Structural plans dated 10/28/2011 by Monroe and Newell Engineers, Inc. and Soils Report No. 110317A by HP Geotech, Inc. dated October 27, 2010 were used as the design criteria. HP's report references a maximum soil design pressure of 2,500 psf. We understand that additional observations and letters will be required for each subsequent addition. We observed the excavation for the new footings located between approximately gridlines 1.0- 3.7/South of A.0-B.0 on April 9, 2012. The foundation excavation had been cut in two levels to approximately the bottom of the existing footers (approximately 7-10 feet below existing surface grade). The soils generally consisted of light to medium brown, silty, clayey sands and gravels with cobbles that were similar to those described in the aforementioned HP Geotech Soils Report. Warren with JL Viele notified GROUND that Edwards Excavating had previously utilized a walk-behind sheepsfoot compactor, and jumping jack to compact the footing subgrade. At the time of our visit, the soils appeared moist, dense, and unyielding under moderate construction equipment loads. Based on our observations of the completed foundation excavation at the previously described location, the soil types and condition of the subgrade appears to be generally consistent with soils capable of providing an allowable bearing capacity of approximately 2,500 psf and in general accordance with the project documents. The drilled holes were observed for specified depth, diameter, and cleanliness. Hedrick utilized a wire brush and an air compressor to clean the holes. We then observed Hedrick utilizing SIKA AnchorFix-1 Professional Fast Set Anchoring Adhesive to epoxy reinforcing dowels into the drilled holes until excess epoxy was observed being displaced from the holes. Based on today's observations, the drilled holes and epoxied reinforcing dowels at the previously described locations appeared to have been constructed in general conformance with the project plans and specifications. Structural plans dated 10/28/2011 by Monroe and Newell Engineers, Inc. and Soils Report No. 110317A by HP Geotech, Inc. dated October 27, 2010 were used as the design criteria. HP's report references a maximum soil design pressure of 2,500 psf. We understand that additional observations and letters will be required for each subsequent addition. We observed the new footing excavation located at approximately gridlines D.0-D.8/1.0 and D.8/1.0-2.0 (on the far northwest end of the residence, downhill side) on February 10, 2012. The foundation excavation had been cut in one level approximately 7 to 10 feet below existing surface grades. The soils generally consisted of light to medium brown, silty, clayey sands and gravels with cobbles that were similar to those described in the aforementioned HP Geotech Soils Report. At the time of our visit, the soils appeared moist, dense, and unyielding under moderate construction equipment loads. A very moist area was observed at the south end of the excavation near the existing footer where snowmelt water from the plastic covering had entered. The foundation grade should be allowed to dry prior to the placement of concrete. Precautions should be taken to keep the bearing soils dry. We observed Edwards Excavation utilize a jumping jack to compact the foundation subgrade. Based on our observations of the completed foundation excavation at the previously described location, the soil types and condition of the subgrade appears to be generally consistent with soils capable of providing an allowable bearing capacity of approximately 2,500 psf and in general accordance with the project documents. We did not verify bearing elevations or excavation depths as part of our observations. We understand additional observations and letters will be required for each subsequent addition. Structural plans dated 10/28/2011 by Monroe and Neweil Engineers, Inc. and Soils Report No. 110317A by HP Geotech, Inc. dated October 27, 2010 were provided as the design criteria and reference a maximum soil design pressure of 2,500 psf. The observed addition will be a one story wood frame structure with a crawl space. The excavation was cut in multiple levels up to 18 feet from adjacent ground levels. We observed that the foundation subgrade soils located 9 feet south of gridlines A.0 / 1.0 to 2.8, generally consisted of undisturbed, native, light brown silty, clayey sands with cobbles that were similar to the soils reported in the HP Geotech report, referenced above. At the time of our visit, the soils appeared moist, dense, and unyielding under moderate construction equipment loads. We observed the excavation contractor compact the foundation subgrade with a walk-behind sheepsfoot roller during our site visit. Based on our observations of the completed foundation excavation at the above locations, the soil types and condition of the subgrade appears to be generally consistent with soils capable of providing an allowable bearing capacity of approximately 2,500 psf and in general accordance with the project documentations. We did not verify bearing elevations or excavation depths as part of