RE: Narrative for Permit Requirements, Asbestos Abatement, Lions Square Lodge North
LVI Environmental Services Inc. (LVI) is planning an asbestos abatement project
at the Lions Square Lodge North Condominiums, 600 East Lions Head Circle, Vail, CO.
The asbestos abatement project is being conducted in preparation for the building
renovation procedures. As an attachment to this narrative, LVI is submitting a work
plan and design for the project explaining in detail the project tasks. In general the entire
building will be under four full containments per Colorado Regulation No. 8 for the
removal of asbestos acoustical ceiling spray and asbestos containing drywall texture.
The project is scheduled to begin on April 7, 2008 and be complete on May 30,
2008. LVI will disable any and all fire alarms and smoke heads prior to the start of
asbestos abatement procedures. Any standpipes located in the area will remain charged
throughout the duration of the abatement project. All electricity will be isolated to the
main electrical panels where a sub - contracted and Colorado licensed electrician will
hard wire temporary GFCI protected power panels to be used for abatement purposes.
LVI will provide 24 -hour fire watch and keep records on site for review.
The attached floor plan drawing shows location of the standard entry and exit
through the personnel decontamination units located on the west side of the building,
the emergency egress location on the east side, the waste load -out and equipment
staging on the north and the negative air machines and exhaust on the south. The upper
level is typical in building footprint and negative air location. Hand held extinguishers
will be located at each stairwell on each floor, at all power panels (building and
temporary), and at close proximity to all negative air machines.
In the case of an evacuation of the building during abatement procedures, all LVI
personnel shall exit down and out at the nearest stairwell location and meet across the
street to the north of the building for roll call. All roadway access on the south side of
the building is to stay open for emergency vehicle access at all times.

Introduction:
On June 10, 2008, Farmer & Associates, Inc. (F &A)'s Air Monitoring Specialist, Billy
Godby, collected final air clearance samples from the Lions Square Lodge North
located at 6060 E. Lions Head Circle in Vail, Colorado.
Prior to testing, a visual inspection was performed to confirm the completeness of
asbestos removal and cleanup. The visual inspection was performed in accordance with
the American Standard for Testing and Materials' "Standard Practice for Visual
Inspection of Asbestos Abatement Projects" All abated asbestos materials, including
contaminated dust, debris or residues were satisfactorily determined to have been
removed.
Air samples were then collected inside the contained area utilizing aggressive air
sampling techniques as described in 40 CFR Part 763, Appendix A to Subpart E (EPA
1995).
Five (5) air samples and two (2) blanks were analyzed utilizing Phase Contrast
Microscopy (PCM) employing the NIOSH 7400 method. The results of the PCM
analyses are as follows:

successfully participates in the American Industrial Hygiene Association's Proficiency
Analytical Testing Program (PAT) for determining concentrations of airborne asbestos

The sample results complied with the State of Colorado Department of Public Health and
Environment regulations for post- abatement air quality.

D &D Environmental Consulting, Inc. (D &D) is pleased to provide the following letter report of findings
regarding the final visual inspection and final clearance air monitoring and analysis performed on the second
floor of the Lions Square Lodge Condominium Complex, North Building. The final clearance air monitoring
samples were collected only after the final visual inspection was completed and successfully passed. The
final visual inspection and the final clearance air monitoring samples were completed on the second floor
Units on June 3, 2008.
A total of seven final clearance air - monitoring samples were collected, including two field blanks, from
inside the single work area located on the second floor of the condominium north building and were
analyzed for airborne fibers. Tables A, lists the sample numbers and the results as fiber per cubic centimeter.
The samples were analyzed using the National Institute for Occupational Safety and Health (NIOSH)
Method 7400, counting rules "A ". The air sample results were reported to be below the State of Colorado's
Air Quality Control Commission's Regulation No.8 maximum allowable asbestos level of 0.01 f /cc and the
Environmental Protection Agency's (EPA) clearance criteria of 0.01 fibers per cubic centimeter (f /cc). Table
A &B lists the sample numbers and the results as f /cc.

D &D Environmental Consulting, Inc. (D &D) is pleased to provide the following letter report of findings
regarding the two -final visual inspections and final clearance air monitoring and analysis performed at the
Lions Square Lodge Condominium Complex, North Building. The two sets of final clearance air monitoring
samples were collected only after the final visual inspection was completed and successfully passed. The
final visual inspections and the final clearance air monitoring samples were completed on May 15, 2008 and
May 23, 2008 of the nine units located on the third floor of the structure.
A total of ten final clearance air - monitoring samples were collected, plus four field blanks, on two different
dates as referenced above, from inside the work areas located on the third floor of the condominium north
building and were analyzed for airborne fibers. Tables A &B, listed below, reference the Units that were
samples according to the dates.
The samples were analyzed using the National Institute for Occupational Safety and Health (NIOSH)
Method 7400, counting rules "A ". The air sample results were reported to be below the State of Colorado's
Air Quality Control Commission's Regulation No.8 maximum allowable asbestos level of 0.01 f /cc and the
Environmental Protection Agency's (EPA) clearance criteria of 0.01 fibers per cubic centimeter (f /cc). Table
A &B lists the sample numbers and the results as f /cc.

RE: Preliminary Results - Lions Square North Building:
The following memo lists the results of the bulk samples collected on 4/1 & 2/2008 at the
Lions Square North Building complex. Representative bulk samples were collected of
ceiling texture, wall texture and drywall /drywall joint compound. The analytical results
are listed as a percent of asbestos within the matrix material as well as the type of
asbestos detected. Although bulk samples were not collected from each of the 27- Units,
all of them were viewed to determine if the materials were homogeneous (looked, felt the
same and were installed during the same time period).
CEILING TEXTURE:
Sixteen representative bulk samples were collected and submitted for rush 2 -hour
turnaround time analysis of ceiling texture material. Of the sixteen samples collected and
submitted for analysis, four were reported as none detected (ND), four were reported as a
trace amount (1% or less), and eight were reported as containing two percent asbestos.
WALL TEXTURE:
Thirty representative bulk samples were collected of wall texture from the various walls
of the units sampled. Of the thirty bulk samples collected, sixteen were reported as ND,
twelve were reported as a trace amount, and two were reported as containing 2 percent
asbestos. In addition, all of the results were reported as drywall joint compound layer.
DRYWALL /DRYWALL JOINT COMPOUND (MUM
Fourteen bulk samples were collected. Of that total, three were reported as ND and
eleven were reported as a trace amount in the mud layer.
Prior to considering a material as unregulated, a confirmation analysis is required that is
intended to confirm the visual estimation method. Therefore, D &D recommends that the
wall texture bulk samples reported as containing a trace amount to two - percent be
resubmitted for point -count analysis.
The analytical results listed above must be considered preliminary only. A final report of
findings will be prepared for the bulk samples collected including recommendations, if
applicable.
