Responses in RED are prepared by Jeffrey P Manley, AIA ,of Martin Manley Architects. Please call if
you have any further comments. (970-328-1299) Also, note on the revised plans that the Bath and
Closet have been interchanged in the Loft. This was done to simplify the plumbing.
BUILDING DEPARTMENT COMMENTS
1 Revise plans to indicate the building construction rype on the plans per IBC chapter 6. Town of Vail
records indicate the construction type is VA. The cover sheet has been changed to denote the
construction type as VA.
2 Include 1 hour floor/ceiling and 1 hour roof/ceiling construction details with UL listings. The existing
construction assemblies (1-hour) of the floor/ceiling between units and the roof/ceiling are not being
altered. The new portion of the loft floor shall be constructed to compty with GA File NO. FC5415 of
the Gypsum association - 600-2003 Fire Resistance Design Manual, 17th addition. This assenbly has
been added to the Detail Sheet
3 Provide fire dampers at all bath fans in the 1 hour ceilings. Panasonic bath fans with damper are
proposed and now noted on the plans.
4 The building is limited to 4 stories per IBC table 502 (R-2 occpuancy, VA construction). The "loft" level
must qualify as a mezzanine and cannot exceed 1/3 of the floor area of the space in which they are
located. The lowest level (100 level) of the building is to be the basement level. This makes the
building 4 levels (above ground) + and Basement. This is puts the loft in the 4th story which is allowed
with the added floor allowance by the provision for sprinklered buildings.
5 Revise stairway to a minimum 36" width and a minimum 36" landing at the top per IBC 1009. The
existing stair framing and landing framing are existing and are not to be altered. Only new finishes.
6 Revise guardrail heights to 42" per IBC 1013. The guard is revised on the detail to be 42" in height.
7 Indicate on electrical plans for a minimum of 50% of new lighting to be high efficacy per 2009 IECC. A
note has been added to the drawings stating that 50°/o of all new light fixture shall have CFL bulbs (high
efficacy)
8 tndicate compliance with IBC 3403 on the structural plans. Please see attached revised
structural note provided by Monroe Newell.

Asbestos Testing &Abatement Requirements
Asbestos testing and abatement protects workers, homeowners, neighbors and emergency services responders from ex-
posure to harmful asbestos. The Town of Vail asbestos abatement program is in addition to the State of Colorado's regu-
lations. It is your responsibility to be in compliance with the State. Please contact the State directly for their requirements
at the contact info listed below.
When is asbestos testing reauired?
ANY building projects disturbing more than these threshold levels of building materials require asbestos testing:
One-and Two-Family Dwellings: 32 square feet
All Others (commercial spaces, hotel rooms, etc): 160 square feet
Definition of a single-family dwelling: any dwelling unit that is used primarily for a single family, including
multi-family/condominium units,and fractional fee units.
Asbestos testing results must be provided with your application for a building permit.
Tests which identify POSITIVE results at more than 1% require abatement by a State-certified and Vail-registered abate-
ment contractor. An asbestos abatement permit must be approved, and the clearance letter must be submitted to the

Per your request, and under the guidelines defined, HERRON� Enterprises USA, Inc. (HERRON�) has concluded the
Environmental Consultation/Limited Asbestos Building Inspection at the aforementioned property.
Local, state and/or federal regulations, including but not limited to the Air Quality Control Division (AQCC) Regulation 8,
OSHA 1926.1101, and EPA (NESHAP) 40 CFR Part 61 may require a comprehensive asbestos inspection prior to a
renovation or demolition. This inspection requires an AHERA and State certified asbestos building inspector identifying and
sampling any friable and non-friable asbestos containing materials which could be affected by the activity.
HERRON�was contracted by the Client,to perform:
a Environmental Consultation/Asbestos Sarvices, Limited Asbestos Building Inspection, specifically tailored to Friable
and Non-Friable Suspect Asbestos Containing Materials (ACM), associated with the aforementioned property,
specifically;

were sampled, under local, state and/or federal regulations, including but not limited to AQCC Regulation 8 and OSHA

Recommendations
The Client has advised that 641 W.Lionshead Cr.,Vail,CO 81657 is a renovation.
As Regulated Asbestos Containing Materials were identified,HERRON�would recommend as a minimum Plan of Action:
l. Asbestos Abatement Project Design (Plan of Action): coordinated with Owner should be developed in order to
direct exact areas and quantities of removal for renovation or demolition purposes.
2. Asbestos Abatement (to facilitate demolition): coordinated with Client, should the material be required to be
removed prior to renovation or demolition activity:
3. Asbestos Air Monitoring: of project on behalf of Client, i.e., compliance to local, state, and/or federal regulations,
including but not limited to AQCC Regulation 8, OSHA 1926.1101,and EPA(NESHAP)40 CFR Part 61, i.e.,final