our observations. The project site is shown on Figure 1. The purpose of the study was to develop recommendations for the fovndation deszgn. The study was conducted in accordance with our proposal for geotechnical engineering services to Alpine Mountain Builders dated September 22, 2010. A field exploration program consisting of exploratory borings was conducted to obta.in information on tkxe subsurface conditions. Samples ofthe subsoils and bedrock obtained during the field exploration were testerl izm the laboratory to deternxine their classification, compressibility or swell and.other engineering characteristics. The results of the field exploratian and laboratory testing were an.alyzed to develop recommendations for foundation types, depths and allowable pressnres for the proposed building foundation. This report summarizes the data obtained during this study and presents our conclusions, design recommendations and other geotechnical engineering considerations based on the proposed construction and the subsurface conditions encountered. PROPOSED CONSTRUCTION The proposed remodel wi11 consist of demo and reconstruction of most of the exis#ing - residence. The builduzg footprint wi11 be in essentially the same location as the existing residence, see Figure 1. The building will be a two story wood frame structure over a walkouY basement level. The attached garage will be single story at the main floor level. Ground floors will be slab-on-grade. Grading for the structure could be relatively extensive due to the relatively steep slope across the lot. We assume relatively light to posszbly moderate foundation loadings, typical of fhe proposed type of constniction. If building loadings, location or grading pIans charige significantly from those described above, w�should be notifed to re-evaIuate the recomrnendations contained in this report. Job Ivo. 11a 3I7A � C��h � -2- STI'E CONDI'I'IONS The lot is occupied by the existing residence approximately as shown on Figure 1. The terrain is steep northerly facing hillside below Forest Road. Slope grades are estimated at about 40 to 60%. Elevation difference across the existing and proposed residence is about 30 feet and elevation difference across the lot is about 70 feet. The building area has undergone considerable grading far constructlon of the existing residence. The existing residence is a two story structure over a walkout basement level apparently founded vn shallow spread footings. There are tarracsd retaining wal]s along the down slope side ofthe residence. Vegetation consists primarily of moderately thick pine and aspen tress with some brush and landscape trees and plants. There aze residences on the adjacent lots and to the north. � FIELD EXPLORATIQN The field exp2oration for the project was wnducted on October 13, 2010. Two exploratory borings were drilled at the locations shawn on Figure 1 to evaluate the general subsurface conditions. The boring locations were limited to the existing driveway due the existing residence and steep hillside slope. The borings were advar►ced with 4 inch diameter continuous flight augers powered by a truck-mounted CME-45B drill rig. The borings were logged by a representative of Hepworth-Pawlak Geotechnical, Inc. Samples of the subsoils and bedrock were taken with 1'/s inch and 2 inch I.D. spoon � samplers. The samplers were driven into the subsoils and bedrock at various depths with blows from a 140 pound hammer fafling 30 inches. This test is sixniIar to the standard penetration test described by ASTM Method D-1586. The penetrtttion resis�ance values are an indication ofthe relative density or consistency of the subsails and t�ardness of the bedrock. Depths at wl�ich the samples were taken and the penetration resistance values are shown on the Logs of Explozatory Borings, Figure 2. The samples were returned to our laboratary for review by the pzoject engineer and testing. - Job No. 110 317A -��� � - 3 - SUBSURFACE CONDiTIONS Crraphic logs of the subsurface conditions encountered at the site are shown on Figu�re 2. The subsoils encountered below the dziveway pavement section and about 7�/Z to 8 feet of fill, consisted of silty and typically clayey sand with gravel and scattered cobbles that transitioned to sand and gravel with cobbles below deptbs of about I9 feet in Boring 2. The sand, and sand and gravel soils contained scattered boulders and was generally medium dense. Limestone bedrock was encountered below depths of about 20%to 32%z feet that extended down to the drilled depths of 26 and 36 feet. At Boring I,there was a . thin Iayer of stiff, sandy clay with shale fragments and an appazent shallow layez�of weathered.clay shale overlying the limestone in both borings. The fill ranged from medium dense silty clayey sand wzth gravel and cobbles in Boring 1 to loose screened . rock in Boring 2. Drilling in the coarse granuIaz.soils with auger equipment was . occasionally dif�icult due to the cobbles and boulders, and generally difficult in the limestone bedrock due to its cemented condition and drilling refusal was encountered in the bedrock. Laboratory testing performed on samples obtained