(DSC) conducted a final visual inspection, performed aggressive clearance air monitoring, and analyzed clearance air - quality samples via Phase Contrast Microscopy (PCM) for an asbestos abatement project encompassing a majority of the main level and the entire lower level of the residential home located at 115 Forest Rd, Vail, Colorado on August 20, 2010. Services were provided at the request of Mr. Roy White with Rocky Mountain Environmental Services, LLC in support of asbestos abatement activities being conducted to remove approximately 6000ft of asbestos - containing drywall from both the main and lower level as well as to remove approximately 680ff of asbestos - containing resilient floor tile from the lower level. All asbestos abatement activities were conducted within a full containment. Monitoring locations and frequencies were determined by the DSC- authorized representative Mr. Steve Shurtliff, EPA accreditation No. 15413. PCM analysis was also performed by Mr. Shurtliff (see Appendix B for certification). DSC's scope of work for this project included performing a final visual inspection and collection and analysis of aggressive final air quality samples by NIOSH 582 - trained personnel for determination of airborne asbestos (as total fibers) concentrations in identified areas. DSC is a participant in the Industrial Hygiene Proficiency Analytical Testing (IHPAT) program with laboratory ID No. 188987 and has a rating of Proficient. The abatement project passed final visual inspection and the analytical results of all samples collected were below the clearance criteria of 0.01 fibers /cubic centimeter (f /cc) (seeAppendixA for Air Sample Analysis). AIR MONITORING Air - quality samples were collected utilizing a modified NIOSH 7400 Method, whereby samples were collected on 25- millimeter (mm) mixed - cellulose, ester - membrane filters with 0.45- micron pore size and analyzed via Phase Contrast Microscopy (PCM) as per the customer's requirements. Sample Collection - PCM samples, when applicable, were collected on 25- millimeter (mm) mixed - cellulose, ester - membrane filters with 0.45- micron pore size with an effective collection area of 385 mm. All filters were pre - assembled by the manufacturer in three stages, conductive sampling cassettes with extension cowls. Air samples were collected at flow rates of approximately 12.5 liters per minute (L /m) using Thomas high - volume pumps with a minimum of 1,200 liters being collected. Flow rates were collected at the beginning and at the end of the sampling period utilizing an airflow rotameter calibrated against a primary -flow calibration instrument. Start times and stop times were recorded for all sampling periods. A field blank and lab blank were also collected and analyzed along with the five (5) clearance samples. PCM Sample Analysis Method - PCM samples were analyzed according to a modified NIOSH 7400 Method —A Counting Rules— although samples were collected on 25 -mm mixed - cellulose, ester - membrane filters with 0.45- micron pore size and analyzed via PCM.