Conclusion
Based on the information generated by this report, we conclude that the aforementioned property does have Regulated
Asbestos Containing Material(s)(RACM), in accordance with local,state, and/or federal regulations for purposes of a

Materials that contain<=1.0%Asbestos are not considered Asbestos Containing Materials by NESHAPS or EPA but may
still be covered by OSHA. OSHA regulations may apply during potential disturbance activities.
Although a material may have been determined to contain r-1.0% Asbestos, or during a demolition, if the composition of
drywall/joint compound(when the joint compound does not cover the entire surface)has been determined to contain<=1.0%
Asbestos,this document serves as a Hazard Communication and should be followed during an activity such as a renovation
or demolition, to ensure that an exposure does not occur. The Maximum Allowable Asbestos Level (MAAL) may not be
exceeded at `any' time in accordance with local, state, and/or federal regulations, including but not limited to AQCC
Regulation 8(State),29 CFR 1926.1101 (OSHA),40 CFR Part 61 EPA(NESHAP),and 40 CFR 763 EPA(AHERA).

3. During a normal inspection, and more specifically when non-destructive sampling techniques are employed, it is not
within the scope of the inspection to remove surface materials to inspect the structures and/or materials which may be
under the surface, i.e., within or under concealed areas such as under carpet, under sub-floors, within chases, walls,
crawlspaces, tunnels, etc., to remove suspect Asbestos Containing Material(s), to move and/or sample electrical wiring
which has not been 'locked out', etc. All said areas are to be assumed as containing >1.0%Asbestos, until such a time
that these areas are made accessible,and/or rendered safe so that sampling can be performed.
4. HERRON� recommends extreme caution during a renovation or demolition of these areas in the event that an area
which was not suspect, visible, accessible and/or specified during the inspection, is discovered to contain or is suspected
of containing an Asbestos Containing Material (ACM). Under local, state and/or federal regulations, should such an
event occur,the Client and or Contractor is required to cease operations which may effect this(these)material(s)until an
inspection is concluded and a determination is made by an AHERA and State Certified Asbestos Building Inspector.

aforementioned materials. Only materials impacted requiring response actions have been instructed and authorized by

Code 101896-0 for Transmission Electron Microscopy (TEM) and Polarized Light Microscopy (PLM)
analysis and the American Industriat Hygiene Association(AIHA), Lab ID 101533-Accreditation Certificate
#480 for Phase Contrast Microscopy (PCM) analysis. This laboratory is currently proficient in both
Proficiency Testing and PAT programs respectively.

you have any further comments. (970-328-1299) Also, note on the revised plans that the Bath and
Closet have been interchanged in the Loft. This was done to simplify the plumbing.
BUILDING DEPARTMENT COMMENTS
1 Revise plans to indicate the building construction type on the plans per IBC chapter 6. Town of Vail
records indicate the construction type is VA. The cover sheet has been changed to denote the
construction type as VA.
2 Include 1 hour floorJceiling and 1 hour roof/ceiling construction details with UL listings. The existing
construction assemblies (1-hour) of the floor/ceiling between units and the roof/ceiling are not being
altered. The new portion of the loft floor shall be constructed to comply with GA File NO. FC5415 of
the Gypsum association - 600-2003 Fire Resistance Design Manual, 17th addition. This assenbly has
been added to the Detail Sheet
3 Provide fire dampers at all bath fans in the 1 hour ceilings. Panasonic bath fans with damper are
proposed and naw noted on the plans.
4 The building is limited to 4 stories per IBC table 502 (R-2 occpuancy, VA construction). The "IofY' level
must qualify as a mezzanine and cannot exceed 1!3 of the floor area of the space in which they are
located. The lowest level (100 level) of the building is to be the basement level. This makes the
building 4 levels (above ground} + and Basement. This is puts the loft in the 4th story which is allowed
with the added floor allowance by the provision for sprinklered buildings.
5 Revise stairway to a minimum 36" width and a minimum 36" landing at the top per IBC 1009. The
existing stair framing and landing framing are existing and are not to be altered. Only new finishes.
6 Revise guardrail heights to 42" per IBC 1013. The guard is revised on the detail to be 42" in height.
7 Indicate on electrical plans for a minimum of 50% of new lighting to be high efficacy per 2009 IECC. A
note has been added to the drawings stating that 50% af all new light fixture shall haue CFL bulbs (high
efficacy)
8 Indicate compliance with IBC 3403 on the structural plans. Please see attached revised
structural note provided by Manroe Newell.

