This letter summarizes the materials testing GROUND Engineering Consultants, Inc.
(GROUND) performed during the construction of the Fire and Emergency Services
Station #3 building, located at 2399 North Frontage Road in Vail, Colorado. The special
inspections and materials testing services were performed at times scheduled by the
Owner, Contractor and/or Subcontractors between June 6 and December 1, 2010.
Representatives of our office were present on a part time basis to periodically observe
the placement of backfill for the footings and for the floor slab. Nuclear-moisture density
testing was periodically performed on the processed soils.
Koechlein Consulting Engineers, Inc (KCE) were onsite to observe the foundation
subgrade soils between 7/13/2010 and 7/20/2010 and provided additional
recommendations regarding the foundation subgrade during these site visits. GROUND
was onsite as requested by the Owner and/or Contractor during this time period to
observe the foundation soils. Please see GROUND's observation reports and KCE's
onsite recommendations and their original soils report #09-033 dated 8/6/2009
regarding preparation of the foundation subgrade soils.
During construction, representatives of our office periodically observed reinforcing steel
placement and performed periodic testing on the structural concrete placed for the
foundation, foundation walls, slabs on deck and other structural elements. Samples of
the structural concrete were periodically tested to determine physical properties and
compressive strength.
Representatives of our office periodically observed reinforcing steel placement and
grouting operations, during masonry construction, when scheduled by the Owner,
Contractor or Subcontractor. At the time of our visits, representatives of our office
observed the placement of reinforcing steel and the general construction of the masonry

. walls. Mortar, grout, and masonry prism specimens were cast periodically during
construction and tested for compressive strength.
Structural steel welded and bolted connections were observed during construction by a
certified welding inspector (CWI). These observations were completed at times
scheduled by the Owner, Contractor or Subcontractor.
Based on the final test results to date, field observations, RFI's received and other
project documentation received from the design team, it is our opinion that the materials
tested and/or observed for the structural concrete, reinforcing steel, structural masonry,
and structural steel were in general accordance with project plans, specifications, and/or
applicable RFI's.
The observations and materials testing as indicated above were performed periodically
to provide data to the Owner, Contractor and Subcontractors to assist them in
evaluating the general compliance of the tested materials and/or work with the contract
documents at the times of our observation and testing. We did not supervise, direct or
have any control over any of the Contractor's or Subcontractor's work. We were not
responsible for the Contractor's or Subcontractor's scheduling of our services, means,
methods, procedures, techniques, or sequences of construction, nor responsible for
ensuring that any Contractor or Subcontractor performed or completed the work in
accordance with the contract documents or construction industry standards.

Re: Criteria for a Temporary Certificate of Occupancy (TCO) for Fire Station 3
Dear Mike,
In order to identify all items needed for the TCO, per our meeting on Wednesday, March 9,
2011, please see the following:
1. A Framing ILC for roof and eave elevations is still needed and shall be submitted prior to
requesting a TCO. Note: review and acceptance by the Town of Vail required prior to a final
inspection approval.
2. A TCO phasing plan shall be submitted by March 16, 2011 showing staging for remaining
work after TCO issuance to include dates of completion.
3. A final report of special inspections is required to be submitted and approved prior to
requesting final inspections
4. Drahota shall submit a copy of the performance bond contract they have with the Town of Vail
in place of the standard DIA contract and funding.
5. Drahota shall call for a planning final inspection on March 21 for the exterior fa�ade.
6. Drahota shall submit a letter to the Chief Building Official that will address what will not be
completed for the TCO and when it will be completed. This will be included as an attachment to
the TCO.
7. As-built's by the engineer of record for the building sewer, water, and gas to be submitted
prior to final inspections.
8. Air balancing reports shall be submitted prior to calling for a final inspection.

Additional items discussed were:
1. Upon completion of partial final inspections, specifically electrical and fire protection systems,
it will be ok to move equipment and furnishings into these areas prior to the TCO.
� 2. Energy envelope data shall be posted at the electrical panel.
� 3. It has been confirmed that the amended final plat was recorded and in our plat files.
� 4. Drainage concerns at the street to be resolved between Drahota and Town of Vail Public
Works.
5. Staging of construction for the retaining wall needs to be coordinated with Public Works.
6. Planned date of occupancy is April I 1, 2011 per Chief Miller.

Attached are revised, stamped drawings for your review. These sheets document
additional changes made to the scope-of-work since issuance of the approved
Permit Set and our last update of August 27, 2010.
1. Sheet A2.0, Flag Note#7:
Door 118 to Mechanical was relocated into the Exercise Room 119
The door leading from the Copy Room 106 to Office 103 was removed
2. Sheet A2.1, Flag Note#7:
Final coordination of refrigerator sizes required relocation of the north wall
of Kitchen 259 12" to the north.
3. Sheet A-2.7, Flag Notes #6 and#7:
At east entry canopy, roof drainage conducted to a downspout on the south
column and connected the to the underground storm drainage system.
Provide notch in bent plate edge for overflow.
Provide a conductor head and downspout from the gutter above the main
entry canopy to the main entry canopy roof.
Revise drainage of roof above east stair to be directed to the gutter system

At the north entry canopy, roof drainage conducted to a downspout on the
east column and discharged at grade. Provide notch in bent plate edge for

At the gable roof on the east side of the building, revise roof pitch from

PEC.

Access ladder provided at the elevator pit.
5. Sheet A4.8, Flag Note#7:
At Training Tower, provide additional guard rail infill at the landings to
provide protection between the landings and the adjacent windows.

A 4x continuous nailer was added to the edge condition of both the east
entry and north entry canopies to allow for flashing anchorage, stiffer edge
condition and better roof inembrane edge attachment.

testi�g was periodieafly perforrned an the processed soils.
Kaechlein Cansufting Engineers, (nc (KC�) w�re on�ite �o observe the �oundaiion
subc�rade sails between 7/'i 3I201 p ar�d 7/20/2a1� and prnvided additicna!
recommendafians regarding the f�undation subgrade during tt�ese site visits. GROUfvD
