The owner or his/her representative, on the advice of the design professional in responsible charge, shall
complete, seal, sign and submit a copy of the Special Inspection Agreement and Structural Tests
Scheduled to the Town of Vail for review and approval. Signatures are required on both pages;
photocopied or faxed signatures are acceptable.
The owner and his /her general contractor, where applicable, shall also acknowledge the following
conditions applicable to Special Inspection Testing:
1. Contractor is responsible for proper notification to the Inspection or Testing agency for items
listed.(Page 1) (IBC 1704)
2. Only the testing laboratory should take samples and transport them to their laboratory.
3. Copies of all laboratory reports and inspections are to be sent directly to the Town of Vail by the Testing
agency on a weekly basis.
4. Inspection agency to submit names and qualifications of on -site special inspectors to the Town of Vail
for review and approval.( Page 2)
5. The special inspector is responsible to immediately notify the Town of Vail Building Official in writing of
any concerns and /or problems encountered.
3. It is the responsibility of the contractor to review the Town of Vail approved plans for additional

The following responses, shown in bold, address the comments in your letter of June 8,
2011. Also we have included four complete, revised sets of drawings.
BUILDING COMMENTS:
Architectural Comments:
General:
1. All plan sheets to be signed and dated per the State of Colorado.
The four sets of revised drawings have all of the sheets stamped and signed.
2. Provide Plans and specifications for the required radon system per Appendix F of the
IRC.
Plans and specifications for the proposed radon mitigations system have been
added to sheet M1.1, the First Level Mechanical Plan.
3. Provide a window and door schedule on the plans showing the location and u value
of glazing per the submitted REScheck program.
A complete window and door schedule, from the manufacturer, is attached with
the location and u value for all openings.
Sheet Al. 1:
1. Provide engineered design for boulder walls four feet or taller per Town of Vail
amended code section 1603.2 IBC.
There are no boulder walls over four feet tall, see sheet A1.1, Site Plan.
2. Provide engineer design for the proposed retaining walls exceeding four feet and or
supporting surcharge.
Refer to details 13, 14, 15, and 16 on structural detail sheet S2.2 for the
construction of the proposed concrete retaining walls. These details are now
referenced on the site plan, sheet ALL
Structural Comments:
General:
Provide structural design criteria or structural calculations identifying all design
criteria in compliance with the building code.
The structural design criteria, which was originally indicated on sheet S2.1, is
now shown on sheet S2.4. Please note that the snow load criteria has been
revised, and also see sheet S1.5 for upgraded snow loads to accommodate
potential drifting snow.
2. Identify on plans all required special inspections per the IBC and include a special
inspection agreement available on the Town of Vail web page.
A table indicating all special inspections requirements is shown on sheet S2.4.
The special inspections agreement will be provided prior to issuance of permit.
Plumbing Comments:
Sheet Al. 1:
1. Revise plans to show building sewer being provided from the public utility on each
individual property. Reference section 701.3 IPC.
Separate sewers are now shown for each individual property on sheet AU, and
are referenced to section 701.3 IPC.
Mechanical Comments:
Sheet MI. 3:
1. Indicate on plans the proposed cfin for the kitchen hoods to identify the need for
outside air. Reference section 505.2 IMC.
Sheet M1.3 has been revised to show the cfm capacity for the kitchen hoods and
section 505.2 IMC has been referenced.
Electrical Comments:
General:
1. Identify on plans the locations of the required CO detectors per the IRC.
The electrical plans have been revised to show required CO detectors per the
IRC.
PLANNING COMMENTS:
The area of the west garage was expanded in order to address building height issues,
however the cover sheet still identifies the garage area as 600 square feet. I believe
this garage may exceed 600 square feet resulting in the need for a new GRFA
analysis to be performed. Please revise the cover sheet and provide a new GRFA
analysis. From the cover sheet calculations the secondary unit is at the allowable
maximum for GRFA. I will be looking to see how the expansion of the garage
square footage is accounted for and affects the allowable GRFA for the site and the
secondary unit.
The size of the west unit garage was not expanded. When the west garage door
was pulled out, to rectify the height issues, the corresponding opposite wall was
also reconfigured to maintain the size of the garage. Please refer to previous
drawings for comparison and/or measure the garage to verify actual area.
2. Please provide a cut sheet of the proposed light fixture in order for staff to determine
that the light fixture will comply with the 14 -10 -7, Outdoor Lighting, Vail Town
Code.
A cut sheet is attached but does not show the reflector which provides the Dark
Sky compliance. We are waiting for a detail drawing from the manufacturer
and it will be provided before permit issuance.
3. Please show the gutters and downspouts on the elevation plans.
Drawings have been revised to show gutters and downspouts on the roof plan
and exterior elevations.
4. Please show the snow guards on the elevations or roof plans.
All snow guards are indicated on the roof plan.

