The following is a summary of the observations perFormed on this date: 1) All observed welded connections of the headed anchor studs to the embed plates were found to be in general compliance with the project plans, except for 13 of the 4248-A embeds because 1 or more welds on each plate had undercut. This is in non-compliance with AWS D1.1-2006, Table 6.1. Item #1 on the Discrepancy List. 2) The attached Observation Summary List has the piece marks and quantities observed on this date. All compliant assemblies were marked with green paint and the non-compliant assemblies were marked with orange paint. West, Suite 215 Vail, CO 81657 Attn: Mr. David Williams As requested, a representative of GROUND arrived at Stresscon, the fabrication facility in Colorado Springs to observe elevator shaft embeds that were fabricated by Zimmerman Metal's. The following is a summary of the observations conducted on this date: 1) All observed embed plate headed anchor stud (HAS) connections on the EWO#2 assemblies were found to be general compliance with the project plans and specifications. One HAS connection was observed to not have a 360 degree flash. This embed was bent 15 degrees in the opposite direction from the missing flash and did not exhibit any cracks. 2) See the attached Observation Summary List for the assemblies observed on this date. The following is a summary of the observations performed on this date: 1) All observed welded connections of wall panels to wall panels at Gridlines BAAA.2 — BGGG.9/72 from the Foundation Level to Level 2, were found to be in general compliance with Stresscon's Drawings, except at three locations, because the welds had undercut. Item #2 on the Discrepancy List. 2) All observed welded connections of wall panels to foundation embeds at Gridlines BAAA.2 — BQQQ.4/72 and BQQQ.4/B62.4-72, were found to be in general compliance with the Stresscon Drawings (Detail 1/S1), except for several locations, because the vertical welds did not have 2 '/2" of weld. Item #3 on the Discrepancy List. 3) All observed welded connections of double tees to inverted tees and panels at Gridlines BAAA.2 — BGGG.2/B62.4-72 on Level 2, were found to be in general compliance with the Stresscon Drawings. 4) All observed welded connections of precast beams to columns, wall panels and precast beams at Gridlines BAAA.2 — BGGG.2/B62.4-72 and BNNN.4- BQQQ.4/B57.2-B62.4 on Level 2, were found to be in general compliance with Stresscon's Drawings. 5) GROUND requested copies of the AWS D1.1 Welding Procedure Specifications (WPS) and Welder Qualification Test Records (WQTRs) for all welders on this project. The following is a summary of the observations and re-observations performed on this date: 1) All observed welded connections of wall panels to wall panels, columns and foundation embeds at Gridlines BTT.9-BSSS.3/B47.5-71 on the Foundation and upper Garage Levels, were found to be in general compliance with the Stresscon Drawings (Details 5/S1, 17/S2, 1/S1, 92/S7, 107/S7 and 70/S5), except for several locations, because of undercut welds, missing welds and two locations that did not have 2 '/2" of vertical weld per Detail 1/S1. These locations were marked with orange paint. Items #5 and #6 on the Discrepancy List. 2) All observed welded connections of the L-beams, inverted tees and precast beams at Gridlines BTT.9-BSSS.3/B47.5-71 on the upper Garage Level, were found to be in general compliance with the Stresscon Drawings (Details 138/S10, 143/S10, 148/S10 and 74/S5), except for several locations missing welds. These locations were marked with orange paint. Item #7 on the Discrepancy List. 3) All observed welded connections of the single and double tees to L-beams, inverted tees and wall panels at Gridlines BTT.9-BSSS.3/B47.5-71 on the upper Garage Level, were found to be in general compliance with Stresscon's Drawings (Details 41/S3, 42/S3, 54/S4, 53/S4, 149/S10, 3/S1, 13/S1 and 29/S2), except at two locations, because of missing connections (at Gridlines BEEE.8-BGGG.2/B47.5 and BGGG.9/69-70), and several missing welds. The following is a summary of the re-observations performed on this date: 1) Items #3 and #6 were re-observed and found to now be in general compliance with the FDG letter dated March 28, 2008; however, two of the panel to foundation connections at Gridlines BTT.9/71 and BQQQ.4/B57.2 were found to be in non- compliance with Detail 1/S1 of Stresscon's drawings, because the foundation embeds were misplaced so they did not have 5" of weld. This item is marked with orange paint. Item #11 on the Discrepancy List. 2) Items #5, #7 and #10 were re-observed and found to now be in general compliance with the Stresscon drawings. 3) Item #8 was re-observed and found to now be in general compliance with the FDG letter dated April 16, 2008. 4) Item #9 was re-observed and found to still be missing the two connections. Item #12 on the Discrepancy List. B) Panel to foundation embed connections are not per Detail 1/S1 and are marked with orange paint. Item#13 on the Discrepancy List. C) Embeds for double tee to panel connections were missing or do not align. They are marked with orange paint. Item #14 on the Discrepancy List. D) Several connections were not complete at the time of these observations. The incomplete connections were not marked with orange paint. Item #15 on the Discrepancy List. 2) All complete precast welded connections that were found to be in general compliance were marked with green paint. The following is a summary of the observations performed on this date: 1) All observed welded connections of precast pieces (that were complete), at Gridlines D-BEE.6/59-72 and C-W/43-59 from the Foundation Level to the Terrace Level, were found to be in general compliance with the approved Stresscon drawings and project specifications, except for several locations that were found to be in non-compliance for the following reasons: A) Embeds were misaligned for L-beam to column connections at Gridlines M/43 on the upper Garage Level and Terrace Level. The following is a summary of the observations and re-observations performed on this date: 1) All observed welded connections of precast pieces at Gridlines R-BTT.9/47-59 from the Foundation to the Terrace Level, were found to be in general compliance with the Stresscon drawings and project specifications. 2) The beam-to-beam connections at Gridlines W/47 on the upper Garage Level and Terrace Levels were welded to one embed and wedge anchored to the other beam. Item #20 on the Discrepancy List. 3) The beam to panel connection at Gridlines B65.4/E on the upper Garage Level was missing the embed in the beam. At the time of our last observations, the beam was not connected to the panel. Item #21 on the Discrepancy List. 4) Several double tee to beam connections at Gridlines R-BTT.9/47-59 were not per Detail 40/S3. The plates were welded to the double tees and wedge anchored to the beams. Item #22 on the Discrepancy List. 5) The panel-to-panel connections at Gridlines B65.4-72/P-BEE.6 from the Foundation to the upper Garage Level, cou�d not be observed at this time, because access could not be provided. Item #23 on the Discrepancy List. 6) Items #12, #17 and #18, were re-observed and found to now be in general compliance with the Stresscon drawings and project specifications. GROUND is requesting approval for the field modification of this connection. Discrepancy Item #73. 5) It was observed that on Level 6 at Gridlines ZZ-EEE/57-61 and R-FF/58-59 that the beams to double angle clip-welded connections have been found in non-compliance per Detail H on Erection Drawing E90. This Detail requires a 5/16" fillet weld on 3 sides and none of these connections have been welded. Discrepancy Item #74.
