. Raw molluscan shellfish shall be identified by an attached tag,to be retained for a period
of 90 days,that states the name and address of the original shellfish processor,the kind
and quantity of shellfish, and the certification number issued by the State or foreign
shellfish control agency, where applicable.
24. Before service or sale in ready-to-eat form, raw, raw-marinated, lightly cooked-
marinated, marinated, or partially cooked fish other than molluscan shellfish shall be
frozen throughout to a temperature of:
a. -4 degrees F (-20 degrees C)or below for 168 hours(7 days) in a freezer; or;
b. -31 degrees F (-35 degrees C)or below for 15 hours in a blast freezer.
25. A bayonette-type, metal stemmed food thermometer accurate to plus or minus two(2)
degrees F must be supplied(scale 0 degrees F to 220 degrees F).
26. Sushi rice preparation pH must be confirmed by laboratory testing of a representative
sample of freshly prepared sushi rice by a certified laboratory. The State of Colorado
laboratory will perform a pH test for sushi rice. A State of Colorado Laboratory testing
request form with contact information is enclosed for your convenience.
27. All equipment must be National Sanitation Foundation(NSF) listed or equivalent.
Any changes to the plans as submitted and hereby approved must receive advance approval from
this Department. A final inspection is required to determine is the kitchen is in compliance with
the Colorado Retail Food Rules and Regulations. Please allow a minimum of three (3)days for
scheduling this opening inspection.

Dear Mr. Naeberle,
We are pleased to transmit with this letter the revisions to the L2 Lobby—Matsuhisa
Restaurant Tenant Improvement Plans for review and approval. The Kitchen and Private
Dining Room for Matsuhisa will be located in Retail Suite 207,just west of the Lobby on
Leve12. The TI Plans have been revised to meet the needs of the new restaurant
operator.
Included with this letter are:
1. One Transmittal Form.
2. Three full size stamped and signed Plans, with a cover date of 14 February 2011.
The revisions are summarized as follows:
a. The Private Dining Room 207.1 has been changed to a Sushi Bar.
b. The ceiling plan has been simplified in Sushi Bar Room 207.1.
c. The kitchen layout has been revised to the new operators design, with the two
hoods and cooking island being rotated 90 degrees for better flow between the
kitchen areas.
d. A third hood has been added for a small cooking station near Grids 12 and H.4.
e. The Double Door 207B has been revised to a single door.
f. The Single Door 207E at Restroom 207.2 has been moved to the Comdor.
g. The corridor corner at Grids 9.3 and G.1 has been squared out to allow for a
revised Walk-In Refrigerator configuration.

INTRODUCI'ION '
This report is a presentation of results of a surfa�e flammability test on a material subrnitted by Elitis, Inc.,
Miarni�,Flocida.
The test was coruducted in accordance wiEh the ASTM Ineernational fire be�t response standard E 84-09a,
Surfaa 8urning CharacterisEics of Building Materiais, sometimes referred to as the Steiner tunnel test. Th�
test is appGcable to exposed surfaces such as walls and teilings.The test is conducted with the specimen
in the ceiling position with the surface to be evaluated exposed face down to the ignition source. The
ASTM E 84 test rnethod is the technical equivalent of NFPA No.255 and UL�To. 723.
This standard is u$ed to measure and desQibe the response of materials,products, or assemblies to heat
and .flame under controlled conditions, but does not by iiself incorporate all factors required for
fire-hazard or fire--risk assessment of materiaLs, products,or assemblies under actual fire conditions.
PURPOSE
The purpose of the test is to provide vnly the comparative measurements of surface fIame spread and
srnoke development of rnaterials with d,at af select grade red oak and fiber-reinEorced cement board,
Grade II,under specific fire exposure conditions.The test exposea a nominal 24foot long by 20-in�h wide
best apecimen to a cantrolled air flow and flaming fire adjusted to spread the flame along the entire length
of a red,oak spe�imen in 5.50 minutes. During the 10-rninute test duration, flamespread over ;the
specimen surface and density of the resuldng smoke are rneasured and rernrded. Test results'are
calculated relative to red oak, which has an arbitrary rating Qf 100, and fiber-reinforced cement board,
Grade A,which has a raHng of 0.
'i'Iie test resultg are ezpressed as Flame Spread Index and Smoke Develc>ped Index, The Fla�me Spread
lndex is defined in ASTM E 176 as "a nvmber or classification indicating a compararive tneasvre derived
from observatians made during the progress of the boundary of a zone of flame under defined test
conditions." The Smoke Developed Index, a term spe�ific to ASTM E 84, is defined as"'a r�umber or
classification indicating a comparative measure derived from smoke obscuration data collected �during
the test for surEace burning characteristics." There is not necessarily a relationship between the two
measusements.
The method does not provide for measurement of heat transmission through the surface teated,the effect
of aggravated flame spread behavior of an assembly msulting firom the proximity of combuatible walls
and ceilings,or classifying a material as nonmmbustible soIely by means of a Flame Spread Index.
The zero reference and other parameters critical to fumace operation aze verified on the day of the test by
conducting a 10-minube test using I J4-inch fiber-reinfarced cement board, Grade II. Periodic tests using
NOFMA certified 23132-inch select grade red oalc Elooring provide data for the 1Q0 reference.

The test sample, selected by the client, �vae identified as Elitis/Natives xVP 625-629, a vinyl wallpaper
with a total weight of ]2.3 ounces pe1'square yard. 7�ree test panels, each measuring two feet wide by
eight feet in length, were prepared by adhering the material to l/4-inch thick fiber-cement board
complying with ASTM Specification C 2186 (Grade Q) and passing ASTM Test Method E 136, using
Gardne�Gibson Dynamifie 111 Heavy Duty Wallcovering Adhesive. The adhesive was ap lied with a
roller to the back of the wallpaper,bookrd 5 to 7 minutes,the mafierial placed onta the smo�side of the
cement board, and smookhed with a brush and roller. 7'his method of sample pzeparation is desccibed in
AST'M E 29U4-08, Standard Fractice fur Speermen Prepmation and Mountirrg of Textile, Paper or Viny1 Wa11 or
Ceiling Coverengs to Assess Surfitct Burning Characteristics, Section 8.2, Wall or Ceiling Coverings Intended
to be Applied Directly to a Noncombusiible Wall or Cei]ing Surface. After dead-stacking overnight, the
prepared panels were transferred to storage racks and conditioned to equi[ibrium in an a�nosphere with
the t�rnperature maintained at 71 f 2°F and the relative humidity at 50 t 5 percent.For testing,the panels
were placed end-to-end on the ledges of the tunneI fumace and the test conducbed wzth no auxiliary
support merhanism.

The test results, calculated on the basis of observed flarne propagation and due integrated area under the
recotded smoke density curve, are presented below.The Plame 5preaci Index obtained in E 84 is rounded
to the nearest number divisible by five. Smoke Developed Indices aze rounded to the nearest nurnber
divisible by five unless the Index is greater than 200.In that case, the Smoke Developed Index is rounded
to the nearest 50 points. The flame spread and smoke development data a�presented graphically at the
end of this report,

Specimen ignition over the burners occurzed at 0.28 minute. Surface flame apread was observed to a
maxirnum distance of 4.54 feet beyond the zero point at 2.15 minutes. The maxirnum temperature
reoorded during the test was 563°F.
