Box 408 Eagle, Colorado 81631 Office (970) 328 -3355 Prepared by: GeoStat Environmental, LLC 12995 Sheridan Blvd, Suite 204 Broomfield, Colorado 80020 Office (303) 460 -1009 Fax (303) 460 -1069 GeoStat Project No. 0112 -10 September 14, 2010 $pri -037y 1) 10 -, ()COS fi5efo 001q L Introduction On September 2" 2010, Mr. Mike Lococo of GeoStat conducted a final visual inspection and final air clearance test at 816 Forrest Road, Vail, CO at the request of Mr. Alex Mintling of Plumb Kendall Construction (PKC). Hudspeth removed Asbestos Containing Material (ACM) from the spill on the first floor of the building. The final inspection and clearances of the work area were performed in accordance with the applicable regulations upon completion of the asbestos removal. The final inspection and clearances consisted of a final visual inspection of the work area and the collection and analysis of final clearance air samples from within the work area. Mr. Lococo is a Project Manager certified in the State of Colorado, and has an EPA accreditation number 13438. II. Final Visual Inspection Upon completion of asbestos removal activities, a final visual inspection of the work area is required by the applicable regulations. A certified AMS must visually inspect the work area for any dust or debris remaining in the area. If any such dust or debris is noted by the inspector, the abatement contractor is required to re -clean the work area until such time that the inspecting AMS does not observe any dust or debris in the work area. The final visual inspection of the site was conducted by Mr. Lococo on September 2 2010. It was determined that the abatement contractor had properly cleaned the work area completely of all dust and debris. No further cleaning was required by the inspecting AMS (see Appendix A). III. Final Clearance Air Monitoring Final clearance air samples must be collected from the work area after completion of a final visual inspection and prior to tear -down of the containment structure. Analysis of each these samples must show airborne asbestos fiber concentrations of less than or equal to 0.01 f /cc. If concentrations exceed this limit, the abatement contractor must re -clean the work area and a second set of final clearance air samples must be collected and analyzed. This process is repeated until the analytical results of the final clearance air samples meet the regulatory standard. Only upon successfully completion of the final clearance air monitoring process may an asbestos abatement project be considered complete, and tear -down of the containment structure can commence. The final clearance air sampling was completed by Mr. Lococo of GeoStat, a certified AMS. IV. Final Clearance Air Monitoring Sa nip] ing Methodology Final air clearance samples were collected using constant flow high - volume air sampling pumps, 25 min air sampling cassettes, flexible tubing and adjustable sampling stands. The sampling cassettes were attached to one end of the flexible tubing and placed on the stands approximately forty-eight (48) inches above the floor. The opposite ends of the flexible tubing were attached to the air sampling pumps. Each pump was activated and operated at a flow rate of approximately ten liters per minute (10 LPM) for a period of approximately one hundred thirty (120) minutes. Aggressive air sampling techniques as well as box fans, as described in 40 CFR GeoStat September 15. 2010 APPENDIX A FINAL VISUAL INSPECTION FORM APPENDIX B ANALYTICAL RESULTS APPENDIX C CERTIFICATIONS 1k 0" ............. .
