Box 3419 Vail Colorado 81658 ATTN: Mr. Mark Stevenson RE: The Tyrolean -Unit #7 Remodel Permit #B 11 -0045 400 East Meadow Drive Vail, Colorado 81657 Dear Mr. Stevenson, As requested, structural steel special inspection was conducted at the above referenced project per the IBC 2009 Edition - Chapter 17: Structural Tests and Special Inspection and the Town of Vail requirements. i arrived on site at 10:30 AM. I met with you on site to review areas ready for inspection. Prior to my arrival this week you contacted me regarding inspection requirements implemented my Ms. Kith McGee with KRM Consultants, Inc. for the non - inspected CJP groove welds as referenced in my 9 -6- 11 report. Ultrasonic welding inspection shall be conducted on all 4 splices on site for further examination of the CJP groove welds per AWS D 1.1: Structural Welding Code — Steel. Ultrasonic inspection was conducted on 4 each CJP groove welded connections as referenced in my 9 -6 -11 report. Inspection began utilizing a Krautkramer USK -6 Ultrasonic Instrument with a KB Aerotech .75 X .75 Diameter transducer (SIN 032KTF) with an approved 700 probe. Calibration and sensitivity levels were set on an AWS DSC Block with a 10" screen range. The sensitivity level was set at 80% full screen height. Scanning was conducted at approximately 15 -20 Db over the sensitivity level. The Upper Roof bent beam single splice CJP groove weld was inspected on both flanges and web. No unacceptable indications were observed during inspection. The 3 CJP groove welded splices above the South window were inspected and all three were observed to have rejectable indications in various flange and web locations. During inspection I observed the following unacceptable fabrication conditions: 1) All beam access holes were torch cut without grinding the radius surfaces to provide a notch free area as required, 2) The center CJP groove weld at the top flange to web "k" area was observed to have 2 gouges that were evident to be thru the web that were un- repaired, 3) The center CJP groove weld at the web: The weld was stopped short approximately I/," of its total length, and 4) The far left CJP groove weld's top flange has un- repaired grinding mark gouges measured to be approximately 1/8" deep by 1 ' /Z" long. Upon completion I contacted Ms. Kim McGee to inform her of the unacceptable welding conditions observed. She is in process of re- engineering repair procedures and additional beam reinforcement plates for installation. Continued inspection shall be conducted on 9- 19 -11. Resp° 'fully Submitted, Sc . I arrived on site at 11:30 AM. I met with you on site to review project requirements and specifications. The steel fabricator and erector for this project is Vail Manufacturing, Inc. in Vail, Colorado. The welder qualification test reports for the welder on site was reviewed and observed to be acceptable with project specifications. Copies of test reports are on file with you. Visual welding inspection was conducted at the Upper Level Floor Framing Plan on the column to beam and beam to beam fillet welded connections as required on Sheet S2: Details 1, 2, 4 & 5 on approved project drawings provide by KRM Consultants, Inc. in Vail, CO: Project # 1010 -06 dated 6- 10 -11. Inspection was conducted on welded material on entire level. All fillet welded connections referenced were observed to be as detailed and in compliance with project specifications. Visual inspection was conducted at the Upper Level Floor Framing Plan on the 3/4" ASTM A325 High Strength beam to beam and beam to column bolted connections. All bolted connections were inspected for "snug tight' ' tension requirements as referenced in the AISC: Manual of Steel Construction and Chapter 17 of the IBC: Table 1704.3 for Periodic Inspection requirements. Inspection was conducted on all bolts. Bolts were observed to be adequately tensioned to a snug tight condition. During my inspection on site I observed two fabricated bent beams that have a total of 4 each Complete Joint Penetration (CJP) groove welds that were not continuously inspected by an approved inspector during the fabrication process as required per Chapter 17: Table 1704.3, Item 5.a.1. I then contacted the project structural engineer Ms. Kim McGee with KRM Consultants, Inc. for inspection recommendations. She informed me that further consideration of this matter is needed prior to implementing an alternative inspection on site. Peterman Asbestos Testing, Project Design, and Consulting INTRODUCTION: On March 14th, 2005, an inspection /survey was conducted and 6 bulk samples were collected from the: The Tyrolean Unit #7 400 E. Meadow Road Vail, CO. 81657. The purpose of the inspection/survey was to locate and sample suspected Asbestos containing materials that might be present in the unit that is planned for renovation. The inspection was made, and the samples were collected by John R. Peterman, an A.H.E.R.A. and State of Colorado Certified Asbestos Inspector. Great care was taken during the inspection and sampling to be as accurate as possible. It should be noted that minimal damage was done to the existing building structures during the inspection so there is no documentation for unseen conditions or stored items. All samples were analyzed by DCM Science Lab in Wheatridge, CO. This laboratory is deemed "Proficient" in the E.P.A.Quality Assurance (QA) program for the determination of asbestos in bulk materials, and is accredited by the American Hygiene Association (AHA). SAMPLING PROTOCOL: A random sampling scheme was used to sample the suspect materials that were discovered. If during any future demolition or renovation work, suspect material is discovered that hasn't been sampled and would be disturbed, work should be halted until the material has been tested. RENOVATION AREA DESCRIPTION: Unit #67, at The Tyrolean, is planned for renovation work that will impact the inside, and some exterior areas of the unit. The exterior walls are covered with plaster, and the interior walls are covered in sheetrock with a texture, and finished wood. The ceilings are covered in sheetrock with texture, and finished wood like the walls. The floors are covered with carpet, ceramic tiles, and stone tiles. The heating is provided by a fireplace and baseboard heating. CONCLUSIONS & RECOMMENDATIONS: Laboratory analysis of the bulk samples indicate that no Asbestos was detected in any of the bulk samples. 4 Date: March 14, 2005 Location: The Tyrolean, Unit W, 400 E.
