General Information: This application is for all changes to approved plans prior to Certificate of Occupancy. An ap-
plication for Design Review cannot be accepted until all required information is received by the Community Development
Department. Design review approval expires one year from the date of approval, unless a building permit is issued and
construction commences.

Re: Lionshead Mini Golf Weed Plan
Please refer to the attached Landscape Plan. It is our intention to fully implement the Landscape Plan.
The area covered by it will be maintained in a weed free state and will be checked for weeds annually
and sprayed as needed,
Any ground that is disturbed during construction of the Mini Golf Course that is not covered by the
Landscape Plan will be revegetated by broadcast seeding our approved Mountain Seed Mix at a rate
of 50 pounds per acre. Before seeding the area will be fertilized with an application of 300 pounds per
acre of 25-25-0 fertilizer. After seeding the area will be covered with weed free straw mulch at a rate of
two tons per acre, a tackifier may be used.
The area will be checked annually for weeds and sprayed as needed.
