As requested, structural steel special inspection was conducted at the above referenced project per
the IBC 2003 Edirion - Chapter 17: Structural Tests and Special Inspection and the Town of Vail
requirements. I arrived on site at 11:00 AM. I met with Mr. Jeremy Leibseit with Trinity Welding, Inc. site
to review project requirements and specifications. The steel fabricator and erector for this project is Trinity
Welding, Inc. in Leadville, Colorado. The welder qualification test reports for the welder on site was
reviewed and observed to be acceptable with project specifications. Copies of test reports are on file with
Mr. Jeremy Leibseit with Trinity Welding, Inc. Upon arrival I observed the fit up prior to welding on one
each W8 X 228 Bent Beam Complete Joint Penetration (CJP) Groove Welded connection on two beam
flanges and one beam web. As per Chapter 17 of the IBC, Table 17043, Item S.a.l: Complete and partial
penetrarion groove welded connections require continuous inspection during the welding process.
Visual welding inspection was conducted on the fit up of the CJP welds on one each W8 X 28
Bent Beam referenced above. Approved drawings utilized for inspection were provided by T. Boyle
Engineering Inc. in Vail, CO: Sheet S2 dated 7-16-09. The weld joints were inspected for proper root
opening, included bevel angles and backing bar tolerances per AWS D1.1 - 2008 Edirion. Inspection
revealed acceptable fit up tolerances prior to welding. Welding was begun by certified welder Mr. Jeremy
Leibseit with Trinity Welding, Inc. I visually observed the welding and cleaning operarions being
performed on the root passes, intermediate passes and cover passes during the welding processes. Preheat
and electrical parameters were observed to be within WPS tolerances provided. The CJP welds listed above
were observed to be visually acceptable with requirements stated in AWS D1.1 - 2008 Edition and project
soecifications.

The framing for the project is 100% complete at this time. The purpose of this inspection was to make a final review
of the framing and specifically to scrutinize the modifications to the original structural framing plans that had been
coordinated between the contractor and myself during construction.
I have deternuned that the framing conforms to the plans issued by this office on 9/8/10 which document the bulk of
the modifications that we have coordinated. The floor headering for plumbing and the steel beam saddie/column cap
at the top of the stairs are not detailed on these plans, but I have inspected these items and approve of them as
constructed.
I hereby approve of this remodel as framed. Feel free to give me a call if you have any questions or comments
regarding this report. 1.

Asbestos Testing & Abatement Requirements
Asbestos testing and abatement protects workers, homeowners, neighbors and emergency services responders from ex-
posure to harmful asbestos. The Town of Vail asbestos abatement program is in addition to the State of Colorado's regu-
lations. It is your responsibility to be in compliance with the State. Please contact the State directly for their requirements
at the contact info listed below.
When is asbestos testinct reauired?
ANY building projects disturbing more than these threshold levels of building materials require asbestos testing:
One- and Two-Family Dwellings: 30 square feet
All Others: 160 square feet
Asbestos testing results must be provided with your application for a building permit.
Tests which identify POSITIVE results at more than 1% require abatement by a State-certified and Vail-registered abate-
ment contractor. An asbestos abatement permit must be approved, and the clearance letter must be submitted to the
Town of Vail before the building permit will be issued

Court of Appeals vacated much of the so-called "Asbestos Ban and Phaseout" rule and remanded it to the EPA. Thus,
much of the original 1989 EPA ban on the U.S. manufacturing, importation, processing, or distribution in commerce
of many asbestos-containing product categories was set aside and did not take effect." - CDPHE

Product Description
Bayseal CC closed-cell spray applied polyurethane foam is a two component,
medium-density, structural insulation system designed for commercial,
residential, and industrial applications.
Closed-cell polyurethane foam yields a high R-value and minimizes air and
moisture infiltration. The spray applied nature of Bayseal SPF systems allows
for tremendous sealing properties which contributes to healthier homes and
workplaces. The rigid nature of Bayseal CC increases overall structural
integrity resulting in more durable structures. Bayseal CC expands during
application to provide increased performance values by sealing the building
envelope.
The Bayseal CC system is comprised of an "A" component or aromatic
diisocyanate manufactured by Bayer MaterialScience, and a blended "B"
component which includes polyols, fire retarding materials, catalysts and non-
ozone depleting blowing agents.

