HomeMy WebLinkAboutUtility Plan, issue.Vail Mountain View on Gore Creek Utilities Plan October 15, 2008 There was a gas line broken at this project today by the earthwork and utilities subcontractor WVC. This is the forth time utilities have been damaged by this subcontractor on this project. Three times were gas, and once was electric. The project was evacuated and emergency crews responded and repaired the line. Also during the incident an excavation was observed that was of a depth that required a trench box. This excavation was for the completion of the water main and installation of a permanent fire hydrant to replace the temporary hydrant on the east side of the project. While no one was working in the excavation at the time, there was evidence of recent (same day) water line work, and no trench box or ladder was present. This excavation requires a trench box for work to continue and due to wet soils, backfill as soon as possible is important. The gas line that was struck is a temporary gas line installed by Arby Inc., a subcontractor for Excel Energy. This line runs along the South, East, and West sides of the project. For the past week Excel and RA Nelson had been working to determine if this line could be used for the permanent gas line. The tracer wire had been compromised and WVC over the past 3 days had been in the process of potholing to find and repair tracer wires, and to confirm the location of the gas line. Based on the condition of the tracer wire and the pipeline, and the lack of adequate bedding material, it was determined that the gas line may have been compromised and it was decided that it would be prudent to replace the South portion of the line. The west portion had already been replaced, and the east run needed to be replaced due to conflicts with a sewer main. RA Nelson's Safety Director is conducting an investigation and will be issuing a formal report today. OSHA responded to a complaint and was onsite 10/15 and again 10/16. WVC has been put on formal notice that they are in default under the terms of their subcontract and in breach of RA Nelson's safety plan. WVC personnel worked with RA Nelson supervisors and management to develop the enclosed plan for completing the work. We believe that with the appropriate change in attitude by WVC, the project is best served by allowing them to complete their contract, but only if the appropriate changes are made. The completion of the utility work on this project is complicated and requires careful coordination and planning. The remaining work consists of: 1. Install 2 fire hydrants, one at the top of the driveway and one near the location of the temporary fire hydrant on the east end. 2. Clean flush and flow test the water mains 3. Install the sanitary sewer along the east end of the project The sanitary sewer to be installed and the temporary gas main on the east end of the site are in the same area. The permanent gas main runs parallel to the building and out of the alignment of the new sewer service. We believe that the most prudent sequence of work is to get the permanent gas main installed ASAP and abandon the temporary line. This will enable us to install the deep sewer without endangering the temporary gas line and will take a potentially compromised gas line out of service. We believe that the best solution to the water hydrant excavation is to rapidly complete this work and get the excavation filled in. The trench box has been installed and the excavation has been clearly marked with caution tape. RA Nelson's excavation permit process will be used for all remaining work on this project. This is a daily pre - planning and communication tool that should prevent future problems and provide accountability for proper planning and coordination. Mike Cuthbertson Vail Valley Regional Manager Vail Mountain View Utility action plan As we are all aware we have encountered several issues of concern at the Mountain View project including the gas line break which occurred today 10- 15 -08. In order to mitigate any further incidents RA Nelson will propose the following plan and process for the remainder of the utilities on this project. Below I have outlined this process for your review and approval. WATER LINE 1. Currently the water line on the east side has been excavated and is ready for the fire hydrant installation. The trench box has been installed and RA Nelson and WVC will insure this work is performed to all the required safety measures and OSHA standards. 2. It is our plan to install the hydrant on 10 -16 -08 including the required thrust blocks and backfill this area after your inspection of this work. 3. Upon completion of this work WVC will proceed with the excavation and installation of the fire hydrant at the drive entrance to the garage. 4. The excavation in this area will be approximately 8' deep so the trench box will again be utilized and the installation will take place on 10 -16 -08 and 10- 17 -08. 5. Upon completion of this hydrant and thrust block we will again backfill this area after your inspection and approval of this installation. 6. This will complete the water line installation and we will then start plans for the clean and flush of the water mains and will coordinate this work with the Fire Dept. GAS LINE 1. The existing temp gas service was cut today while attempting to install the new gas service above the existing line. In order to properly coordinate this work it is necessary to install the permanent line in its proper location prior to the excavation and installation of the sewer line. 2. The progress as of today is as follows. The west tie in has been hand excavated and the trench along the south side of the building is excavated. In order to continue with this work we will need to complete the following work. 3. WVC will need to excavate from the exposed break around the corner to the north following the building and crossing the site approximately 50' to the north. 4. At the south east corner of the site the final grade has not been established due to missing walls. At this location the exposed pipe will be sleeved with schedule 40 pipe and protected with barricades from damage until the final grading is achieved. This will take approximately 2 weeks to complete the final grade in this area 5. By installing the new line now and before the new sewer is installed the benefits and safety mitigation are as follows. 6. Currently the temp gas service is directly over the sewer service excavation. By moving this line over now we will not have to encounter this line until we cross it at the new tie in. 7. At the cross over location we will leave this area partially exposed and covered with road base. In this way we can easily identify the cross over location during the sewer line excavation. 8. If we are allowed to continue this work on 10 -16 -08 we can have this work installed and backfilled in 2 days. SEWER SERVICE LINE 1. The third and last line to install is the sewer service line. This line runs from the south east side of the site to the north along the project. 2. This excavation will be approximately 16' deep and will require a trench box for this excavation and installation. 3. With the gas line installed shallow to the west we will only need to cross this line perpendicular to at the tie in location. 4. Currently if we are allowed to proceed with this plan we will be prepared to start the excavation and installing of the sewer service on Monday and will take approximately 4 days to complete this work. In closing RA Nelson will insure proper supervision and safety enforcement of this work at all times during this process. Any construction deviations or changes to this plan will not take place without prior approval of the TOV and / Vail Fire Prevention District. WVC holds a Town of Vail On approach to the site of the gas line cut, I observed a 8 - 9 ft deep hole in and around work involving a fire hydrant. The hole was cut 1:1, wet, and tools were observed in the bottom of the hole. Frontage Rd. E. Arrived to hear from job supervisors that a two inch flexible line had been cut. ETA from Excell was relayed to E 2 as up to 3 hours due to a problem with the responding vehicle. Evacuated job site of approximate 70 workers and staged them at Ford Park. Confirmed utilities to be shut off and made entry into affected area. Condition report revealed the possibility of mitigating the break utilizing fire department clamps. Establish command location, staging and called for additional resources for RIT and possible Rehab. Sent four firefighters and a supervisor on air into the area for clamping procedures. RIT and Safety issues were addressed and gas was shut off at the break within 30 minutes. Sent mitigation group to Rehab and created Security group from RIT team.
