Asbestos testing and abatement protects workers homeowners neighbors and emergency services responders from ex
posure to harmful asbestos The Town of Vail asbestos abatement program is in addition to the State of Colorados regu
lations It is your responsibility to be in compliance with the State Please contact the State directly for their requirements
at the contact info listed below
When is asbestos testing required
ANY building projects disturbing more than these threshold levels of building materials require asbestos testing
One and Two Family Dwellings 32 square feet
All Others commercial spaces hotel rooms etc 160 square feet
Definition of a single family dwelling any dwelling unit that is used primarily for a single family including
multi family condominium units and fractional fee units
Asbestos testing results must be provided with your application for a building permit
Tests which identify POSITIVE results at more than 1 require abatement by a State certified and Vail registered abate
ment contractor An asbestos abatement permit must be approved and the clearance letter must be submitted to the
Town of Vail before the building permit will be issued

Even recent construction projects may include asbestos containing materials so buildings of any age require testing
The 1989 Ban on asbestos containing materials is commonly misunderstood In fact in 1991 the US Fifth Circuit
Court of Appeals vacated much of the so called Asbestos Ban and Phaseout rule and remanded it to the EPA Thus
much of the original 1989 EPA ban on the US manufacturing importation processing or distribution in commerce
of many asbestos containing product categories was set aside and did not take effect CDPHE
Asbestos test results and abatement permit applications should be submitted to Town of Vail Community Development

This permit is granted subject to Colorado Air Quality Control Commission Regulation No 8 Part B adopted December 21 2007
and effective January 30 2008 the Colorado Air Pollution Prevention and Control Act 25 7 101 or 25 7 501 et seq CRS and
the following provisions It is only for the purpose of allowing asbestos abatement

By performing work under this permit the abatement contractor agrees that the Division may revoke or suspend this permit
should the Division find that the contractor
has violated or has aided and abetted in the violation of 25 7 101 or 25 7 501 et seq CRS or Regulation No 8 Part B or
an order of the Division or Commission
has failed to meet any permit and notification requirement or failed to correct any violations cited by the Division during any
inspection within a reasonable period of tine as may be determined by the Division
has used misrepresentation or fraud in obtaining this permit or
has committed any act or omission which does not meet generally accepted standards of the practice of asbestos abatement
As a contractor you may be subject to other licenses and permits depending on the requirements of the county and municipality
in which the work is being performed The Colorado Department of Public Health and Environment Air Pollution Control

Summary of Work
This project Statement of Work SOW addresses the specific work as listed herein to he
performed by the abatement contractor All Nvork shall he in strict compliance with this
project SOW the State of Colorado CDPHE Regulation 8 the United States
Environmental Protection Agency US EPA Regulations 40 CFR 61 NESHAP
Asbestos applicable Occupational Safety and Health Administration OSHA
regulations and Federal and State of Colorado Department of Transportation CDOT
regulations The SOW for this project includes comprehensive asbestos removal
decontamination and disposal operations at the location listed below
115 Forest Rd
Vail CO
The scope of work is as follows
Removal and disposal of approximately 6000 square feel of drywall to studs
and joint compound on walls located throughout the building that is scheduled for
demolition
Approximately 680 square feet of floor tile is being abated
All ACM will be placed in pre labeled asbestos bags for disposal
A negative pressure differential of at least 002 inches of water shall be
maintained during the entire duration of ACM removal activities
The Owner has applied for a project manager waiver from CDPHE
All abatement is to be accomplished within negative air containment per Colorado Reg
No 8 All work shall be completed in accordance with the schedule requirements of this
project SOW Work shall commence as soon as possible and all abatement should be
completed in a timely manner
Included in the abatement contractors containment preparation requirements will be a
personal decontamination unit constructed to provide shower facilities with hot and cold
water taps and separate air locks from the clean room shower and equipment room The
abatement contractor shall also provide waste load outs for the rnost convenient and safe
transport to the staging area or for direct load out into waste trailers ACBM waste shall
be placed into an approved waste container immediately after leaving the containment If
waste trailers are used all ACBM waste rnost go to an EPA approved asbestos landfill