Environmental Consideration and
Substrate Temperatures
Applicators must recognize and anticipate climatic conditions prior to
application to ensure highest quality foam and to maximize yield. Ambient
air and substrate temperatures, moisture, and wind velocity are all critical
determinants of foam quality. Extreme ambient air and substrate temperature
will influence the chemical reaction of the two components, directly affecting
the yield, adhesion and the resultant physical properties of the foam insulation.
To obtain optimum results, Bayseal CC should be spray-applied to substrates
when ambient air and surface temperatures are between 50°F and 120°F. All
substrates to be sprayed must be free of dirt, soil, grease, oil and moisture
prior to the application of Bayseal CC. Moisture in any form: excessive
humidity (>85%R.H.) rain, fog, or ice will react chemically will adversely affect
system performance and corresponding physical properties. Application should
not take place when the ambient temperature is within 5°F of the dew point.
Wind velocities in excess of 12 miles per hour may result in excessive loss
of exotherm and interfere with the mixing efficiency of the spray gun affecting
foam surface texture, cure, physical properties and will cause overspray.
Precautions must be taken to prevent damage to adjacent areas from fugitive
overspray.
Processing Parameters
Store at 65° to 85°F in a dry and well-ventilated area. Material in containers should
be maintained at 65°F to 75°F while in use. Heated trailers, hotboxes, or heated
tank storage may be necessary. Material temperature should be confirmed with a
thermometer or IR gun. (continued)

Credentials/Certifications
Bayseal CC is a Class I formulation, as set forth under Underwriters
Laboratories ASTM E-84 (UL 723, NFPA 255, UBC Standard 8-1).
Processing Equipment (continued)
2:1 transfer pumps are recommended for material transfer from container to the
proportioner.
The plural component proportioner must be capable of supplying each
component within ± 2% of the desired 1:1 mixing ratio by volume. Hose
heaters should be set to deliver 125°F to 135 °F inaterials to the spray gun.
These settings will ensure thorough mixing in the spray gun mix chamber
in typical applications. Optimum hose pressure and temperature will vary
with equipment type and condition, ambient and substrate conditions, and
the specific application. Some equipment may require you to heat drums to
achieve optimum material temperafure. It is fhe responsibility of the applicator
to properly interpret equipment technical literature particularly information
that relates acceptable combinations of gun chamber size, proportioner
output, and material pressures. The relationship between proper chamber
size and the capacity of the proportioner'sp re-heater is critical. Contact your
local BaySystems representative for specific recommendations, pricing, and
availability of spray and auxiliary equipment.
CAUTION: Extreme care must be taken when removing and
reinstalling drum transfer pumps so as NOT to reverse the "A"
and "B" components.
Thermal Barrier
IRC and IBC codes require that SPF be separated from the interior of a building
by a an approved fifteen (15) minute thermal barrier, such as 1l2" gypsum walf
board or equivalent, installed per manufacturer's instructions and corresponding
code requirements. There are exceptions to the thermal barrier requirement:
(11 Code authorities may approve coyerings based on fire tests speafic to the
SPF application. For example, covering systems that successfully pass large
scale tests may be approved by code authorities in lieu of a thermal barrier;
(2) SPF protected by 1" thick masonry does not need a thermal barrier. Certain
materials that offer protection from ignition, called "ignition barriers," may not be
considered as thermal barrier aiternatives unless they comply with NFPA 286.
Applicators should request test data and code body approvals or other written
indications of acceptability under the code to be sure that the product selected

by posting appropriate signs warning that afl "hot work" such as welding,
soldering, and cutting witfi torches should take place no less than 35 feet from
any exposed foam. If "hot work" must be performed all spray polyurethane
foam should be covered with an appropriate fire or welder's blanket, and a fire
watch should be provided.
Vapor Retarder
Bayseal CC qualifies as a vapor retarder as defined by the Intemational Code
Council and ASHRAE (class ll) at a minimum thickness of 1 inch. Building
construction types with a, rsistent, high moisture drive require additiona! moisture
remediation, as local buil ing codes dictate. This is includin climate zones 5 and
higher in the U.S., as defined in 2004 Supplement to the IR�, Table N1101.2.
Per Pass Application
Applicators should limit Bayseal CC thickness to 3" per pass for optimal processing
and physical properties. Second passes 'rf necessary should be applied after 10
minutes of cure hme. If additional passes are needed, applirators should wait 30
minutes between passes for optimal foam processing.
Handling and Safety
Respiratory protection is MANDATORY! Contact Bayer MaterialScience for a
copy of the Model Respiratory Protection Program developed by API or visit
their website at www.polyurethane.org . Avoid contact with skin, eyes, and
clothing. Open containers carefully, allowing any pressure to be relieved slowly
and safely. Wear chemical safety goggles and rubber gloves when handling
or working with these materials. In case of eye contacf, immediately flush
with large amounts of water for at least fifteen minutes, consult a physician
immediately. In case of skin contact, wash area with soap and water. Wash
clothes before reuse.
Fire Hazard
Fires involving either of these components may be extinguished with carbon

