Mr.Tony Faulhaber JL Viele Construction,Inc 1000 S. Frontage Rd West#202 Vail, Colorado 81657 Attn: Mr. Faulhaber, This letter summarizes the materiais testing GROUND Engineering Consultants, Inc. (GROUND) performed during the improvements to the Golden Bear within the Sitzmark Lodge in Vail, Colorado. The materials testing services were performed at times scheduled by the Owner, Contractor and/or Subcontractors between October 21 and November 30,2011. A certified welding inspector (CWI) visited the project site on October 21, 2011 to observe the fieid welded connections. Based on the fied observations to date, RFI's received and/or project documentation received from the design team, it is our opinion that the observed structural steel field welded connections were in general accordance with project plans, specifications, and/or appiicable RFI's. The observations and materials testing as indicated above were performed periodically to provide data to the Owner, Contractor and Subcontractors to assist them in evaluating the general compliance of the materials and/or work with the contract documents at the times of our observation and testing. We did not supervise, direct or have any control over any of the Contractor's or Subcontractor's work. We were not responsible for the Contractor's or Subcontractor's scheduling of our services, means, methods, procedures, techniques, or sequences of construction, nor responsible for ensuring that any Contractor or Subcontractor performed or completed the work in accordance with the contract documents or construction industry standards. Viele Construction 1000 S.Frontage Rd., Suite 202 Vail,CO 81657 RE: Report of Findings—Limited Asbestos Renovation Survey For The Golden Bear, 183 Gore Creek Drive,Unit 3A Ciurently Wilderness Wonders Vail,Colorado D&D Project No. 110707 Dear Mr.Faulhaber: D&D Environmental Consulting, Ina (D&D)is pleased to provide the results of the walk-through and collection of representarive bulk samples of suspect asbestos containing drywall and drywall joint compound(mud)material that may be impacted by the interior renovation activities for the retail space located at 183 Gore Creek Drive,Unit 3A, Vail,Colorado. The walk-through,collection of representarive bulk samples, and delivery of the bulk samples to an independent analytical laboratory were completed on July 29, 2011 by a D&D Building Inspector, certified and accredited by the Environmental Protection Agency(EPA)and the State of Colorado. UNDERSTANDING& SCOPE OF WORK D&D understands that a representative of J.L. Viele Consiruction was seeking a professional environmental consulting company to conduct a walk-through to view and collect representative bulk samples of drywall and drywall joint compound material that will be impacted by the interior renovations and submit the bulk samples to an independent analytical laboratory for asbestos content analysis based on standard turnaround rime. Reportedly, the renovation activities will impact the drywall and drywall joint compound material only. This report of findings was prepared based on the asbestos content report from the analytical laboratory. ANALYTICAL PROCEDURES The bulk samples collected of suspect asbestos containing materials were delivered to Aerobiology Laboratory Associates, Inc.a National Voluntary Laboratory Accreditation Program(NVLAP)asbestos laboratory located in Lakewood,Colorado for standard turnaround time analysis. All bulk samples are archived for six-months unless otherwise stipulated by the client. According to the analytical laboratory, the bulk samples were analyzed in accordance with EPA Method 600/R- 93/116. Small portions of the sample were placed in Series: E High Dispersion Refractive Index Liquid on a microscope slide. The prepared samples were observed at 100X(power)under polarized light using a McCrone Dispersion Staining Objective. The characterisrics of the fibers were compared to the known properties of asbestos fibers for dispersion color, polarity, extinction and general morphology. Sample content(given by percentage)was made by visual estimates by comparison of asbestos fibers to total materials. Mr.Tony Faulhaber II Asbatos Rrnovauon Survey-183 Gore Crcek Dr. Vail.CO I 110707 j August 3,201 I Page 2 RESULTS The US Environmental Protection Agency(EPA),the Occupational Safety and Health Administration(OSHA), and the State of Colorado Air Quality Control Commission considers a material as asbestos containing if the reported amount of asbestos is greater than one-percent. According to the analytical report, the three-bulk samples of drywall/joint compound were reported as none detected(ND) for asbestos content.Copies of the chain of custody form and the analytical results are attached. RECOMMENDATIONS AND SUMMARY Based on the analytical report, the drywall/drywall joint compound will not require special handling prior to commencing renovation activities to the wall or ceiling surfaces. If additional suspect asbestos containing materials are identified during the demolition/renovations,all activities must stop until these materials are sampled. Work shall not resume until the results are reported and removal completed,if required. D&D's liability is limited to the amount billed for this project. D&D appreciates the opportunity to assist you with your asbestos sampling needs. If you have any questions regarding this report of findings, please do not hesitate to call me at the office(303)730-7877 or on my mobile phone(303)870-1032.