The abatement contractor shall affix applicable OSHA warning signs along the perimeter
of the containment areas and waste containers to regulate the work area The building
owner shall provide service for utilities such as water and electricity The abatement
contractor shall implement Ground Fault Circuit Interrupt GFCl as well as providing a
sufficient number of subpanels to maintain all interior work areas under air
filtrationpressure differential and provide service to HEPA vacuums This should be
done at all times until the abatement is complete and has been cleared with final visual
and clearance samples
The abatement contractor shall place the work area under a negative air filtrationpressure
differential before pre cleaning may start The required pressure differential shall be at
least 002 inches of water for full containments and shall have at least four 4 air
exchanges per hour The single family residence located at 115 Forest Rd Vail CO has
approximately 25000 cubic feet of air space total in one containment which requires a
total of four 4 negative air machines NEG assuming four air exchanges per hour and
also assuming one 1 additional NEGs for backup safety factor Air filtrationpressure
differential units shall be exhausted to the exterior of the residence through existing
windows where possible
The single family residence located at 115 Forest Rd Vail CO shall have ACBM
removed as follows the containment shall be constructed per Colorado Reg No 8
requirements The abatement contractor shall install critical barriers consisting of two 2
layers of six millimeter 6mm polyethylene sheeting on all windows doors vents
plenums soffit vents gable vents etc and secure all polyethylene with two layers of
tape spray glue and staples The abatement contractor shall then install and initiate
negative airflow followed by construction of the decontamination chambers and load out
chambers
Worker protection shall include disposable Tyveke or equivalent coveralls and the
wearing of the proper level of respiratory protection equipped with HEPA filters in
accordance with the abatement contractors written Respiratory Protection Program The
abatement contractor shall provide Tyveke or equivalent coveralls to DSCs Industrial
Hygienist
The abatement contractor shall thoroughly wet all ACBM to be removed with amended
water consisting of a fifty percent 50 polyoxyethylene ester and fifty percent 50
polyoxyethylene ether The ACM shall be placed in approved six millimeter 6mm
polyethylene waste bags Decontaminate the exterior of each waste bag by wet wiping or
HEPA vacuuming All waste wraps and bags shall be sealed with duct tape the neck of
the bag or ends of wraps shall be bent over itself goose neck and an additional three 3
wraps of duct tape applied A proper waste manifest shall be required of all offsite
asbestos shipments per EPA NESHAPS Standard 40 CFR Part 61 Subpart M A copy
of the waste manifest shall be made available to the Client upon completion of the
project All waste generated from this project shall be labeled and disposed of in a
landfill approved for accepting asbestos waste

The work area and the decontamination area shall be thoroughly cleaned after all work is
finished The use of HEPA vacuums and cleaning cloth will be utilized on each surface
of the containment The cleaning cloth maybe used one time only and then disposed of as
contaminated waste Continue this cleaning until there is no visible dust or debris on the
surfaces in the work area Re clean all surfaces of the work area using HEPA vacuums
and damp rag cleaning methods Change all secondary filters on the air filtrationpressure
differential units and properly dispose of them as ACM
Upon completion of final cleaning DSCs AMS shall be called in to conduct a final
visual clearance of all surfaces of the work area including walls floors ceilings
equipment containment barriers decontamination units load out units and other items in
the work area Once the final visual is satisfactory a set of five 5 air samples shall be
collected using NIOSH 582 clearance methods All samples will be analyzed utilizing
Phase Contrast Microscopy PCM The clearance criteria using PCM will require all
five samples to be below 001 fibers cubic centimeter
When the area has cleared the contractor shall remove all critical barriers and all
asbestos warning signs clean the area of any remaining taperesidue and make sure the
work area is ready to be turned over to the Owner

EXECUTIVE SUMMARY
On June 29 2010 Mr Steve Shurtliff of DS Consulting Inc DSC performed a full building inspection and
collected asbestos bulk samples at 115 Forest Rd Vail Colorado in order to identify potentially
hazardous friable and non friable asbestos containing materials ACM within the above referenced
single family residence scheduled for full demolition The Colorado Department of Public Health and
Environments CDPHE Regulation 8 Part B defines an asbestos containing material ACM as a material
containing more than 1 asbestos
Mr Shurtliff performed asbestos bulk sampling of multiple varieties of drywall surfacing materials in a
total of eleven 11 locations within the upper and lower level plaster surfacing material in a total of
three 3 locations on the exterior of the residence and resilient floor tile miscellaneous material in a
total of one 1 location within the lower level see Appendix A for sample locations
All fifteen 15 bulk samples were analyzed by Aerobiology Laboratory Associates Inc NVLAP
200860 0 in Denver Colorado with eight 8 laboratory results initially reporting positive for asbestos
between 15 and 4 Chrysotile see Appendix C for laboratory results The bulk sample with the
highest percentage of asbestos within each homogeneous group was then re analyzed using the point
count method to determine the exact percentage of asbestos within the sample in order to verify its
percentage of asbestos was greater than 1 All three 3 bulk samples re analyzed by the point count
method were reported as containing greater than 1 Chrysotile asbestos

