EXECUTIVE SUMMARY
On March 11, 2011, Mr. Brandon Sinkbeil with DS Consulting, Inc. (DSC) conducted a final visual
inspection, performed aggressive clearance air monitoring, and analyzed clearance air-quality samples
via Phase Contrast Microscopy (PCM) for an asbestos abatement project at the residence located at 1116
Sandstone Dr.,Unit 102,Vail, Colorado.
Services were provided at the request of Mr. Richard Capra with Rocky Mountain Abatement in support
of asbestos abatement activities being conducted to remove +�-900 ft2 of asbestos-containing drywall
and clean up asbestos-containing debris within a containment encompassing a partion of the lower level
and all of the upper level of the condo. Monitoring locations and frequencies were determined by Mr.
Brandon Sinkbeil; having EPA and State of Colorado accreditation No. 13442. PCM analysis was also
performed by Mr.Sinkbeil (see Appendix 8 for certification),
DSC's scope of work for this project included performing a final visual inspection and collection and
analysis of aggressive final air quality samples by NIOSH 582-trained personnel for determination of
airborne asbestos (as total fibers) concentrations in identified areas. DSC is a participant in the Industrial
Hygiene Proficiency Analytical Testing (IHPAT) program with laboratory ID No. 188987 and has a rating
of Proficient. The abatement project passed the �nal visual inspection and the analytical results of all
samples collected were below the clearance criteria of 0.01 fibers/cubic centimeter (f/cc) (see Appendix A

AIR MONITORING
Air-quality samples were collected utilizing a modified NIOSH 7400 Method, whereby samples were
collected on 25-millimeter (mm) mixed-cellulose, ester-membrane filters with 0.45-micron pore size and
analyzed via Phase Contrast Microscopy (PCM) as per the customer's requirements.
Sample Collection - PCM samples, when applicable, were collected on 25-millimeter (mm) mixed-
cellulose,ester-membrane filters with 0.45-micron pore size with an effective collection area of 385 mm.
All filters were pre-assembled by the manufacturer in three stages, conductive sampling cassettes with
extension cowls.
Air samples were collected at an average flow rate 15.0 liters per minute (L/m) using Thomas high-
volume pumps with a minimum of 1,200 liters being collected. Flow rates were collected at the
beginning and at the end of the sampling period utilizing an airflow rotameter calibrated against a
primary-flow calibration instrument. Start times and stop times were recorded for all sampling periods.
A field blank and lab blank were also collected and analyzed along with the five (5) clearance samples.

Asbestos testing and abatement protects workers, homeowners, neighbors and emergency services responders from ex-
posure to harmful asbestos. It is your responsibility to be in compliance with the State. Ple se contact the State directly

When is asbestos testin4 reauired?
ANY building projects disturbing more than these threshold levels of building materials req ire asbestos testing:
One- and Two-Family Dwellings: 32 square feet
All Others (commercial spaces, hotel rooms, etc): 160 square feet
Definition of a single-family dwefling: any dwelling unit that is used primarily for a single family, including
multi-family/condominium units, and fractional fee units.
Asbestos testing results must be provided with your application for a building permit.
Tests which identify POSITIVE results at more than 1% require abatement by a State-certi ied abatement contractor. The
clearance letter must be submitted to the Town of Vail before the building permit will be is ued.

EXECUTIVE SUMMARY
On February 16, 2011, Mr. Steve Shurtliff of DS Consulting, Inc. (DSC) p rformed a limited asbestos
inspection and performed asbestos bulk-sampling at 1116 Sandstone Dr, Un' 102,Vail, Colorado in order
to identify potentially hazardous friable and non-friable asbestos-containi g materials (ACM) within a
portion of the above-referenced, single-family residence. The Colorado Dep rtment of Public Health and
Environment's (CDPHE) Regulation 8, Part B defines an asbestos-containing aterial (ACM) as a material
containing more than 1% asbestos.
Mr. Shurtliff collected a total of three (3) asbestos bulk-samples of drywall from the following locations
within the residence:
• Lower Level Closet Wall
• Lower Level North Wall
• Stairwell Wall
All three (3) bulk-samples were analyzed by Aerobiology Laboratory Associ es, Inc. (NVLAP#200860-0)
Denver, Colorado with all three (3) laboratory results of drywall being ositive for asbestos at 2%
Chrysotile each (see Appendix C for laboratory results).

