and emailed you on 92/18/09 without response to resolve this issue_
Please verify this work was or was not completad. If the work has been complmtsd we naad payment in full.
$57.87. If work was never performed we need to collect the plan check fee as we did our part In reviewing this
permit for approval. Ths plan' cheok fee is $21.22.
1 have also faxed the original application par your request to 970- 328 -7034.

and emailed you on 12/16/09 without response to resolve this issue.
Please verify this work was or was not completed. If the work has been completed we need payment in full,
$57.87, if work was never performed we need to collect the plan check fee as we did our part in reviewing the
permit for approval. The plan check fee is $21.22.
