As requested, a representative of GROUND visited the project to perForm construction materials
testing. While on-site, we observed the reinforcing steel for the slab on grade, pour#10 and the
bottom of the pre-cast ramp. The reinforcing steel was observed for bar size, bar number,
spacing, lap, and clearance. The reinforcing steel appears to have been placed in general
accordance with the project specifications.
We observed the CMU for 8' lift (excluding lintels) for the upper level garage, air handling
mechanical room. The CMU was observed for mortar, reinforcing steel placement, joint
reinforcing, and the overall condition of the CMU. The CMU appears to have been placed in
general accordance with the project specifications. Jeff with PCL was notified of the results.

As requested, a representative of GROUND visited the project to perForm construction materials
testing. While on-site we observed reinforcing steel for columns 463, 467, 468, 469, 470, and
475 for level 2, pour 2. Reinforcing steel observations were made for number of bars, grade of
bars, diameter of bars, spacing of bars, lap length and clearance. A few horizontal ties were not
observed to be tied securely in place. Jeff Thompson with PCL was immediately notified and
had the areas corrected. The reinforcing steel then appeared to generally conform with project
plans and specifications.
Vogelman was also observed on-site and had place CMU from elevation 8' to 12' on gridline 43
from D to L.9 and on L.9 from 43 to 35; from elevation 8' to 11' 4" on gridline 35 from L.9 to R
and on gridline 41 from L.9 to N; from elevation 0' to 4' on gridline R from 34 to 35. The
masonry walls were observed for reinforcing steel spacing, grade, and diameter, as well as the
general construction of the wall. These walls appeared to generally conform with the project
plans and specifications. Jeff Rodriguez with PCL was informed of today's observations.

As requested, a representative of GROUND visited the project to perForm construction materials
testing. We observed reinforcing steel placed by J.D. Steel in Columns 471, 472, 474, 466, and
479 on level 2. The reinforcing steel was observed for specified number of bars, grade,
diameter, spacing, lap length, and clearance. The column reinforcing steel appeared to
generally conform to the project plans and specifications. Manny with J.D. Steel was notified.
We observed on the interior masonry walls on upper basement level, Gridline 43 from D to M,
elevation 12' to 16'; Gridline 42 from H.8 to M.L. elevation 21' to 16', and Gridline R from 34 to
35 elevation of 4-8'. We observed the masonry construction for specified reinforcing steel,
spacing, mortar and grout placement, and the general construction of the wall. The walls
appeared to be constructed in general compliance with the project plans and specifications.
Post Tension tendon stressing and elongations were recorded for the southwest corner of the
upper garage. The post tension elongations were observed for level 1, pour 3. Re-stressing of
banded tendons was observed for level 2, pour 1. These results will be forwarded upon
completion.

testing. While on-site we observed the placement of CMU, mortar, construction of mortar joints,
reinforcing steel grade and size, and the general construction of the walls for the lower
basement in rooms L122, 123, 125, and 127. These walls appeared to be constructed in
general compliance with project plans and specifications. Jeff R. with PCL was notified of our
observations.
We also observed the reinforcing steel and post-tension tendons for terrace level pour 2. The
reinforcing steel was observed for specified bar sizes, number of bars, spacing of bars, lap
length and clearance. The post-tension tendon were observed for number of tendons, spacing
of tendons, tendon profiles, and the overall condition of the tendons. We observed missing
back-up bars, missing tendons, missing top mat bars, missing bottom mat bars, and missing
construction joint bars at various locations throughout the deck. Scott with PCL and Pedro with
JD Steel were immediately informed of our observations. JD Steel installed the missing tendons
and reinforcing steel. The post-tension tendons and reinforcing steel then appeared to be
placed in general compliance with project plans and specifications. Scott with PCL was notified
of today's observations.

and 127 from 3' to 4'. These walls appeared to be constructed in general compliance with
project plans and specifications. Jeff R. with PCL was notified of our observations.
We also observed the reinforcing steel and post-tension tendons for placed for level 3 pour 1.
The reinforcing steel was observed for bar size, number of bars, spacing of bars, lap length and
clearance. The post-tension tendons were observed for number of tendons, spacing of
tendons, tendon profiles, and overall condition of the tendons. We observed missing back-up
bars, bursting steel, top mat bars, bottom mat bars, and corner bars at various locations
throughout the deck. Manny with JD steel was immediately informed of our observations and
installed the missing reinforcing steel. The post-tension tendons and reinforcing steel then
appeared to have been placed in general compliance with project plans and specifications.
Andy with PCL was informed of today's observations.
The reinforcing steel placed for footings 1182, 1175, and 1176 was observed for specified size
of bars, spacing, quantity, lap length and clearance were observed. The reinforcing steel
placed for the footings appeared to have been placed in general compliance with project plans
and specifications. Andy and Jeff with PCL were notified of these observations.

basement rooms, L122, 123, 125, & 127 from 4-8' and rooms L128, 130, & 136 from 0-3'. The
masonry walls appeared to be constructed in general compliance with the site specifications.
We observed reinforcing steel for columns 377, 378, 385, 395, & 405 on the Terrace Level. We
observed the reinforcing steel for bar size, number of bars, spacing, clearance, and lap splice.
The reinforcing steel appears to be placed in general compliance with the site specifications.
In addition, we observed reinforcing steel for slab on grade, pour #15. We observed the
reinforcing steel for bar size, number of bars, spacing, clearance, and lap splice. The
reinforcing steel appears to be placed in general compliance with the site specifications. Jeff &
Andy with PCL were notified of our observations.

In addition, we observed reinforcing steel for the terrace level, columns 404, 376, 375, 384, and
385 and on level 3, shear wall 7. We observed the reinforcing steel for bar size, number of
bars, spacing, clearance, and lap splice. The reinforcing steel appears to be placed in general
compliance with the site specifications. Andy with PCL was notified of our observations. We
also observed J.D. Steel stressing Post Tension Tendons on Level 1, Pour 2.