PROJECT OVERVIEW
I Introduction
A full building inspection and bulk sampling for ACM was conducted at 115 Forest Rd Vail Colorado by
Mr Shurtliff on June 29 2010 at the request of Mr Kevin ODonnell with Beck Building Company Mr
Shurtliff is a Colorado State Certified Building Inspector having EPA Accreditation 15413 see Appendix
B for certificate The purpose of the inspection was to identify sample and assess potentially hazardous
friable and non friable ACM within the residence
II Structural Design
The structure is a wood framed two level single family residence
III Sampling and Analytical Procedures
The inspection assessment and sampling were conducted by an EPA and AHERA accredited Building
Inspector qualified by experience education and training in the recognition of potential ACM and
approved bulk sampling techniques The asbestos bulk sampling was conducted on suspect ACM with a
limited number of bulk samples being collected within the residence
The inspection assessment and sampling were performed in accordance with Environmental Protection
Agency AHERA recommended procedures These procedures call for the visual inspection of the area of
concern and the collection and analysis of representative bulk samples of suspect material
Some minor destructive sampling was conducted Walls columns and perimeter pipe chases were not
broken into in order to locate and quantify suspect ACM It should be noted that additional ACM might be
located in these and other inaccessible areas
Random bulk samples representative of the suspect asbestos containing building materials ACBM of
each homogeneous area HA were collected according to the guidelines published as Environmental
Protection Agency EPA Final Rule Title II of the Toxic Substances Control Act TSCA 15 USC Sections
2641 through 2654 and in compliance with 40 CFR Part 763 Representative sampling is based on the
following criteria
1 The distribution of the suspect material throughout the HA
2 The suspect materials physical characteristics and application
3 Random sampling patterns determined for each HA
Suspect materials sampled and analyzed should be considered to be representative of materials in each
HA if they exhibit similar physical characteristics and the application of the sampled material can be
correlated to the application of un sampled material Bulk samples collected were analyzed utilizing the
EPAs Method for the Determination of Asbestos in Bulk Building Materials EPA 600R116 July 1993
and the McCrone Research Institutes The Asbestos Particle Atlas as methods references Analysis of the
bulk samples was performed on the date reported as listed in the bulk sample analysis report

Condition assessments were performed by the accredited inspector at the time of inspection Condition
assessments are listed in the following section Ratings of good damaged and significantlydamagedaremeanttoindicatetheoverallconditionofthematerial
A material in good condition has no visible damage or deterioration or showing only very limiteddamageordeterioration
A material in damaged condition has the following characteristics The surface is crumbling blistered
water stained gouged marred or otherwise abraded over less than one tenth of the surface if the
damage is evenly distributed one quarter if the damage is localized Accumulation of powder dust or
debris similar in appearance to the suspect material on surfaces beneath the material can be used as
confirmatory evidence A material in significantly damaged condition has one or more of the followingcharacteristics
The surface is crumbling or blistered over at least one tenth of the surface if the damage is evenlydistributedonequarterifthedamageislocalized
One tenth one quarter if localized of the material is hanging from the surface deteriorated or
showing adhesive failure
Water stains gouges or mars are over at least one tenth of the surface if the damage is evenlydistributedonequarterifthedamageislocalizedAccumulationofpowderdustordebris
similar in appearance to the suspect material on surfaces beneath the material can be used as
confirmatory evidence
Response action recommendations for asbestos containing HAs are listed in the section VII
Recommendations may be for more than one HA if materials are alike Recommendations are either
general or immediate An immediate recommendation indicates the presence of asbestos greater
than 1 within the bulk sample or a bulk sample in the same HA and should be addressed accordingly
A general recommendation indicates asbestos does not exist greater than 1 within the bulk sample or
a bulk sample in the same HA and no further abatement activities are required for removal of the
material Any sample reporting a TRACE amount of asbestos must be considered to be positive for
asbestos greater than 1 unless it is analyzed by the point count method to be less than 1
V Inspector Comments
Asbestos was identified during the bulk sampling conducted at 115 Forest Rd Vail Colorado A total of
fifteen 15 bulk samples were collected from the interior and exterior of the above referenced
residence Eight 8 bulk samples tested positive greater than 1 for asbestos

The laboratory results of the potential ACM sampled at 115 Forest Rd Vail Colorado on June 29 2010
indicate that eight 8 bulk samples of drywall and resilient floor tile collected from the resident were
reported as positive for asbestos greater than 1 Chrysotile
If the above referenced ACM is to be removed from the residence and the total quantity exceeds any of
the regulatory trigger levels of 50 linear feet on pipes 32 square feet on other surfaces or the volume
equivalent of a 55 gallon drum a Colorado licensed asbestos abatement contractor is required to
perform the removal including formal notification to the Colorado Department of Public Health and
Environment prior to the abatement of the ACM