All trees shall be 10 feet minimum from the edge of roadway to drip line.
Any trees planted within the right of way shall 10 feet minimum from edge of

2. Provide stamped plans /clacs for all walls 4 feet or greater in height, including boulder
walls.
Refer to details 13, 14, 15, and 16 on structural detail sheet 52.2 for the
construction of the proposed concrete retaining walls. These details are now
referenced on the site plan, sheet A1.1. There are no proposed boulder walls
over 4 feet high.
3. Revocable permit required for private improvements located in Town Right of Way.
An application for `Revocable Permit to Erect or Maintain Improvements on a
Public Right -of -Way" was submitted to public works on June 23.
Hopefully I have interpreted your comments correctly, and have provided you with
everything you need to be able to issue a permit.

NOTE: These samples have not been analyzed using the Point Count method. These
samples have been analyzed by the PLM method, which is faster, less expensive and less
accurate than Point Counting.
According to the State of Colorado, if PLM analysis shows 10% asbestos or less, the
material can be either assumed to be Asbestos Containing Material (ACM), or the
material can be analyzed by the Point Count method for verification above or less than
1% asbestos.

Asbestos Inspection Report
If the Point Count result is greater than or equal to 1 % asbestos, then the State of
Colorado, OSHA and EPA all DO consider the material to be an ACM. If the Point
Count result is less than 1% asbestos, the State of Colorado does NOT consider this to be
an ACM. HOWEVER, OSHA and EPA DO consider the material to be an ACM no
matter how small the percentage. No matter what the Point Count result, OSHA and
EPA Federal Regulations DO require a complete asbestos abatement if the material is to
be disturbed.
CONCLUSIONS:
2% asbestos was found in the texture. 5% asbestos was found in the floor tile under
the stairs. These samples have not been analyzed using the point count method,
described above. Unless Point Counting is done, this material is assumed to be an
Asbestos Containing Material by the State of Colorado, OSHA and EPA.
Less than 1% asbestos was found in the joint compound / sheetrock, when measured
as a composite material. This is meaningless unless the texture is found to be less than
1% asbestos, in which case the State of Colorado will not care, but OSHA and EPA still
DO require a complete asbestos abatement.
Indoor Environmental Services recommends that material containing any amount
of asbestos be disturbed and disposed of only by a professional asbestos abatement
contractor.
NOTE: Neither I nor Indoor Environmental Services get anything from recommending
these contractors. I recommend them because they have a reputation for being responsive
and doing a good job.

Asbestos Inspection Report
This inspection includes only suspect materials that are visible to the inspector. If other
suspect materials become visible during demolition, they must be sampled before they
are disturbed. Examples of suspect materials include but are not limited to: linoleum,
floor tile, ceiling tile, mastic, sheetrock, joint compound, roofing materials, etc. There are
over 3000 known asbestos containing materials.
SAMPLING PLAN:
Colorado State Requirements - For each homogenous area of less than 1000 square
feet, a minimum of three samples must be collected in random areas. For each
homogenous area of 1000 — 5000 square feet, a minimum of five samples must be
collected in random areas. Areas of 5001 square feet or greater require a minimum of 7
samples. These laws are defined by the EPA (AHERA) and Colorado State Regulation 8.
Determination of homogenous sampling areas — Homogenous Areas are determined by
type of suspect material, texture, color and location.
Determination of random sampling areas — The samples were collected in a random
manner.

