While on-site we observed that the shoring wall beginning at the southeast corner of the excavation and continuing to the west approximately 150 feet had failed. We were notified by Brian Conway with PCL that the failure occurred between 6 PM on 1/11/08 and 6:30 AM 1/12/08 and after surveying, the wall had moved approximately 4 feet out and 4 feet down. At the time of our visit a large volume of soil had been previously placed by WVC at the toe of the wall to prevent any further movement. We did not observe any standing water in or around the wall failure; however the outside temperature was well below freezing. The shoring wall appeared to be stable in its current condition. We have been requested to provide a third party review of the shoring wall failure and mitigation. We should be provided with most recent shoring wall plans, be notified when any excavation continues in this area, and be provided any reports specifically those prepared by Yenter Companies and JR Harris. While on-site, we observed WVC Construction and PCL Construction prepare and begin to remove the failed shoring wall at the east side of the South Wall. Each side of the failed shoring wall was saw cut. WVC Construction utilized a Cat 322 C hoe with a thumb to perform excavation and demolition work. WVC Construction removed approximately 20 foot of shoring wall exposing the sidewalk tunnel wall south of the project and the planter foundation walls (approximately 13' high). WVC Construction removed shoring and the soils behind it to approximately 40' west of the southeast corner to the elevation of the tunnel footing (approximately 13'). The soils behind the shoring wall were saturated from the west side of the footing to the west, where a constant water leak was observed. We notified Cowboy with PCL Construction of our observations. While on- site we observed the condition of the footing subgrade for footings 211 and 212 at the east side of the job site. At the time of our observation the footings were excavated to the bottom of footing elevation. The subgrade was observed to contain no loose debris, frost or standing water, was free from organic material and appeared firm. Additionally the material disturbed by the excavation process appeared to have been compacted with a plate compactor. Based on our observations, the footing subgrade material in the observed condition appeared to be in general accordance with Koechlein Consulting Engineers' report titled Geotechnical Investigation, Proposed Ritz-Carlton Residences at Vail, West Day Lot, Vail, Colorado, Job No. 05-148, dated November 29, 2005, prepared for Vail Resorts Development Company. We notified Alan with PCL Construction of our observations. We understand that the footing excavation will remain covered and heated. However, if for any reason the footing subgrade material conditions change or differ from what was observed as described herein prior to placement of concrete we should be notified to reevaluate the subgrade conditions. While on-site we observed WVC Construction had excavated the foundation for the east Sewer Pump Station. At the time of our observation, the footings were excavated to the bottom of the pre-cast pump station elevation. The sub-grade was observed to contain no loose debris, frost, or standing water, was free of organic material and appeared firm. Screened rock was placed as a leveling course. Based on our observations, the pump station sub-grade material in the observed condition appeared to be in general accordance with Koechlein Consulting Engineers report titled Geotechnical Investigation, proposed Ritz Carlton Residences at Vail, West Day Lot, Vail, Colorado, Job Number 05-148, dated November 29, 2005, prepared for Vail Resorts Development Company. IF these conditions change prior to the placement of concrete we should be notified to re-observe the subgrade conditions. We notified Jeff with PCL Construction of our observations. Visual observations indicated above optimum moisture content and the material was excessive deflecting under the weight of loader. Troy with PCL was notified of our observations and West Valley will remove the pumping and wet areas. We were requested to return on February 29, 2008. We observed the reinforcing steel for the footings at gridlines 475/BEEE.8, BEEE.8/48.5. The reinforcing steel was observed for bar size, bar number, spacing, lap, cleanliness and clearance. We observed the reinforcing steel for bar size, bar number, spacing, clearance, and lap splice. The reinforcing steel appeared to be placed in general accordance with the site specifications. Also we observed doweled epoxy bar in this location. We observed hole depth, cleanliness, epoxy placement, reinforcing steel placement, and spacing. The dowels were placed and epoxied in general accordance with the manufacturers and project specifications. We notified Jeff with PCL or our observations. Additionally we observed PCL placing 200 psi B&B Excavating flow fill for areas where outstanding structural fill moisture density test were performed. The test included structural fill test 65 at approximately gridline BGGG.2/66 and 75 at approximate gridline TVV.5-47.7. We notified Jeff with PCL or our observations. The reinforcing steel appears to be placed in general accordance with the project plans and specifications. Jeff with PCL was informed of our observations. We observed PCL placing flow fill around the footers 1240-1246. It is our understanding that PCL elected to use fiow fill because of a near by electrical trench and ground water issues. To complete the job they mixed 3/4" gravel with the native soil to the last 4ft of backfill.
