General Information: This application is required for all proposals involving minor changes to buildings and site Improve-
ments, such as roofing, painting, window additions, landscaping, fences, retaining walls, etc. Applicable Vail Town
Code sections can be found at www,vallgov.com under Vail Information —Town Code Online_ All projects requiring de-
sign review must receive approval prior to submitting a building permit application. An application for Design Review
cannot be accepted until all required information is received by the Community Development Department, as outlined in
the submittal requirements. The project may also need to he reviewed by the Town Council acid /or the Planning and
Environmental Commission. Design review approval expires one year from the date of approval, unless a building per-
mit is issued and construction commences.

RE: Retaining wall replacement project at 425 Forest Rd.
SCOPE OF WORK: Remove existing 35 year old deteriorating timber retaining wall
And replace with a faced concrete block retaining wall system
(Versa -Lok). The "Versa -Lok" retaining wall system was used on
both Lots 4 & 6 14 years ago and has endured the stress of freeze/
thaw, high snow fall years, plowing ect. Please note; these blocks
are solid, not hollow.
FOREST RD.: (3) Tree's and (1) bush will have to be removed per our consulting
DRIVEWAY EDGE arborist Mark Steele of "Precision Tree Work's). It was
TREE REMOVAL determined that the damage to the root structure during
Construction will have a terminal impact on these tree's.
Relocation of these tree's was considered, however, with the
tree proximity to the driveway edge, approximately 40% of the
root structure is beneath the asphalt. Relocating the tree's will
have a terminal impact on the root structure.
TREE: (2) Blue spruce tree's (6" & 8 ") will be preserved at the top of
PRESERVATION forest Rd. by reconfiguring the West end of the new retaining wall.
TREE: (4) 10' Blue spruce tree's will be planted along the Forest Rd.
REPLACEMENT driveway edge. The new tree's will be placed by Mark Steele
of "Precision Tree Work's ". Replacement tree planting will take
place Spring 2012 per the recommendation from Mark.
PROJECT TIME: Upon approval by Community Development: Tree removal
LINE (1) day; Existing timber wall demolition and disposal (2) days;
Compacted structural footing (road base) (2) days; Wall
construction (4) days; Final grading, erosion matt placement
and clean-up (3) days. The conservative time estimate for project
start to completion is (14) day's weather pending and crew time
off.

PROJECT: The existing timber retaining wall will not survive the 2012
URGENCY run -off.