SCOPE OF STUDY
This report presents the results of a subsurface Soil and Foundation Investigation for a
proposed duplex to be constructed on Lot 5, Block 1, Vail Village 6" Filing, at 616 Forest Road,
Town of Vail, Eagle County, Colorado. The purpose of the subsurface soil and foundation
investigation was to determine the engineering characteristics of the foundation soil and to
provide recommendations from a geotechnical point of view for the foundation design, grading,
and drainage. Geologic hazard studies are outside of our scope of services.
SITE DESCRIPTION
Lot 5 is a 1.034 -acre lot, on the south side of the west portion of Forest Road, in the Vail
Village Subdivision, 6' Filing, Town of Vail, Eagle County, Colorado. The topography of the lot
is moderate to steep. Drainage is to the north and northwest. Vegetation on the lot consisted of
an aspen forest with few pines. There was one to two feet of snow on the lot at the time of
drilling. Lot 5 has an existing duplex on it. All of the surrounding lots have buildings.

PROPOSED CONSTRUCTION
We understand that the existing duplex will be replaced with a new duplex. The new
duplex will have four stories stepping into the hillside. It will have a wood frame construction,
on a concrete foundation. Loads are anticipated to be light, typical of residential construction.
If the finalized plans differ significantly from the above understanding, we should be
notified to reevaluate the recommendations of this report.
FIELD INVESTIGATION
The field investigation, conducted on December 21, 2010, consisted of boring, logging
and sampling two test borings. The test boring locations are shown on Drawing No. 1. Drilling
of the test borings was done with a 4 -inch diameter, continuous flight, power auger, on a rubber
track mounted drill rig. We show the soil profiles of the test borings on the Subsurface
Exploration Logs, Figure No's one and two. Soil samples for laboratory soil analysis and
observation were taken at selected intervals by advancing Standard Split Spoon and California
Samplers with an automated 140 -pound hammer, falling 30 inches.
SUBSURFACE SOIL AND GROUNDWATER CONDITION
The soil profiles encountered in the test borings were fairly uniform. Boring No. I had
about four feet of fill with topsoil and dark sand over light to yellowish brown, clayey sand over
brown, clayey -sandy gravel with cobbles and boulders to the maximum depth explored of 21
feet, where refusal was encountered. Boring No. 2 also had some fill with topsoil, road base and
sand, over brown, sandy gravel with cobbles and boulders to the maximum depth explored of 19
feet. Ground water was not encountered in either boring at the time of drilling. Test boring No. 2
was drilled with a 6 -inch auger and was filled once drilling was complete. Test boring number
one was left open and was covered with a rock for future observation of possible ground water.

At a follow up site visit, 48 hours after the drilling, there was no water in the test boring. The 4-
inch auger was sheared off on the gravel in test boring number one, and the bottom half
remained in the hole.
We sampled the soil in the test borings at random intervals. The soil samples were
brought to our laboratory for observation and analysis.
FOUNDATION RECOMiMENDATIONS
The proposed residence should be supported with conventional type spread footings,
designed for a maximum allowable soil bearing pressure of 3000 psf. The footings should be
placed on undisturbed sandy gravel with cobbles and boulders. To prevent the potential of a
differential settlement, we recommend that the entire foundation be constructed on the clayey,
sandy gravel with cobbles and boulders.
The undersigned engineer should observe the foundation excavation to verify that the soil
conditions are as anticipated from the borings. For any additional foundation alternatives,
please contact our office.
We recommend a minimum width of 16 inches for the continuous footings and 2 feet for
the isolated footing pads. Continuous foundation walls should be well reinforced, top and
bottom, to span an unsupported length of at least 10 feet. A minimum backfill cover as required
by the local building department should be provided for frost protection of the footing subsoils.
The foundation excavation should be free from excavation spoils, frost, organics and
standing water. Any overexcavation within the proposed foundation, should be backfilled, in 8
inches loose, level lifts and compacted to 98% of the maximum dry density and within 2 percent
of the optimum moisture content as determined in a laboratory from a Standard Proctor test
(ASTM D -698). Structural fill, placed under footings, should be tested by the engineer or her
representatives on regular basis.

