We have corrected the plans per the plan check comments.

Per initial discussions with Martin Haeberle and subsequent E-mail correspondence, the
three (3) accessible restrooms on the first floor of the building will fulfill the
requirements for accessible restrooms within the stipulated travel distance. The restroom
in the Arthrex suite does not need to provide an additional accessible restroom.
We have slightly relocated the drinking fountain and adjacent sink and have added the
ADAAG clearance diagram.
Although the bench to be provided in the locker room is movable, we have shown
clearance dimensions and diagram to show possible accessibility.
We have reversed the swing of door 4 to address the maneuvering clearance requirement.

1. Clarify that the existing DWV system is adequate to support the additional loads
proposed. Reference sections 710 and 916 of the IPC. We added schedules on P2.0
plan showing the new load. We verified existing plumbing plans and the new load will
not exceed d.f.u. of 4" waste line and therefore the existing waste line is adequate to
serve the new d.f.u.
2. Clarify that the existing water supply is adequate for the additional water fixtures
proposed. Reference section 604.3 IPC We added schedules on P1.0 and P2.0
showing total new fixture count and DHW calculation. Per the new f.u. calculation we
would need 1" CW line into the space. We verified in field that the remodeled space is
already served with existing 1" CW supply line and thus we are not increasing the load.
3. Clarify back flow protection device(s) to be used for hose bib connections. Reference
section 608 IPC We added a ca�l for installing BFP on proposed hose bibs in plumbing
fixture connection schedule remark, see P2.0 plan.

1. Please clarify occupancy classifications used to determine required outside air

D&D Environmental Consulting, Inc. (D&D) is pleased to provide the results of the walk-through and collection of
representative bulk samples of suspect asbestos containing drywall/drywall joint compound (mud) and ceiling tiles
that may be impacted by the interior renovation activities at the referenced site. The walk-through, collection of
representative bulk samples, and delivery of the bulk samples to an independent analytical laboratory were
completed on August 10, 2011 by a D&D Building Inspector, certified and accredited by the Environmental
Protection Agency (EPA) and the State of Colorado.
UNDERSTANDING & SCOPE OF WORK
D&D understands that a representative of J.L. Viele Construction was seeking a professional environmental
consulting company to conduct a walk-through to view and collect representative bulk samples of suspect asbestos
containing materials impacted by the interior renovations and submit the bulk samples to an independent analytical
laboratory for asbestos content analysis based on rush 2-hour turnaround time. This report of findings was prepared
based on the asbestos content report from the analytical laboratory.
ANALYTICAL PROCEDURES
The bulk samples collected of suspect asbestos containing materials were delivered to Aerobiology Laboratory
Associates, Inc. a National Voluntary Laboratory Accreditation Program (NVLAP) asbestos laboratory located in
Lakewood, Colorado for rush 2-hour turnaround time analysis. All bulk samples are archived for six-months unless
otherwise stipulated by the client.
According to the analytical laboratory, the bulk samples were analyzed in accordance with EPA Method 600/R
93/116. Small portions of the sample were placed in Series: E High Dispersion Refractive Index Liquid on a
microscope slide. The prepared samples were observed at 100X (power) under polarized light using a McCrone
Dispersion Staining Objective. The characteristics of the fibers were compazed to the known properties of asbestos
fibers for dispersion color, polarity, extinction and general morphology. Sample content (given by percentage) was
made by visual estimates by comparison of asbestos fibers to total materials.

RESULTS
The US Environmental Protection Agency (EPA), the Occupational Safety and Health Administration (OSHA), and
the State of Colorado Air Quality Control Commission considers a material as asbestos containing if the reported
amount of asbestos is greater than one-percent. According to the analytical report, the three-bulk samples of
drywalUjoint compound and the three-bulk samples of ceiling tiles were reported as none detected (ND) for asbestos
content. Copies of the chain of custody form and the analytical results are attached.
RECOMMENDATIONS AND SUNIMARY
Based on the analytical report, the drywall/drywall joint compound and the ceiling tiles will not require special
handling prior to commencing interior renovation activities. If additional suspect asbestos containing materials aze
identified during the demolition/renovations, all activities must stop until these materials aze sampled. Work shall
not resume until the results are reported and removal completed, if required.
