is required to request tree removal /replacement in the Town of Vail. This form must be signed by a Town of Vail au-
thorized representative who has inspected the tree(s). To request an inspection, please call Tom Talbot, Wildiand Coor-
dinator, at (970) 477 -3509.
Applicant has 30 days from the date of this application to submit a mitigation plan to the Town of Vail Wildiand Coordi-
nator.
