West, Suite 215 Vail, Colorado 81657 Attn: Mr. O'Kelly, As requested, representatives of GROUND were present to participate in Swiss Hammer testing of the concrete associated with Level 1 Pour 1 that was placed on August 6, 2008. The anticipated testing was a result of questions arising from irregular compressive strength test results on cylinder set 136 (Lab ID 934) cast at 11:30 on that day by James Ford. Ground Engineering provided a Swiss Hammer apparatus and PCL Construction also provided a separate Swiss Hammer apparatus to provide comparable information. Upon arriving on-site, the Ground Engineering representatives checked in at the PCL trailer and together with Jeff and Brad from PCL proceeded to the area in question. Based on the locations provided on the test forms, there were 13 separate sets of tests performed on the fresh concrete placed for this pour with a set of cylinders cast for each sample tested. The irregular results were associated with the 8`h test performed that day. The location of the placement of questionable concrete was determined to be near the stairwell between approximately Gridlines XX-LLL, and 57.0-61. The overall area represented by the pour extended from approximately Gridlines XX-PPP and 47.5- 71. GROUND and PCL agreed to perform Swiss Hammer tests throughout the area of the pour to establish a baseline of information for the areas where acceptable compressive strength test results were obtained and then focus several additional tests in the area the concrete in question was likely placed. Swiss Hammer tests were also completed in areas represented by two other pours where acceptable compressive strength test results were obtained in order to provide a reference for the Swiss Hammer readings. The initial tests were completed at 15 separate locations by PCL and GROUND along approximately Gridlines 67, 62, 59, 55 and 50. The results shown below are listed from South to North at approximately 50 ft intervals. Upon completion of the initial round of testing, it appeared that the Swiss Hammer readings from each company were relatively consistent. Ground took 10 additional readings in the area where the concrete in question was placed. As shown below, the readings in this area were consistent with those obtained from the other areas throughout this pour and the other two pour areas. Based on this information, it appears that the concrete in questions is performing in a consistent manner with the concrete placed in areas where acceptable compressive strength test results were obtained. While on-site we observed the placement of post-tension tendons and reinforcing steel for level 3, pour 2. The post tension tendons were observed for specified quantities, profile heights, and general placement within the forms. The reinforcing steel was observed for specified sizes, quantities, splice lengths, clearance, and general placement within the forms. Several chairs were observed to be broken and/or missing across the pour area. Several back-up bars and hairpin bursting steel bars were observed to be missing in many of the banded tendons. Severa� bars also needed to be placed and/or tied securely across the pour area as well. Manny with JD Steel was informed and immediately began correcting the discrepancies. The pour area will be re-observed in the morning prior to concrete placement. Andy with PCL and Manny with JD Steel were informed of today's observations. While walking through the Terrace Level, Pour 3, we observed several areas of the finished concrete appeared to be scaling and brittle. These areas are located a few feet south of the shear wall. West, Suite 215 Vail, Colorado 81657 Attn: Mr. Cody O' Kelly, As requested, a GROUND Engineering Consultants representative was present to perform Swiss Hammer testing of the concrete associated with Terrace Level Pour 6 placed on February 27, 2009. Upon arrival on site we met with Eddy Cotton and Shawn Akin of the OAC, as well as Andrew Rose with PCL. In order to establish a baseline for comparison, GROUND performed Swiss Hammer tests on several locations of Terrace Level Pour 5 where acceptable compressive strength results were obtained. GROUND then proceeded to perform Swiss Hammer tests on various locations of Terrace Level Pour 6. A greater number of tests were performed for pour 6 due to its larger size. The test results appeared to be consistent for each individual pour. While the average of the results for Terrace Level Pour 6 was lower than for Terrace Pour 5, two additional factors should be considered. Terrace Level Pour 5 was poured on November 20, 2008 providing over 100 days of cure time which typically increases the compressive strength. Additionally, the average of the results for Terrace Level Pour 6 were within 15% of the average of the Terrace Level Pour 5 results after only 17 days of cure time. Based on the test data available, the concrete for Terrace Pour 6 appears to be performing as designed structurally. However, ongoing observations should be completed for scaling or other cosmetic defects. Please review the results included in Appendix A and feel free to call with any questions.