from tkie borings included natural moisture content and density, gradation analyses, .p�tterberg]imi�s and unconfined compressive strength. Results of swell-consolidation testing performed on relatively undisturbed driye samples, presented on Figure 4, indicate the natural sand soils possess moderate compressibility under conditions of loading and wetting with a law hydro- compression potential and the sandy clay with shaie fragment soils have low compressbility. Results ofgradation analyses perfortned on a sma11 diameter drive sample(minus 1%z inch fraction) of the natural silty sand and gravel soiIs are shown on Figure 5. The laboratory testing is summaxized in Table 1. No free water was encountered in the borings at the time of drilling or when checked 1 day Iater. The subsoils and beclrock were slightly moist to moist. 1ob No. l 10 317A NC-�cPbecYl ' -4- FOUNDATION BEARING CONDITIQNS We expect the silty clayey sand with gravel soils will typically be exposed at excavation subgrade. Spread footings bearing on these soiIs shou�d be feasible for foundation support with some risk of settlement. The risk of settlement is primarily if the bearing soils become wetted and precautions should be taken to prevent wetting. Dz-illed piers en@-bearing in the limestone bedrock are a feasible foundation alternative and would provide a re]atively low risk of£oundation movement. � Pmvided below are recommendations for a spread footing�undation system if recommendations for dn�led pier foundatian are desired,we should be contacted. DESIGN RECOMMENDATIONS FOUNDATIONS Considering the subsurface conditions encountered in the exploratoty borings and the nature of the proposed construction,we recommend the buildi�ng be fi�unded with spread foatings bearing on the natural granular soils with some risk of settlement. Existing fill below foatuag areas will need to be removed to expose the natural granular soils. The design and construction criteria presented below should be observed for a spread footing foundation system I) Faotings placed on the undisturbed natwral granular soils should be desigrxed for an allowable bearing pressure of 2,500 psf. Based on experience, we expect settlement of footings designed and constructed as discussed in this section will be up to about 1 inch. There could be some additional settlement if the sand bearing soils become wetted. The magnitude of the additional settlement would depend on the depth and extent of the wetting but naay be on the order of%z inch. �: Job No. 110 3 l7A ��h ' -5 2) The footings should have a minimum width of 18 inches�For continuous walts and 2 feet for isolated pads, 3) Exterior footings and footings beneath unheated areas should be provided with adequate soil cover above their bearing elevation for frost protection. Placement of foundations at least 48 inches below exterior grade is typically used in this area. 4) Continuous foundation walls should be well reinforced top and bottom to span local anomalies and better withstand the effects af some difFerential setttement such as by assuming an unsupported length of at least 12 feet. Foundation walls acting as retaining struc�ures shouId also be designed to resist lateral earth pressures as discussed iun the "Foundation and Retaining � Walls" section ofthis report. � 5) All existiag fi11, topsoil, debris and any loose or dishtrbed soils should be removed and the f+ooting bearing level e�ctended doum to the firm natural gran.ular sozls. FOUNDATION AND RETAINING WALLS Foundation walls and retaining structures up to about 12 feet tall which are laterally supported and can be expected to undergo on�y a slight amount of deflection should be designed for a lateral earth pressure coxnputed on the basis of an equivaient fluid unit weight of afi least 50 pcf for backfill consisting of the on-site granular soils. Cantilevered reta.ining structures up to about 12 feet tall which are separate from the buiiding and can be expected to deflect sufficientiy to mobilize the fizli active earth pressure conditian � should be designed for a laxeral earth presstu-e computed on the basis of an equivalent fluid unit weight of at least 45 pcf for backfill consis�ing of the on-site granular soils. We shot�d be contacted if retaining walls over 12 feet high are proposed at the site. All foundation and retaining structwres should be designed for appropriate hydrostatic and surchaxge pressures such as adjacent footings, traffic, construction materials and equipment. The pressures recommende� above assume dra,ir�ed conditions behind the walls and a horizontal backfilI surface. The buildup of water behind a wall or an upward sloping backfill surface will increase the]ateral pressure imposed on a foundation wall or retainin structure. Backfill should be placed in uniform tifts and compacted to at Ieast 90%of the maximwm standazd Proctor density(SPD) at a moisture content near optimum Backfill in pavement and walkway areas should be compacted to at least 95% SPD. Care shauld be taken not to overcompact the backfill or use large equipment near the wall, since this could cause excessive lateral pressure on the wall. Some