PROJECT OVERVIEW
I. Introduction
A limited asbestos inspection and bulk-sampling for ACM was conducted at 116 Sandstone Dr, Unit 102,
Vail, Colorado, by Mr. Shurtliff on February 16, 2011 at the request of M . Bill Simms with Fine Line
Construction, Inc. Mr. Shurtliff is a Colorado State Certified Building Inspec or; having EPA Accreditation
#15413 (see Appendix A for certificate). The purpose of the inspection was identify, sample and assess
potentially hazardous friable and non-friable ACM within the residence t at have been affected by a
recent remodel.
II. Structural Design
The structure is a wood-framed, two-level, single-family residence.
III. Sampling and Analytical Procedures
The inspection, assessment and sampling were conducted by an EPA and AHERA accredited Building
Inspector qualified by experience, education and training in the recog ition of potential ACM and
approved bulk-sampling techniques. The asbestos bulk-sampling was cond cted on suspect ACM with a
limited number of bulk-samples being collected within the residence.
The inspection, assessment and sampling were performed in accordance wi h Environmental Protection
Agency/AHERA recommended procedures. These procedures call for the vi ual inspection of the area of
concern and the collection and analysis of representative bulk-samples of su pect material.
Some minor destructive sampling was conducted. Walls, columns and per meter pipe chases were not
broken into in order to locate and quantify suspect ACM. It should be noted hat additional ACM might be
located in these and other inaccessible areas.
Random bulk-samples, representative of the suspect asbestos-containing uilding materials (ACBM) of
each homogeneous area (HA), were collected according to the guidelines ublished as Environmental
Protection Agency (EPA) Final Rule: Title II of the Toxic Substances Control ct (TSCA), 15 USC, Sections
2641 through 2654 and in compliance with 40 CFR, Part 763. Representat ve sampling is based on the
following criteria:
1. The distribution of the suspect material throughout the HA.
2. The suspect material's physical characteristics and application.
3. Random sampling patterns determined for each HA.
Suspect materials sampled and analyzed should be considered to be repres ntative of materials in each
HA if they exhibit similar physical characteristics and the application of t e sampled material can be
correlated to the application of un-sampled material. Bulk-samples collecte were analyzed utilizing the
EPA's Method for the Determination of Asbestos in Bulk Buildina Materials ( PA 600/R/116, July, 1993)
and the McCrone Research Institute's The Asbestos Particle Atlas as method references. Analysis of the
bulk-samples was performed on the "date reported," as listed in the bulk-sa ple analysis report.

IV. Notes on Report Format
Suspect materials alike in appearance and application were sampled as As. Suspect materials were
divided into three classifications:
1. Surfacing Material: sprayed or troweled onto structural building me ber.
2. Thermal System Insulation (TSI): any type of pipe, boiler,tank, or duct insulation.
3. Miscellaneous Material: other suspect materials, floor tile, sheet inyl/linoleum, ceiling tiles,
insulation, and finishing materials.
Condition assessments were performed by the accredited inspector at the ime of inspection. Condition
assessments are listed in the following section. Ratings of "good," " maged," and "significantly
damaged" are meant to indicate the overall condition of the material.
A material in "good" condition has no visible damage or deterioration, o showing only very limited
damage or deterioration.
A material in "damaged" condition has the following characteristics: The s rface is crumbling, blistered,
water-stained, gouged, marred or otherwise abraded over less than on -tenth of the surface if the
damage is evenly distributed (one-quarter if the damage is localized). Acc mulation of powder, dust or
debris similar in appearance to the suspect material on surfaces beneath he material can be used as
confirmatory evidence. A material in "significantly damaged" condition has ne or more of the following
characteristics:
• The surface is crumbling or blistered over at least one-tenth of the s rface if the damage is evenly
distributed (one-quarter if the damage is localized).
• One-tenth (one-quarter, if localized) of the material is hanging from the surface, deteriorated, or
showing adhesive failure.
• Water stains, gouges, or mars are over at least one-tenth of the su ace if the damage is evenly
distributed (one-quarter if the damage is localized). Accumulatio of powder, dust or debris
similar in appearance to the suspect material on surfaces beneath he material can be used as
confirmatory evidence.
Response-action recommendations for asbestos-containing HAs are isted in the section VII.
Recommendations may be for more than one HA, if materials are alike. ecommendations are either
"general" or "immediate." An immediate recommendation indicates the resence of asbestos greater
than 1% within the bulk-sample, or a bulk-sample in the same HA, and shou d be addressed accordingly.
A general recommendation indicates asbestos does not exist greater than 1 o within the bulk-sample, or
a bulk-sample in the same HA, and no further abatement activities are equired for removal of the
material. Any sample reporting a "TRACE" amount of asbestos must be c nsidered to be positive for
asbestos greater than 1% unless it is analyzed by the point-count method to e less than 1%.
V. Inspector Comments
Asbestos-containing material was identified during the bulk-sampling cond cted at 1116 Sandstone Dr,
Unit 102,Vail, Colorado. A total of three (3) bulk-samples of drywall were c llected from the lower level
north wall, the lower level closet wall and the stairwell wall of the above-re renced residence. All three
(3) bulk-samples of drywall tested positive greater than 1% for asbestos.