� EXECUTIVE SUMMARY
On April 27, 2011, Mr. David Sinkbeil with DS Consulting, Inc. (DSC) conducted a final visual inspection,
performed aggressive clearance air monitoring, and analyzed clearance air-quality samples via Phase
Contrast Microscopy (PCM) for an asbestos abatement project at the condominium residence located at
641 W. Lions Head, Unit 421,Vail, Colorado.
Services were provided at the request of Mr. Richard Capra with Rocky Mountain Abatement in support
of asbestos abatement activities being conducted to remove approximately 550ft2 of asbestos-containing
drywall from a full containment encompassing a portion of the upper level and main level of the
condominium residence. The final clearance also included a final visual inspection of the containment
area which passed. Monitoring locations and frequencies were determined by Mr. David Sinkbeil; having
EPA and State of Colorado accreditation No. 13015. PCM analysis was also performed by Mr. Sinkbeil (see
Appendix B for certifications).
DSC's scope of work for this project included performing a �nal visual inspection and collection and
analysis of aggressive final air quality samples by NIOSH 582-trained personnel for determination of
airborne asbestos (as total fibers) concentrations in identified areas. DSC is a participant in the Industrial
Hygiene Proficiency Analytical Testing (IHPAT) program with laboratory ID No. 188987 and has a rating
of Proficient. The abatement project passed the final visual inspection and the analytical results of all
samples collected were below the clearance criteria of 0.01 fibers/cubic centimeter (f/cc) (seeAppendixA

. PROJECT INTRODUCTION
On April 27, 2011, Mr. David Sinkbeil with DS Consulting, Inc. (DSC) conducted a final visual inspection,
performed aggressive clearance air monitoring, and analyzed clearance air-quality samples via Phase
Contrast Microscopy (PCM) for an asbestos abatement project at the residence located at 641 W. Lions
Head, Unit 421,Vail, Colorado.
Services were provided at the request of Mr. Richard Capra with Rocky Mountain Abatement in support
of asbestos abatement activities being conducted to remove approximately 550ftz of asbestos-containing
drywall from a full containment encompassing a portion of the upper level and main level of the
condominium scheduled for renovation. Monitoring locations and frequencies were determined by Mr.
David Sinkbeil; having EPA and State of Colorado accreditation No. 13015. PCM analysis was also
performed by Mr. Sinkbeil (see Appendix B for certifications).
SCOPE OF WORK
DSC's scope of work for this project included performing a final visual inspection and collection and
analysis of aggressive final air quality samples by NIOSH 582-trained personnel for determination of
airborne asbestos (as total fibers) concentrations in identified areas. DSC is a participant in the Industrial
Hygiene Proficiency Analytical Testing (IHPAT) program with laboratory ID No. 188987 and has a rating
of Proficient. The abatement project passed the final visual inspection and the analytical results of all
samples collected were below the clearance criteria of 0.01 fibers/cubic centimeter (f/cc) (see Appendix A
for Air Sam ple Analysis).
AIR MONITORING
Air-quality samples were collected utilizing a modified NIOSH 7400 Method, whereby samples were
collected on 25-millimeter (mm) mixed-cellulose, ester-membrane filters with 0.45-micron pore size and
analyzed via Phase Contrast Microscopy (PCM) as per the customer's requirements.
Sample Collection - PCM samples, when applicable, were collected on 25-millimeter (mm) mixed-
cellulose, ester-membrane filters with 0.45-micron pore size with an effective collection area of 385 mm.
All filters were pre-assembled by the manufacturer in three stages, conductive sampling cassettes with
extension cowls.
Air samples were collected at an average flow rate 13.0 liters per minute (L/m) using Thomas high-
volume pumps with a minimum of 1,200 liters being collected. Flow rates were collected at the
beginning and at the end of the sampling period utilizing an airflow rotameter calibrated against a
primary-flow calibration instrument. Start times and stop times were recorded for all sampling periods.
A field blank and lab blank were also collected and analyzed along with the five (5) clearance samples.
PCM Sample Analysis Method - PCM samples were analyzed according to a modified NIOSH 7400
Method—A Counting Rules—although samples were collected on 25-mm mixed-cellulose, ester-
membrane filters with 0.45-micron pore size and analyzed via PCM. Air sample results contained in this
report have been calculated with blank sample corrections.

Gentlemen:
Monroe & Newell Engineers, Inc. conducted a structural site visit at Montaneros Lodge
on May 25, 2011. The purpose of this site visit was to observe the framing of the
renovation of Unit 312 visually and to determine, if the framing had been constructed in
general compliance with the intent of the structural construction documents that were
prepared by our office, dated September 13, 20]0 and the additional bent steel detail,
dated May 5,201 l. We noted the following:
1. The new loft expansion framing had been constructed in general compliance
with the intent of the construction documents and appeared to be mostly
complete.
2. The new bent steel beam had been installed as shown in the detail,dated May
5, 2011 and all connectors and support posts were in place.
3. The existing 2 x 12 edge beam at the west edge of the stair to the loft level
was only toe-nailed to the existing loft edge beam that was located at the
south edge of the existing loft. To connect these beams sufficiently the
contractor shall install a Simpson L90 angle at the inside comer of the joint
between these two beams. Lag- bolts, which are protruding into this
connection, shall be cut off flush with the face of the wood beam.
