After most of the demolition of the Wildflower has been completed and the existing structure revealed,the design and construction team has concluded that the existing building was not built per Type I-A Building Standards and requirements. However,the Building looks to be designed and constructed as a Type II-B`Unprotected Non-Combustible Building'. Some of the clues that helped us conclude the building should be classified at a Type II-6 are as follows: i) The existing building is a 4-story building with mezzanine, R-i occupancy group allows 4 stories plus the building is sprinklered allowing s additional story. CLEANUP All subcontractors are required to keep their work areas clean and organized. All areas should be cleaned up at the end of each shift(broom clean). This jobsite is very high profile and can be seen from existing areas. Unorganized work and storage areas will not be tolerated. ■ Each subcontractor will be responsible for participating in a composite clean-up crew. All debris resulting from the work needs to be removed to a dumpster. ■ All trash and material deliveries through occupied or completed areas shall be covered and carts shall have wheels protected or taped to limit tracking of materials. ■ Should work take place in finished or existing areas, the subcontractors are required to clean up and return work area to original condition before work commenced (i.e. vacuums, rags, dusting). ■ Temporary partitions will be required for dust control and the sealing of HVAC systems to prevent contamination will be required (i.e. poly and media filter). ■ This subcontractor is responsible for removal from the project of all of the subcontractor's materials and trash that appear on any of the EPA lists of Identified Hazardous Waste or that display a hazardous characteristic. These materials are not to be discarded in GE Johnson Construction Companies trash piles or dumpsters. ■ Food trash to be placed in acceptable locations and disposed of properly in Bear-Proof containers. EMPLOYEE SITE/ DELIVERY ACCESS ■ No construction vehicle shall be parked on or along roads within Vail Village right-of-ways. ■ All employees must park their vehicles at the offsite location designated by their employer(Vail Parking Garage). Parking in unauthorized locations will result in the removal of the vehicle and/or worker from the jobsite. ■ Absolutely no parking is allowed in the Lodge at Vail parking garage site except the following vehicles. o Delivery/Haul off trucks. o Company issued vehicles, which have approved by the Town of Vail. o Other vehicles specifically authorized by GEJCC. ■ Absolutely no unauthorized vehicles will be allowed on the jobsite or in parking garage. ■ Jobsite traffic must yield to public/guest traffic at all times. ■ Every effort must be made to allow Owner's business to continue in an uninterrupted and safe manner. There is a special importance on this project for good housekeeping and for accommodation of non- construction personnel on the site in a safe and reasonable manner. ■ The general public is located around the Lodge at Vail and therefore around the jobsite. Proper attire, language, and attitude are required at all times. Employees violating this rule will be removed from the jobsite. ■ All employees will conduct themselves as professionals while on this project. ■ Subcontractors shall be responsible for any impairment of the buildings life safety systems. On March 31,20I 1, AGW collected bullc samples o f suspect buildiug materials which will be impacted during renovation activities planned for tha Wildflower Restaurant and Mickey's Piano Bar. In ttddition to coilecting asbestos bulk samples, AGW conducted a visual mold evaluation of the areas scheduled for renovation. The following report presents the results of the limited asbestos survey,and summarizes our physical findings. Visual Observations and Findings-Mold Evaluation • No significant water dalnage or mold growth was observed on any of the accessible building materials in the Wildflower Restaurant or Mickey's Piano Bar. Asbestos Survey Activities AGW was requested to provide a limited asbestos survey relative to the materials anticipated to be impacted by renovation activities. AGW's scope of work for this project included the collection of bulk samples of suspect asbestos containing inaterials by personnel certified as Asbestos Building Inspectors by the Colorado Department of Public Health and Environment (CDPHE}, and the Environmental Protection Agency(EPA). This limited asbestos inspection included only the materials anticipated to be impacted byrenovation activities in the Wildflower Restaurant and Mickey's Piano Bar. Other suspect materials exist within the remainder of the Lodge that were not sampled during this event, Asbestos Sampling Procedure Suspect materials were grouped into homogeneous areas based on their physical characteristics, color, texture, and functional application, Suspect materials were divided into one of three categories based on the followinghomogeneous area classification schemes recognized by regulatory agencies: 1) Thermal systems insulation(TSI): any type of pipe,boiler, tank or flue insulation. 2) Surfacing materiat: sprayed or troweled onto structural building members. 3) Miscellaneous: a11 other suspect materials, including flooring, ceiling tiles, floor coverings,other insulations, finishing materials, etc. Random bulk samples were collected according to the g�.iidelines published as Environmental Protection Agency Final Rule: Title TI of the Toxic Substance Control Act (TSCA), 15 USC, Sections 2641 through 2654 and in compliance with 40 CFR,Part 763 and the CDPHE Regulation #8. A11 samples collected were placed in sealed bags for delivery to the labaratory. Samples collected were given unique identificatioia numbers in accordance with esta.blished AGW protocols for sainple numbering. Asbestos Analytical Testing Methods All collected bulk satnples were analyzed by Polarized Light Microscopy(PL1V�for asbestos content in accordance with guidelines established by the EPA's Method for the Determination of Asbestos in Bulk Building Materials,inethod EPA/600/R-93/116, p�p��� . AGW recommends that a11 renovation work which rnay impact these materials be conducted by a licensed abatement contractor,utilizing certified asbestos abatement workers and adequate control measures in compliance with Federal asbestos regulations {29 CFR 1926.1101 - CSass I, II, and/or III Asbestos Work) and CDPHE Air Quality Control Cominission's Regulation Number 8. AGW is available to provide project specifications,asbestos abatement oversight, andlor air sampling at the conclusion of the asbestos abatement,if requested. While AGW was able to sasnple a rope gasket which appeared to have been removed from an access door located in the cooking hood's exhaust duct, AGW could not deteriniz�e whether this gasket material was the same throughout the exhaust duct system. AGW recommends that,if the hood or the hood exhaust duct is to be removed, tlie gasket material be further evaluated. This can be performed once the drywall ceiling has been removed from the kitchen. AGW did not observe any significant moisture or mold impact on the accessible building materials in either the Wildflower Restaurant or Mickey's Piano Bar. If mold growth is encountered during the course of renovation activities, AGW is available to complete a supplemental evaluation, and provide remediation recommendations,if warranted. Drawing P1.1 of the permit set has a Kitchen Water Heater Worksheet included in its contents. The new maximum hourly hot water usage totals 191 GPH. At a temperature rise of 100 degrees F, 262,395 BTUH minimum output is required. This compares to the previous restaurant need of 470,645 BTUH minimum output to meet the demand of 343 GPH. Thus, it is evident the demand for hot water has decreased for this remodel project. The total hotel water usage varies from 1,000,000 to 2,000,000 gallons per month. The hotel has five (5)boilers that provide 120 degree water throughout the building. Each boiler has 2,000,000 BTiJH gas input and 1,700,000 BTUH output for a total capability of 8,500,000 BTUH output. There is an existing booster heater in the utility room below this referenced kitchen (which serves this kitchen)with a 13.5 kW heating element that raises the water temperature to 140 degrees F. The dishwasher specified for this project uses 69.8 GPH and has an integral 12 kW booster heater that raises the final rinse temperature of the water from 140 degrees F to 180 degrees F. A thermostatic mixing valve will be installed at the hand sinks and lavatories to provide 110 degree F water at these point-of-use locations.