VII. Recommendations
IMMEDIATE:
The laboratory results of the potential ACM sampled at 1116 Sandstone Dr, Unit 102, Vail, Colorado, on
February 16, 2011, indicate that all three (3) bulk-samples of drywall collected from the residence
reported as positive for asbestos at 2% Chrysotile.
Since the above-referenced ACM had been significantly damaged by the remodel and the total quantity
exceeds the regulatory trigger level of 32ftz on surfaces, the entire residence deemed as a "Major
Asbestos Spill" per Colorado Regulation No. 8, Part B -Asbestos (Reg. 8).
This area is consequently subject to the requirements in Reg. 8, Section III.T.1. - Major Asbestos Spills
(see Appendix C�, including but not limited to: restricting access, posting warning signs, and de-energizing
the HVAC systems to these areas; immediately contacting the Air Quality Control Division of CDPHE;
using Colorado-certified, asbestos-abatement supervisors and workers to establish non-detect air
pressure and abate the contaminated areas; and collecting aggressive, final air-clearance samples. Please

III.T.1. Major Asbestos Spills
In the event of an asbestos spill involving greater than the trigger levels, the building owner or
contractor shall:
III.T.1.a. Restrict access to the area and post warning signs to prevent entry to the area by
persons other than those necessary to respond to the incident.
III.T.1.b. Shut off or temporarily modify the air handling system to prevent the distribution of
asbestos fibers to other areas.
III.T.1.c. Immediately contact the Division by telephone, submit a notification in compliance
with subsection III.E. (Notifications) and, if in an area of public access, apply for a
permit in accordance with subsection III.G. (Permits).
III.T.1.d. Be exempted from the requirements to have a certified Supervisor on-site at all
times, until such time as the immediate danger has passed. Any cleanup or asbestos
abatement that must occur after the immediate danger has passed shall be
supervised by a person certified by the Division.
III.T.1.e. Using certi�ed Supervisors and certified Workers in accordance with section II.
(Certification Requirements) of this Regulation, seal all openings between the
contaminated and uncontaminated areas and establish non-detect air pressure
within the contaminated area in accordance with paragraph III.J. (Air Cleaning and
Non-detect Pressure Requirements). This is to be accomplished using polyethylene
sheeting to cover areas such as doorways, windows, elevator openings, corridor
entrances,grills, drains,grates, diffusers and skylights.
III.T.1.f. HEPA vacuum or steam clean all carpets, drapes, upholstery, and other non-clothing
fabrics in the contaminated area, or discard these materials.
III.T.1.g. Launder or discard contaminated clothing in accordance with subsection III.R.
(Waste Handling).
III.T.1.h. HEPA vacuum or wet clean all surfaces in the contaminated area.
III.T.1.i. Discard all materials in accordance with subsection III.R. (Waste Handling).
III.T.1.j. Following completion of subparagraph III.T.1.a. through III.T.1.i. above, comply with