settlement of deep foundation wall backfill should be expected, even if the material is placed correctly, and could result in distress to facilities constructed on the hackfill. Use o£a select granular ixnported ma#erial, such as aggregate base course, and increasing compactian to 100% SPD can be done to Izmit fii1 settleznents. � The lateral resistance of foundation or retaini.ng wall footings will be a combination of the sliding resistance of the footing on the foundation materials and passive earth pressure against the side of the:footing. Resistance to sliding at the bottoms of the footings can be calculated based on a coef�xcient of friction of 0.45. Passive pressuxe of compacted backfili against the sides ofthe footings can be calculated Using an equivalent fluid unit weight of 350 pe� The coefficient of friction and passive pressure values recomxnended above assume ultimate soil strength. Suitable factors of safety should be included in the design to Iimit the strai�n which wi11 occur at the ultiznate strength, particularIy in the case of passive resistance. Bxisting fill below slab areas should be removed or evaluated for suitability at the time of construction. To reduce the effects of some differential movement, floor slabs should be separated from all bearing walls and coluxnns with expansion joints which allow unrestrained vertical movement. Floor slab control joints should be used to reduce damage due to shrinkage crackin . The r uirements for 'oint g �! J spacing and slab reinforcement should be established by the designer based on experience and the intended slab use. A minimum 4 inich layer of free-dxaining gravel should be placed beneath basement level slabs to facilitate drainage. This material should consist of minus 2 inch aggregate with at least 50%retained on the No.4 sieve and less than 2% passing the No.200 sieve. All fill materials for support of floor slabs should be co�mpacted to at least 95%of maximum standard Proctor density at a moistuze content near optimum. Required fill can consist of the on-site granular soils devoid of vegetation, topsoil and oversized rocks,or a suitable granular ma.terial such as aggregate base course can be imported. UNDERDRAIN SYSTEM Although free water was not encountered during our exptoration, it has been our experience in mountaiunous areas that local perched groundwater can develop dwing times ofheary precipitation or seasonal iunoff. Frozen ground during spring runoffcarx also create a perched condition. We recornmend below-grade construction, such as retaining walls, crawlspace and basement areas,be protected frorn vcretting and hydrostatic pressure buildup by an underdrain system. Job No. 110 317A ��Ch Y . _ g' The drains should consist of drainpipe placed neaz the bottom of the wall backfill surrounded above the invert Ieve1 with free-draining granular material. The drain should be placed at each level of excavation and at least 1 foot below lowest adjacent finish grade and sloped at a minimum 1%to a suitable gravity outlet. Freo-draining granulaz material used in the underdrain system should contain less t1�an 2%passing the No. 240 sieve, less than 50%passing the No. 4 sieve and have a maximum size of 2 inches. The dra.in gravel backfill should be at least 1 h feet deep. The unde�rdrain systexn should include at least 4 inches of free draini.ng gravel belaw the basement level floor slabs. SITE GRADING There could be a nsk of construction-induced slope instability at the site due to expected relatively extensive grading. We assume the cut depths for the basement level will not . exceed one level, about 10 to 12 feet. Fills should be limited to about 8 feet deep and not be placed on the steep downhill slope. (DSC) performed a limited asbestos inspection and performed asbestos bulk-sampling at 45 Forest Rd, Vail, Colorado, in order to identify potentially hazardous friable and non-friable asbestos-containing materials (ACM) within the above- referenced; single-family residence scheduled for renovations. The Colorado Department of Public Health and Environment's (CDPHE) Regulation 8, Part B defines an asbestos-containing material(ACM)as a material containing more than 1o/o asbestos. Mr. Shurtliff performed asbestos bulk-sampling of a single type of drywall (surfacing material) in a total of seven (7) locations throughout the residence as well as insulation (miscellaneous material) in a total of one (1) location within the residence. No other suspect asbestos-containing materials were found within the residence. All eight (8) bulk-samples were analyzed by Aerobiology Laboratory Associates, Inc. (NVLAP #200860-0) in Denver, Colorado with all eight(8) laboratory results reporting as negative for asbestos (see Appendix B for laboratory results). 