This letter surnmarizes the special inspections GROUND Engineering Consuitants, Inc. (GROUND)
performed for 1476 West Haven Circle #17 located in Vail, Colorado. The special inspection services
were perforrned at times scheduled by the Contractor and/or Subcontractors on May 2"d, 2012.
Representatives of our office provided periodic observations for the proposed addition foundation
walls as scheduled by the Owner, Contractor or Subcontractor. At the time of our visit, representatives
of our office observed the piacement of epoxied dowel reinforcements for the proposed addition
foundation wall.
Based on the final test results to date, field observations, and other project documentation received
from the design team, it is our opinion that the materials observed for the epoxied dowel reinforcing for
the addition foundation walls were in general accordance with project plans, specifications, or
applicable RFI's.
The observations as indicated above were performed periodically to provide data to the Contractor
and Subcontractors to assist them in evaluating the general cor�ipliance of the tested materials andlor
work with the contract documents at the times of our observation. We did not supervise, direct or have
any control over any of the Contractor's or Subcontractor's work. We were not responsible for the
Contractor's or Subcontractor's scheduling of our services, means, methods, procedures, techniques,
or sequences of construction, nor responsible for ensuring that any Contractor or Subcontractor
performed or completed the work in accordance with the contract documents or construction industry

Here are the square footages for Coldstream #17. They reflect the new square footages after the addition and
are straight from the architect's office:
• Level 1 = 1196 GSF (including garage)
• Level 2 = 1183 GSF
• Leve13 = 1146 GSF
• TOTAL = 3525 GSF (including garage)
For electrical squarefootage please use 50 percent of the above numbers:
• TOTAL ELECTRICAL = 1762 (3525 /2)
Hope this makes sense. I will send you the asbestos report as well as soon as I get the PDF inaker to work. We
just moved in a new office space and are waiting for permanent network etc...

PROJECT OVERVIEW
1.0 Introduction �
On March 22, 2012, Ms. Edie Stevenson with DS ConsulCing, Inc. (DSC) conducted a limited asbestos
inspection and collected asbestos bullc-samples of suspect asbestos-containi►lg matel-i�ls (ACM) �vitl�itl
Unit 17 of the Coldstream condos located at 1476 West Haven Dr, Vail, Color-ado.
The purpose of the limited inspection was to identify and sarnple potentially hazardous friable and non-
friable ACM ttlat may be itnpacted by upcoming construction/ retnodeling activities.
2.0 Scope of Work
The scope of the limited inspection and bulk-sa►npling was limited to specific areas of the residence
defined by the construction company. These areas included the lower level bathroom and office. The