SLAB CONSTRUCTION
The natural on -site soils, exclusive offill, topsoil, and organics, are suitable to support
lightly loaded slab -on -grade construction. The subgrade for the slab -on -grade construction
should be proof compacted to detect and remove soft spots. They should backfill over - excavated
soft spots and other underslab fill with the on -site soil, free from topsoil and organics, or other
suitable material, compacted to a minimum of 95 percent of the maximum standard Proctor
density (ASTM D -698). Suitable material should be free from topsoil, organics and rock
fragments greater than 3 inches.
The concrete slab should be constructed over a 4 -inch layer of clean gravel consisting of
-3/4 inch gravel with at least 50% retained on the No. 4 sieve and less than 3 percent passing the
No. 200 sieve.
They should reinforce the concrete slab -on -grade and score control joints according to
the American Concrete Institute requirements and per the recommendations of the designer to
reduce damage due to shrinkage. The concrete slab should be separated from the foundation
walls and columns with expansion joints to allow for independent movement without causing
damage.
RETAINING WALLS
Foundation walls retaining earth and retaining structures that are laterally supported
should be designed to resist an equivalent fluid density of 60 pcf for an "at- rest" condition.
Laterally unrestrained structures retaining the on -site earth should be designed to resist an
equivalent fluid density of 40 pcf for the "active" case.
Passive earth pressure of 300 psf can be used for the lateral pressure against the sides of
the footings. Resistance to sliding at the bottom of the footings can be calculated based on a
coefficient of friction of 0.50. Undisturbed soil or a structural, non - swelling fill compacted to
100 percent of the maximum dry density and within 2 percent of the optimum moisture content

shall be used to resist lateral loads at the sides of the footings.
The above design recommendations assume drained backfill conditions and a horizontal
backfill surface. Surcharge loading due to adjacent structures, weight of temporary stored
construction materials and equipment, inclined backfill and hydrostatic pressure due to
undrained backfill should be incorporated in the design. Every attempt should be made to
prevent the buildup of hydrostatic pressure behind the retaining wall.
UNDERDRAIN SYSTEM
To reduce the risk of surface water infiltrating the foundation subsoil, installation of a
foundation perimeter drain is recommended (see Figure No. 5). Even though, ground water was
not encountered during the drilling or at the follow up reading 24 hours after the drilling, it is
anticipated that ground water seepage will be present, especially during the spring runoff and
snow melt. We recommend a perimeter drain behind every step of the foundation. The
foundation perimeter drain should consist of a minimum size of a 4 -inch diameter perforated
pipe sloped to a suitable gravity outlet, or to a sump pump location. If ground water, or signs of
fluctuating ground water levels are encountered during construction, we should be contacted for
an additional consultation and /or modification of the underdrain recommendations. The drain
should slope at 1/4 inch per foot if flexible or at 1/8 of an inch if rigid pipe is used. The bottom
of the trench adjacent to the footing should be lined with a polyethylene moisture barrier glued to
the foundation wall. The drain pipe should be placed over the moisture barrier and covered with
a minimum of 6 inches of -3/4 inch free - draining granular material. Geotextile (Mirafi 140N or
equivalent) should be used to cover the free - draining gravel to prevent siltation and clogging of
the drain. The backfill above the drain should also be free draining, granular material to within 2
feet of the ground surface to prevent a buildup of hydrostatic pressure. The top one foot of the
backfill material should consist of a relatively impervious fill. The backfill should be sloping
away from the building.

