It is your responsibility to be in compliance with the State. Please contact the State directly for their requirements at the contact info listed below. When is asbestos testin4 reauired? ANY building projects disturbing more than these threshold levels of building materials require asbestos testing: One-and Two-Family Dwellings: 32 square feet All Others (commercial spaces, hotel rooms, etc): 160 square feet Definition of a single-family dwelling: any dwelling unit that is used primarily for a single family, including multi-family/condominium units, and fractional fee units. Asbestos testing results must be provided with your application for a building permit. Tests which identify POSITIVE results at more than 1% require abatement by a State-certified abatement contractor. The clearance letter must be submitted to the Town of Vail before the building permit will be issued. Peterman Asbestos Testing, Project Design, and Cansulting INTRODUCTIQN: On April 22nd, 2004, an inspection/survey was conducted and 30 bulk samples were collected from the: The Lodge at LionsHead 380 E. LionsHead Circle Various Units and Common Areas in Phases T, II, and I�I Vail, CO. 81658 The purpose of the inspection/survey was to locate and sample suspected asbestos containing materials that might be present in the designated area of the unit that is planned for renovation. i The inspection was made, and the samples were collected by John R. Peterman, an A.H.E.R.A. and State of Colorado Certified Asbestos Inspector. Gz�eat care was taken during the znspection and sampling to be as accurate as possible. It should be noted that minimal damage was done to the existing building structures during the inspection so there is no documentation for unseen conditians or stored items. All samples were analyzed by DCM Science Lab in Wheatridge, CO. This laboratory is deemed "Proficient" in the E.P.A.Quality Assurance (QA) program for the deternunation of asbestos in bulk materials, and is accredited by the American Hygiene Association (AHA}. SAMPLING PRUTOCOL: A random sampling scheme was used to sample the suspect materials that were discovered. If during any future demolition or renovation work, suspect material is discovered that hasn't been sampled and would be disturbed, work should be halted until the material has been tested. 3 . � A & D Asbestos Testing and Consult�ng John R. LionsHead CircIe Vail, CO. 8Ib5$ RENOVATION AREA DESCRiPTION: The Lodge at LionsHead consists of three buildin�s that were constructed at different dates. Phase I built in 1970-1971, Phase II built in 1974, and Phase III built in 1978. The buildings are five stories tall with parking areas in the basement area. For the pvrgoses of this report/inspection, each building is considered a separate homogeneous area. A Representative of LionsHead Lodge assisted in the inspection by escorting, and providing access and building information. The exterior walls are concrete with a rough texture applied to the outside. The interior of the building has had some renovation throughout the years. The original areas that have not been renovated were the facus of this inspection. The interior wa11s are sheetrock with a light texture. In some areas it appears that a troweled on texture has been applied over the original sheetrock and light texture. The ceilings in Phases T & TI are sheetrock with texture like the walls and a spray on texture on the concrete areas and same sheetrock areas. Care should be taken in the sheetrock ce�ling area to check for the spray on texture on the concrete above the sheetrock. Phase III ceilings are sheetrock with light texture like the walls. No acoustic spray on texture was observed in Phase IZI. The parking garage in Phase III has a concrete ceiling with a spray on fireproofing. The floors are covered with carpet for the most part; also some ceramic tiles. There is some sheet vinyl flooring that was not tested, as it will not be disturbed by the planned activities. The heating is provided by hot water baseboard heating and fireplaces. No visible suspect T.S.I. was observed in the units. The boiler rooms were not inspected at the time of this inspection. 4 � - A & D Asbestos Testing and Consulting John R. L'zonsHead Circle Vail, CO. 81658 CONCLUSIONS & RECOMMENDATIaNS: Laboratory Analysis of Che bulk samples indicated that Phases I and ZI tested positive for Asbestos in the spray on ceiling texture, and in the composite sheetrock samples. Exterior wall texture on Phase I also contains Asbestos. The spray on ceiling texture in Phases I and II contain Asbestos in excess of the Regulatory Limit (Greater than 1% Asbestos}. The exterior wall texture on Phase I also contains Asbestos in excess of the Regulatory Limit. These Asbestos containing materials must be handled as per Colo. Reg. #8 prior ta disturbance. The composite sheetrock samples in Phases I and II contain Asbestos that is less than the Regulatory Limit. The Asbestos content of the sheetrock mud was verified by the required Point Count Analysis. The composite sheetrock is not considered to be Asbestos containing and need not be abated prior to disturbance. The O.S.H.A. regulations that protect warkers from air borne Asbestos fibers are still in place. Sulk samples are prepared for analysis using a 90X-80X stereo microscope in a hepa filter hood which provides a contamination-free enviranment. The sample is then analyzed by polarized light microscopy(PLM) at 100X. When the sample consists of more than one layer, each layer is prepared and analyzed separately. Fiber and matrix materials are identified by the characterization of optical proQerties including color and pleochrorism, form, cleavage, relief, birefringence, extinction, orientation, #winning, interference#igure and okher ctistinguishing features. Dispersion sta�ning is also used to further aid in mineral identification. All percentages of asbestos, okher fibers and non-fibrous constituents are calculated from the values obtained from the stereo and PLM microscopes analysis. In-house and NIST standards as well as a chart prepared by R.D. Terry and G.V. Chilinger for'The Joumal of Sedimentary Petrology", (Volume 24, pp. 228-234, 1955) provide a guide for estimating percentages. All samples are archived for six months unless other arrangernents are made 6y the client. ACCREDITAT(�N: DCMSL is accredited by the AIHA{since 1986). Our laboratory number is 101526. Bulk samples are prepared for analysis using a 10X-80X stereo microscope in a hepa filter hood which provides a contamination-fres environment. The sample is then analyzed by polarized light microscopy(PLM)af 100X. When the sample consists af more than one layer, each layer is prepared and analyzed separately. Fiber and matrix materials are identified by the characterization of optical properties including color and pleochrorism, form, cleavage, relief, birefringence, extinction, orientation, twinning, inkerterence figure and other distinguishing features. Dispersion staining is also used to further aid in mineral identification. All percentages of asbestos, other fibers and non-frbraus constituents are calculated from the values obtained from the stereo and PLM microscopes analysis.