2 PROJECT OVERVIEW I. Introduction A limited asbestos inspection and bulk-sampling for ACM was conducted at 45 Forest Rd, Vail, Colorado, by Mr. Shurtliff on October 7, 2010 at the request of Mr. Mark Donaldson with Victor Mark Donaldson Architects. Mr. Shurtliff is a Colorado State Certified Building Inspector; having EPA Accreditation #15413 (see Appendix B for certificate). The purpose of the inspection was to identify, sample and assess potentially hazardous friable and non-friable ACM within the residence and garage that may be impacted by upcoming renovations. II. Structural Design The structure is a wood-framed, multi-level, single-family residence with attached garage. III. Sampling and Analytical Procedures The inspection, assessment and sampling were conducted by an EPA and AHERA accredited Building Inspector qualified by experience, education and training in the recognition of potential ACM and approved bulk-sampling techniques. The asbestos bulk-sampling was conducted on suspect ACM with a limited number of bulk-samples being collected within the residence. The inspection, assessment and sampling were performed in accordance with Environmental Protection Agency/AHERA recommended procedures. These procedures call for the visual inspection of the area of concern and the collection and analysis of representative bulk-samples of suspect material. Some minor destructive sampling was conducted. Walls, columns and perimeter pipe chases were not broken into in order to locate and quantify suspect ACM. It should be noted that additional ACM might be located in these and other inaccessible areas. Random bulk-samples, representative of the suspect asbestos-containing building materials (ACBM) of each homogeneous area (HA), were collected according to the guidelines published as Environmental Protection Agency (EPA) Final Rule: Title II of the Toxic Substances Control Act (TSCA), 15 USC, Sections 2641 through 2654 and in compliance with 40 CFR, Part 763. Representative sampling is based on the following criteria: 1. The distribution of the suspect material throughout the HA. 2. The suspect material's physical characteristics and application. 3. Random sampling patterns determined for each HA. Suspect materials sampled and analyzed should be considered to be representative of materials in each HA if they exhibit similar physical characteristics and the application of the sampled material can be correlated to the application of un-sampled material. Bulk-samples collected were analyzed utilizing the EPA's Method for the Determination of Asbestos in Bulk Buildina Materials (EPA 600/R/116, July, 1993) and the McCrone Research Institute's The Asbestos Particle Atlas as methods references. Analysis of the bulk-samples was performed on the "date reported," as listed in the bulk-sample analysis report. IV. Notes on Report Format Suspect materials alike in appearance and application were sampled as HAs. Suspect materials were divided into three classifications: 1. Surfacing Material: sprayed or troweled onto structural building member. 2. Thermal System insulation (TSI): any type of pipe,boiler,tank, or duct insulation. 3. Miscellaneous Material: other suspect materials, floor tile, sheet vinyl/linoleum, ceiling tiles, insulation, and finishing materials. Condition assessments were performed by the accredited inspector at the time of inspection. Condition assessments are listed in the following section. Ratings of "good," "damaged," and "significantly damaged" are meant to indicate the overall condition of the material. A material in "good" condition has no visible damage or deterioration, or showing only very limited damage or deterioration. A material in "damaged" condition has the following characteristics: The surface is crumbling, blistered, water-stained, gouged, marred or otherwise abraded over less than one-tenth of the surface if the damage is evenly distributed (one-quarter if the damage is localized). Accumulation of powder, dust or debris similar in appearance to the suspect material on surfaces beneath the material can be used as confirmatory evidence. A material in "significantly damaged" condition has one or more of the following characteristics: The surface is crumbling or blistered over at least one-tenth of the surface if the damage is evenly distributed (one-quarter if the damage is localized). One-tenth (one-quarter, if localized) of the material is hanging from the surface, deteriorated, or showing adhesive failure. Water stains, gouges, or mars are over at least one-tenth of the surface if the damage is evenly distributed (one-quarter if the damage is localized). Accumulation of powder, dust or debris similar in appearance to the suspect material on surfaces beneath the material can be used as confirmatory evidence. Response-action recommendations for asbestos-containing HAs are listed in the section VII. Recommendations may be for more than one HA, if materials are alike. Recommendations are either "general" or "immediate." An immediate recommendation indicates the presence of asbestos greater than 1% within the bulk-sample, or a bulk-sample in the same HA, and should be addressed accordingly. A general recommendation indicates asbestos does not exist greater than 1% within the bulk-sample, or a bulk-sample in the same HA, and no further abatement activities are required for removal of the material. Any sample reporting a "TRACE" amount of asbestos must be considered to be positive for asbestos greater than 1%unless it is analyzed by the point-count method to be less than 1%. V. Inspector Comments No asbestos-containing material was identified during the bulk-sampling conducted at 45 Forest Rd, Vail, Colorado. A total of eight (8) bulk-samples were collected from drywall and insulation within the above- referenced residence. All eight(8) bulk-samples tested negative for asbestos. 