equipped with NIOSH approved self-contained breathing apparatus.
Cleaning of Spills or Leakage
Cover the area with an iner� absorbent material such as clay or vermiculite
and transfer to metal waste containers. Saturate with water but do not seal
the container with the isocyanates and water mixture. The area should then
be flushed with large.amounts of water, in the case of the "B" component, or a
5% aqueous ammonia, in the case of the "A" component. Dispose of these
materials in compliance with federal, state and local regulations.
Caution: Isocyanates will react with water and generate
carbon dioxide. This could result in rupture of closed
containers.

Basic Use: Fiber glass building
insulation is intended for use in either
residential or commercial construction
as thermal and acoustical insulation in
ceilings, walls and floors.
The product is available unfaced or kraft-
faced and is designed for use in standard
wood stud and steel frame assembiies.

tion markets. In addition to its thermal
properties, Fiber Glass Building
Insulation also provides excellent
acoustical performance. It is compres-
sion packaged for ease of handling, and
its broad availability of R-values, sizes
and facings ensures the right product
for the jof�.
Composition and Materials: The
product is composed of yellow,
uniformly textured, inorganic fibrous
g�dSS.
Sizes: Available standard sizes are
listed in the table at left. Contact
CertainTeed for non-standard sizes.
Limitations: The National Electrical
Code prohibits installation of any insu-
lation over or within 3" (76 mm) of
recessed light fixtures, unless approved
insulated ceiling QC) lighting fi�ures
are used.
Standard kraft facing is flammable and
should not be left exposed. Kraft-faced
insulation must be installed behind
and in substantial contact with the
unexposed surface of the ceiling, floor
or wall finish. Special care should be
taken when working with an open
flame. Where a flame spread rating of
25 is required, insulation must be
unfaced or have a FSK (flame-resistant
foil) facing.
Because of potential skin irritation,
unfaced building insulation should
not be installed in an exposed installa-
tion where it will be subject to human
contact.
All building insulation should be
kept dry. Wet fiber glass insulation will
lose its effectiveness until it dries. Fiber
glass will often dry naturally and
regain its original R-value. However,
under conditions where the insulation
will not dry thoroughly it should be
removed and allowed to dry or be
replaced.

5. INSTALLATION
For most areas, vapor retarders should
be installed on the warm-in-winter
side of the insulation (toward the inte-
rior). For some warm and humid
areas, the vapor retarder should be
installed facing the exterior. Check
local practice and/or building codes.
Installation in wood framing:
Studs—Faced insulation fits between
wood studs with flanges stapled either
to the faces or sides of the studs. Pull
flanges taut while stapling every
8"_12° (203-305 mm) to prevent fish-
mouths. Unfaced rigid fit insulation is
pressure fitted between studs.
Ceiling Joists—Faced insulation is
placed between joists with vapor
retarder facing down. Flanges can be
stapied to bottom faces or sides of
joists if insulation is installed before
ceiling finish. Only unfaced insulation
is installed over existing insulation.
Floor Joists—Faced insulation is
installed with the vapor retarder fac-
ing up and in contact with the floor.

INSULATION
All insulation must be supported
between joists on an approved sup-
port such as wire.
Cathedral Cei/ings—Faced insulation
with vapor retarder facing down is sta-
pled between the rafters. A 1" air space
is recommended between insulation
and roof sheathing. If unfaced insula-
tion is used, a separate vapor retarder
shouid be installed where required.
Installafion in steel framing:
• Standard practice for installing fiber
glass batts in steel studs is to friction
fit batts into stud cavities. When batts
completely fill stud cavities they are
constrained by studs at their edges
and by wall facings front and rear.
When faced batts are used, stapling
flanges are usually left folded.
• When fiber glass batts are installed
in steel ceiling or floor joists or rafters
from below, they must be supported
with wire or a ceiling finish material.
• Ventilation and vapor retarder
requirements are the same as with
wood framing.
