Frontage Rd. West, Suite 215 Vail, Colorado 81657 Attn: Mr. Cody O' Kelly, As requested, a GROUND Engineering Consultants representative was present to perform materials testing. We observed the Level 2, Pour 4 reinforcing steel for a third time as well as the post tension tendons and stud rails. The rebar and pt tendons appeared to be in general accordance with the project plans and specifications. We also observed reinforcing steel placed for columns 338, 355, 353, 358, & 359 on the Terrace Level and the southeast placing boom pour back holes for Level 5 to Lower Level Garage. The reinforcing steel was observed for bar sizing, spacing tolerances, lap splices, and form clearance. In the Upper Garage level we observed improper splice lengths for the pour back holes. The Lower Level Garage pour back hole was not prepared. These two locations will need to be re-observed prior to the concrete placement. The reinforcing steel placed for the previously mentioned columns appeared to be in general accordance with the project plans and specifications. Manny & Alex with J.D. The reinforcing steel appeared to be in general accordance with the project plans and specifications. In addition, we observed the holes drilled for the reinforcing steel dowels for column 465 on Levels 1, 2, & 3. We observed the reinforcing steel for bar size, spacing, clearance, and lap splices. We observed the drilled holes for depth, diameter and the overall cleanliness. Prior to the placement of the epoxy, we observed PCL utilize a wire brush and shop vacuum to clean the holes. We arrived on-site, at approximately 7:50 am for post tension tendon stressing. We spoke with a representative of J.D. Steel at 8:OOam, our requested time, and they informed us that they still had several more tendons to prepare and we would not be needed until 9:00 am. At 9:00 am we talked to J.D. Steel again and they informed us that they were still not ready. We checked in periodically until 11:30 am when we were again informed it would be some time before they would be ready. The other ram stressed one beam on Level 1, Pour 4A until approximately 11:30 am, when it also broke. We were informed at 1:30 pm that no more stressing would be completed on this day. We perFormed pull testing on two drilled and epoxied vertical reinforcing steel bars for CMU walls on the lower basement level, room L106 west wall. The first bar failed at approximately 1.8 Kips. The second bar failed at approximately 6 Kips. After the first bar was pulled out of the concrete, we observed that the hole appeared to have not been cleaned out properly prior to the placement of epoxy and the hole was approximately 3 '/2" to 4" in depth, as opposed to the epoxy manufacturer's specified depth of 6 3/4" for the #6 size bar used. The holes at BGGG.9-53, BGGG.9-48.1, and YY-53 appeared to have not been cleaned properly, the holes at YY-48.1 appeared to be in general accordance with the site specifications of 4 '/2" depth for#4 size bars.
