Dear Mr. Haeberle,
We respectfully request that the TCO for permit B12 -0029 be extended to July 14, 2014. The
landscaping is nearing completion and all other open items are being addressed. As you know,
we took on the task of completing this project, but could not have anticipated the extent of
work remaining. I apologize for not receiving a CO prior to the TCO expiration. We have
worked continuously since receiving this permit and will not stop until a CO is acquired. The

The purpose of this letter is to inform you that the existing chimney flues located at 660
Lionshead Place are hereby ordered to be abated per section 401.2 of the Uniform Abatement
Code 1997 edition as adopted by the Town of Vail. As defined in section 302(9) of the Uniform
Abatement Code 97 edition. `Whenever, for any reason, the building or structure, or any portion
thereof is manifestly unsafe for the purpose for which it is being used, "this building is
considered a dangerous building as previously defined.
This abatement notice has been ordered based on an engineers report filed with this office and
upon a field observation by the Chief Building Official.
All repairs, work, alterations to the chimney flues and associated appliances shall be done via a
new building permit or expansion of the scope of work of the existing building permit.
Action shall be taken within 30 days of the receipt of this letter in order to stay further action as
allowed by the Uniform Abatement Code 97 edition..

This letter summarizes the special inspection services GROUND Engineering Consultants, Inc.
(GROUND) performed during the improvements to the residence at 45 Forest Road located in Vail,
Colorado. The special inspection and materials testing services were performed at times scheduled by
the Owner, Contractor and /or Subcontractors between December 19"', 2011 and August 14`h, 2012
and according to GROUND's proposal # 1112 -1841 dated December 19th, 2011.
During construction, representatives of our office were present on a part time basis to periodically
observe the open -hole /foundation subgrade soils and foundation perimeter drain installation.
Representatives of our office were periodically on -site during the installation of the permanent
walls /retaining walls, including underpinning and the installation of soil nails, reinforcing steel and wire
mesh and shotcrete placements. Samples of the grout and shotcrete were periodically tested to
determine physical properties and compressive strength. THESE OBSERVATIONS AND TESTING
RESULTS WERE SUBMITTED TO THE WALL DESIGNER, COGGINS AND SONS, FOR FINAL
APPROVAL.
Representatives of our office periodically observed epoxied doweled reinforcing steel placement and
performed periodic testing on the structural concrete placed for the foundation, foundation walls,
retaining walls, slabs on deck and other structural elements. Samples of the structural concrete were
periodically tested to determine physical properties and compressive strength.
A certified welding inspector (CWI) visited the project site on a part-time basis, when scheduled by the
Contractor and /or Owner, to observe the structural steel field welded and bolted connections, as well
as metal deck welded connections. Fabrication shop inspections were also performed.

Based on the field observations to date, RFI's received and /or project documentation received from
the design team, it is our opinion that the materials tested and /or observed for the foundation soils,
foundation perimeter drain, reinforced structural concrete, and structural steel were in general
accordance with project plans, specifications, and /or applicable RFI's.
The observations and materials testing as indicated above were performed periodically to provide data
to the Owner, Contractor and Subcontractors to assist them in evaluating the general compliance of
the materials and /or work with the contract documents at the times of our observation and testing. We
did not supervise, direct or have any control over any of the Contractor's or Subcontractor's work. We
were not responsible for the Contractor's or Subcontractor's scheduling of our services, means,
methods, procedures, techniques, or sequences of construction, nor responsible for ensuring that any
Contractor or Subcontractor performed or completed the work in accordance with the contract
documents or construction industry standards.

This letter summarizes the materials testing GROUND Engineering Consultants, Inc.
(GROUND) performed during the renovation of the Lion Square Lodge East project
located in Vail, Colorado. The special inspection and materials testing services were
performed at times scheduled by the Owner, Contractor and /or Subcontractors between
May 16, 2012 and February 7, 2013.
During construction, representatives of our office were present on a part time basis to
periodically observe the placement of backfill for the foundation soils, footings,
foundation walls, floor slab and other structural fills, as well as for the waterline trench.
Nuclear- moisture density testing was periodically performed on the processed soils.
Representatives of our office periodically observed drilled and epoxied reinforcing steel
placement and performed periodic testing on the structural concrete placed for the
foundation, foundation walls and other structural elements. Samples of the structural
concrete were periodically tested to determine physical properties and compressive
strength.
Representatives of our office periodically observed reinforcing steel placement and
grouting operations as scheduled by the Owner, Contractor or Subcontractor during
masonry operations. At the time of our visits, representatives of our office observed the
placement of sections of reinforcing steel and the general construction of the masonry
walls. Mortar, grout, and masonry prism specimens were cast periodically during
construction. Samples of the mortar, grout and masonry prisms specimens were tested
for compressive strength.
Structural steel welded and bolted connections were observed during construction by a
certified welding inspector (CWI). These observations were completed at times
scheduled by the Owner, Contractor or Subcontractor.
The spray applied fireproofing (SFRM) was periodically observed for coverage and
measured for thickness.

Based on the final test results to date, field observations, RFI's received and other
project documentation received from the design team, it is our opinion that the materials
tested and /or observed for the foundation soils, structural concrete, reinforcing steel,
masonry, structural steel and spray applied fireproofing were in general accordance with
project plans, specifications, or applicable RFI's.
The observations and materials testing as indicated above were performed periodically
to provide data to the Owner, Contractor and Subcontractors to assist them in evaluating
the general compliance of the tested materials and /or work with the contract documents
at the times of our observation and testing. We did not supervise, direct or have any
control over any of the Contractor's or Subcontractor's work. We were not responsible
for the Contractors or Subcontractor's scheduling of our services, means, methods,
procedures, techniques, or sequences of construction, nor responsible for ensuring that
any Contractor or Subcontractor performed or completed the work in accordance with
the contract documents or construction industry standards.