SITE GRADING AND DRAINAGE
The site surrounding the building structure should slope away from the building in all
directions. A minimum of 12 inches in the first 10 feet is recommended in unpaved areas, and
three inches in the first 10 feet in paved areas. The top of the granular foundation backfill
should be covered with a minimum of one foot of relatively impervious fill to reduce the
potential of surface water infiltrating the foundation subsoils. Exterior backfill should be
compacted at or near the optimum moisture content to at least 95% of the maximum standard
Proctor density under pavement, sidewalk and patio areas and to at least 90% of the maximum
standard Proctor density under landscaped areas. Mechanical methods of compaction should be
used. Do not puddle the foundation excavation.
Permanent, unretained cut and till slopes should be graded at two horizontal to one
vertical or flatter with slope heights not exceeding 20 feet. Slopes exceeding 20 feet should have
benching incorporated. To protect the newly created slopes, erosion control methods should be
used. Also, drainage of the site above the slopes should be controlled to prevent slope instability.
If water seepage is encountered during the slope construction, additional studies will be required
to evaluate the slope stability.
Surface water naturally draining toward the proposed building site should be diverted
around and away from it by means of drainage swales or other approved methods. The roof
drains and downspouts should extend and discharge beyond the limits of the backfill.
LAWN IRRIGATION
It is not recommended to introduce excess water to the foundation soils by installing
sprinkler systems adjacent to the building. The installation of the sprinkler heads should insure
that the spray from the heads will not fall within 10 feet of foundation walls, porches or patio
slabs. Lawn irrigation must be controlled.

engineering standards for similar methods of testing and soil conditions at this time. There is no
other warranty either expressed or implied.
The findings and recommendations of this report are based on field exploration,
laboratory testing of samples obtained at the specific locations shown on the Location Sketch
Figure No. 1 and on assumptions stated in the report. Soil conditions at other locations may vary,
which may not become evident until the foundation excavation is completed. If soil or water
conditions seem different from those described in this report we should be contacted immediately
to reevaluate the recommendations of this report.

Comments:
Exposed earth in unvented crawl space foundations is covered with a continuous vapor retarder (less than or equal to 0.1 perm). All joints
of the vapor retarder are overlapped by 6 inches and are sealed or taped with edges extending at least 6 inches up the stem wall and
securely attached.
Air Leakage:
❑ Joints (including rim joist junctions), attic access openings, penetrations, and all other such openings in the building envelope that are
sources of air leakage are sealed with caulk, gasketed, weatherstripped or otherwise sealed with an air barrier material, suitable film or

(e) Plumbing and wiring: Insulation is placed between outside and pipes. Batt insulation is cut to fit around wiring and plumbing, or
sprayed /blown insulation extends behind piping and wiring.

Sunrooms that are thermally isolated from the building envelope have a maximum fenestration U- factor of 0.50 and the maximum
skylight U- factor of 0.75. New windows and doors separating the sunroom from conditioned space meet the building thermal envelope
requirements.
Materials Identification and Installation:
Lj Materials and equipment are installed in accordance with the manufacturer's installation instructions.

Cj Duct tightness test has been performed and meets one of the following test criteria:
(1) Postconstruction leakage to outdoors test: Less than or equal to 478.1 cfm (8 cfm per 100 ft2 of conditioned floor area).
(2) Postconstruction total leakage test (including air handler enclosure): Less than or equal to 717.1 cfm (12 cfm per 100 ft2 of
conditioned floor area) pressure differential of 0.1 inches w.g.
(3) Rough -in total leakage test with air handler installed: Less than or equal to 358.6 cfm (6 cfm per 100 ft2 of conditioned floor area)
when tested at a pressure differential of 0.1 inches w.g.
(4) Rough -in total leakage test without air handler installed: Less than or equal to 239.0 cfm (4 cfm per 100 ft2 of conditioned floor area).
Heating and Cooling Equipment Sizing:
