Construction Activities/Observations: Field engineer arrived on site, as requested, to observe a proof-roll
conducted in the westbound lane of South Frontage Road from Sta.6+50 to Sta. 9+00.
The material observed was slightly silty sand with scattered cobbles. The proof-roll was performed using a fully loaded
water truck.
At the time of this observation, field engineer observed rutting and cracking of the subgrade in areas spauning 20 to 50
feet from approximately Sta. 1+00 to Sta. 8+50.
Based on our observations, it is our opinion that the subgrade has not been properly prepared and has failed to meet
project specifications.

compacting the subgrade for the curb and gutter on East Lionshead Circle on the west side from Sta. 0+00 to Sta.
4+00, on the east side from Sta. 0+00 to Sta. 4+00, and on the south side of Frontage Road from Sta. 6+25 to Sta.
8+50.
The material being compacted with a jumping jack was a slightly silty sand with scattered cobbles. Field engineer
probed the soil using a density pin. The material at the subgrade elevation was dense.
Based on our observations and probing using a densiry pin, it is our opinion that the subgrade for the curb and gutter on
East Lionshead Circle on the west side from Sta. 0+00 to Sta. 4+00, on the east side from Sta. 0+00 to Sta. 4+00,
and on the south side of Frontage Road from Sta. 6+25 to Sta. 8+50 has been properly prepared and has met project
specifications.

Construction Activities/Observations: Field engineer arrived on site, as requested, to observe the material exposed
in the exca�ation for the spread footing along Gridlines E-E to E.F/90' West of E.1.
The excavation varied in depth fram approximately 9 to 10 feet from the adjacent ground surface. The material.
exposed in the base of the excavation consisted of a natural,gray to light brown, slightly moist,dense, slightly silty sand
with scattered cobbles. Approximately 12 inches of ponded water was observed in the base of the excavation. The
soils surrounding the ponded water were soft. No organic materials were observed in the excavation.
The contractor excavated to approxirnately 18 inches in the area that had soft soils, placed geogrid, and then placed
and compacted 3/4" angular rock. The rock extended beyond forms at 1/1 (Horizontal to Vertical) slope. The rock
was compacted and was medium dense to dense.
Based on our observations, it is our opinion that the subgrade for the spread footing along Gridlines E E to E.F/90'
West of E.1 has been properly prepared and has met project specifications.

Construction Activities/Observations: Field engineer arrived on site, as requested, to observe a proof-roll of the
subgrade for the south side of the South Frontage Road between Sta. 4+40 to Sta. 5+59.
The proof-roll was performed using a fully loaded water truck. The soil between Sta. 5+10 and Sta. 5+59 (25' x 49')
was observed to be deflecting and pumping. Some deflection occurred between Sta. 4+87 and Sta. 4+96 (7' x 9.5')
along the south side. The aforementioned areas have failed to meet project specifications.
There was very slight to no deflection between Sta. 4+40 and Sta. 5+10. Excluding the 25' x 49' soft spot between
Sta. 5+10 and Sta. 5+59, and the T x 9.5' spot on the south side between Sta. 4+87 and Sta. 4+96, the subgrade has
been properly prepared and has met project specifications.

inch minus rock.
At the time of the observation, field engineer did not observe any pumping or rutting of the subgrade.
Based on our observations, it is our opinion that the subgrade on the South Frontage Road eastbound lane from Sta.
5+20 to Sta. 6+00 and on East Lionshead Circle from Sta. 0+00 to Sta. 1+50 (southbound lane) has been properly
prepared and has met project specifications.
