INTRODUCTION:
On Aug 9, 2010 an ins�ection/survey was conductcci and 4 bulk
samples wcrc collected from:
Villa Valhalla
384 Gorc Creek I?r. #1
Vail, Co.
The purpose of the inspection/survey was to locatc: and samp�e
suspected Asbestos containing materials that might be present in the unit # 1
that is planncd for r�;novation.
Z7�c inspection was made, and the samples were collectcd by John R.
Petcrman, an A.H.�.R.A. and State of Colorado and Utah Certified Asbestos
Inspector. Grcat carc was taken during the inspection and sampling to be as
accuratc as possible. It should be noted that minimat damage was done to
the existing building structures during the inspection so thcrc: is no
documentation f�r unseen conditions or stored items.
All samplcs were analyzcd by DCM Science Lab in Whcatridge, CO.
'I'his laboratory is deemed "Proficient" in the E.P.A.Quality Assurance (QA}
program for the determination of asbestos in bulk materials, and is
accredited by the American Hygiene Association (AHA).
SAMPLING PROTnCUL:
A random sampling schcme was used to samplc the suspect materiais that
were discovered. lf during any future demolition or renovatior� w�rk,
suspect material is discovered thal hasn't bccn sampled and would be
dislurbed, work should be halted until the material has been tested.

RF,NOVATION AREA DESCRIPTION:
Villa Valhalla Unit # 1 is planned for selective interior renovation. The walls
are sheetrock. The ceilings are sheetrock like the walls. 7'he floors are
sheetrock like the walls. T'he floors are covered with carpet and sheet vinyl
flooring.
CONCLUSiONS & R�CUMMENDATIONS:
Laboratory analysis, of the bulk samples indicate that Asbestos was
detected in all of the bulk sampies taken. 'The sheet vinyl floorin�; contains
Asbestos that is excess of the Re�ulitory limit. (Greater than 1% Asbestos).
The sheet vinyl will need to be handled as per Colo. Re�. #$ if disturbed, or
covered in place. The sheetrock contains Asbestos that is less than the
Regulit�ry limit. The Asbestos content of#.he sheetrock was verified by the
required Point Count Analysis.

The Villa Valhalla Association, of which I am President, is happy to approve the
kitchen renovation project of Johnfry LLC who are the owners of Villa #1 located at
385 E. Gore Creek Drive, in the Town of Vail.
We understand that this project has no implications for the outside of our building; it
is a kitchen remodel with wall and soffit removal.
It is our understanding that this project will have a fully approved permit before
demolition and construction begin.
We further understand that Ms. Kit Johnson, the representative of Johnfry LLC, has our
Association rules regarding construction and will make every attempt to follow these
rules.

1) All lum-ber shall-be Doug-Fir #2 S4S or better, all wolmamzed ]umber shall be
Hem-Fir: #2 S4S or better (all lumber within 18" of Snish grade or in contact with

diaphragms.

3) All conshvction must comply wiUr the 2009 International Building Code as
adopted by the Town of Vail Building Department and the "General Conditions of
the Contract for Construction" (AIA Document A201). All construction must be
completed within the tolerances described in the "Residential Constniction
Performance Guidelines" as prepazed by the NATIONAL ASSOCIATION OF
HOIvfE BUELBERS.
4) All beams, joists, and rafters aze to bear_upon walls or beams, or be hung with
SIMPSON Strong-Tie hangers or approve equals.
5) All structural members aze shown in their final positions properly braced and
supported; during construction it may be necessary to temporarily shore portions
of the structuro while other portions are being constivcted. Contact the engineer ;
for consultation as required.
6) General Contractor is responsible for all non-specified connections, contact
engineer for details as required. The engineer is to appmve all structural
substitutions.
7) General Contractor is to coordinate all diaphragn penetrations (i.e., chimneys,
plumbing, sewers, etc.) through floors, walls, roofs and foundation walls with
appropriate sub-contractors. Geneial Contractor is responsible for the means,
methods, techniques, sequences, procedures, worlananship, seasonal scheduling
and job-site safety associated with this project The General Contractor is to
verify all dimensions and elevations with the architectural drawings. Notify
engineer of all conflicts and omissions between various elements of the working
drawings and the existing conditions prior to commencing with that portion of the
prc;e:t. General Contractor is [o inspect remodel projects and verify prior use and
proper disposal of existing materiais (i.e., asbestos, etc:) as required by code.
Identity of all existing hidden shuctural plans based upon plans by ANDERSON,
BARKER, RINKER ARCHTTECl'S dated 5 June 1967.
8) All materials exposed to exterior and or moisture conditions shall be treated for