4 VI. Attn: Mr. Tony Faulhaber, � As requested, a representative of GROUND visited the project on this date to observe the condition of the foundation subgrade for one of the proposed additions to 45 Forest Road. Structural plans dated 10/28/2011 by Monroe and Neweil Engineers, Inc. and Soils Report No. 110317A by HP Geotech, Inc. dated October 27, 2010 were used as the design criteria. HP's report references a maximum soil design pressure of 2,500 psf. We understand that additional observations and letters will be required for each subsequent addition. We observed the new footing excavation located at approximately gridlines C.3-D.6/4.8 (in- between the columns on the northeast end of the residence, downhill side) on March 19, 2012. The foundation excavation had been cut in one level approximately 7 to 10 feet below existing surface grades. The soils generally consisted of light to medium brown, silty, clayey sands and gravels with cobbles that were similar to those described in the aforementioned HP Geotech Soils Report. At the time of our visit, the soils appeared moist, dense, and unyielding under moderate construction equipment loads. A very moist area was observed at the south end of the excavation near the existing footer where snowmelt water from the plastic covering had entered. The foundation grade should be allowed to dry prior to the placement of concrete. Precautions should be taken to keep the bearing soils dry. We observed Edwards Excavation utilize a jumping jack to compact the foundation subgrade. Based on our observations of the completed foundation excavation at the previously described location, the soil types and condition of the subgrade appears to be generally consistent with soils capable of providing an allowable bearing capacity of approximately 2,500 psf and in general accordance with the project documents. We did not verify bearing elevations or excavation depths as part of our observations. We observed holes drilled by Hedrick Construction into both sides of the existing footing for the interior footing extension at approximately gridlines D.0-E.0/2.0; and into the interior (west) side of the existing footing for the interior footing extension at approximately gridlines D.8/4.0-4.8 and D.6-D.8/4.8. The drilled holes were observed for specified depth, diameter, and cleanliness. Hedrick utilized a wire brush and an air compressor to clean the holes. We then observed Hedrick utilizing SIKA AnchorFix-1 Professional Fast Set Anchoring Adhesive to epoxy reinforcing dowels into the drilled holes until excess epoxy was observed being displaced from the holes. Based on today's observations, the drilled holes and epoxied reinforcing dowels at the previously described locations appeared to have been constructed in general conformance with the project plans and specifications. Also, we observed reinforcing steel and wire mesh placed by Coggins and Sons for Wall A between soil nails 16 to 18, fourth row down from the top of the wall. We observed the reinforcing steel for specified bar size, spacing, clearances, and lap splices. We observed the wire mesh for specified size, clearances, and lap splices. Based on today's observations, the reinforcing steel and wire mesh at the previously described locations appeared to have been placed in general conformance with the project plans and specifications. Additionally, at the request of Tony F. with JL Viele, we observed that Edwards Excavating had installed a foundation perimeter/wall drain along the west and northwest sides of the building foundations. Along the west side of the building foundation, west of gridline 1.0 from approximately gridlines A.0 to E.O, 6-inch PVC drain-pipe was installed. Along the northwest side of the building foundation, north of gridline F.0 from approximately gridlines 1.0 to 3.0, 4- inch PVC drain-pipe was installed. At approximately gridlines E.0/1.0, the 4" and 6" PVC pipes converge and drain out to daylight on the north edge of the property. Both the 4 and 6-inch PVC drain pipes were perForated at approximately 4 and 8 o'clock, and were surrounded by what appeared to be free-draining granular material placed over Mirafi fabric. Samples of the grout and shotcrete were periodically tested to determine physical properties and compressive strength. THESE OBSERVATIONS AND TESTING RESULTS WERE SUBMITTED TO THE WALL DESIGNER, COGGINS AND SONS, FOR FINAL APPROVAL. Representatives of our office periodically observed epoxied doweled reinforcing steel placement and performed periodic testing on the structural concrete placed for the foundation, foundation walls, retaining walls, slabs on deck and other structural elements. Samples of the structural concrete were periodically tested to determine physical properties and compressive strength.