the scope of the inspection. The limited asbestos inspection did not constitute a full building inspection
and does not fulfill the asbestos inspection requirements for structures that are to be demolished.
3.0 Site Description
The condo is a �vood-framed, single-family residence within a larger t�uilding.
4.0 Certifications
The limited asbestos inspection and bulk-sampling was conducted by Ms. Edie Stevenson �vith DSC. DSC
is a Colorado Department of Public Health and Environment (CDPHE) certified Asbestos Consulting Firm,
RegistraCion Na 14912. Ms. Stevenson is a CDPHE certified Buildi�lg Inspectoi; having certification
nurnber 18406 (seeApper��clixA �orcertificntes).
5.0 Inspection, Sampling & Analytical Procedures
5.1 Inspection Procedures
The limited asbestos inspection was conducted by an Environnlental Pi-otection Agency (EPA) and
CDPHE certified Building Inspector. The inspection procedures included identifying and sampling
suspect ACM within the pre-defined areas, submitting samples to an accredited laboratory for analysis,
classifying the materials and assessing their condition, and compiling a final report detailing the
inspection and the analytica] results of the bulk-samples.
5.2 Sampling Procedures
Statistically random bul}<-samples representative of the suspect ACM of each homogeneous area were
collected according to the guidelines published as EPA Final Rule: Title [f of the Toxic Substances
Control Act (TSCA), 15 USC, Sections 2641 through 2654 and in compliance with 40 CFR, Part 763 and
CDPHE Regulation Number 8, Part B - Asbestos (Reg. 8).
DSC has collected the appropriate number of bul]<-samples to meet all regulatory requirements for the
classification and quantity of each homogei�eous area. Some minor destructive sampling �vas
conducted; however, walls, colurnns and perimeter pipe chases were not broken into in order- to locate
�
and quantify suspect ACM. ft should be noted that additional ACM mig}lt be (ocated in these and other
inaccessible areas.
Materials containing less than 1% asbestos are not regufated by CUPHE Regulation 8, Part B -Asbestos. I,
Howevei•, all demolition/abate«�ent activities should be perfo►•med following tlie applicable
Occupational Safety ancj Health Administratiot� (OSHA) regulations. This would include, but not limited �
to, the appropriate asbestos training for the type of material being removed/disturbed as well as having
a properly trained supervisor onsite, using wet re«�oval methods, wearing adequate personal
protective equipment (HEPA-filtered particulate i•espirators), medical surveillance of worl:ers,
personal-exposuce air monitoring, area air monitoring in occupied buildings, etc. There may also be
landfili disposal requirements for these materials, depending on the facility. DSC recommends that all
demolition/renovation areas involving any amount of asbestos be subjected to visual inspections and a
final clearance air testing by a CDPHE-certified Air Monitoring Specialist (AMS) after the work has been
completed, but before any containrnents are dismantled and the at�ea is reoccupied.
5.3 Analytical Procedui-es
All asbestos bulk-samples were analyzed by a National Voluntary Laboratory Accreditation Program
(NVLAP) accredited laboratory via Polarized I,ight Microscopy (PI.M) for asbestos content (see
Appendix B for lcrborotory r�e�ort).
'I'he percentage of asbestos within each individual bulk-sample can vary ciepending on sample location,
homogeneity oF the rnaterial, and the type of application. Any sample reporting a °TRACE" arnount of
asbestos must be considered positive for asbestos greater tha�� 1% unless it is re-analyzed utilizing the
point-count method and verified to be less than 1%,.
6.0 Homogeneous Areas
A Homogeneous Area (HA) includes inaterials th�t are uniforii� in appeara�zce, color, texture anci date of
application. The asUestos content of the bull<-samples collected within a homogeneous area can be
applied to the entire homogenous area if they conform to the above characteristics and the regulated
minimum sample quantities of each type of material are collected and analyzed.
6.1 Material Friability
A material can either be fricrbte oi• non frinUle. A friable material is one rhat, when di-y, can be
pulverized, or reduced to powdel- by hand pressui-e, a non-friable rilaterial cannot. A non-friable
material may become friable iFits condition had deterioratec� or has been impacted by forces that h�ve
rendered it friable.
6.2 Material C►assifications
S�iYlpled i��atei�ials are divided into one of the follo�ving three categories:
• Surfacing M�terial: sprayed or troweled onto structural building members
• Thermal System Insulation (TS/): any rype of pipe, boiler, tank, or duct insulation
• Miscellaneous Material: all other►7�aterials not classified in the above two categories
6.3 Material Conditions
Sampled tt�aterials are placed into one of the three following categories of conditions:
� Good: none to very little visible damage or deterioration

• Damaged: the surface is crucnbling, blistered, water-stained, gouged, marred or otherwise abraded
over less than one-tenth of the surface if the damage is evenly distributed, or one-quarter if the
damage is localized
• Significantly Damcrged: the surface is crumbling, blistered, water-stained, go«ged, marred or
otherwise abraded over greater than one-tenth of the surface if the damage is evenly distributed,
or one-quarter if the damage is localized
6.4 Sainple Quantities
DSC collected at least the minimum number of samples fi-orn each homogeneous area necessary to meet
al( regi�latory requirements for the quantity of material to be disturbed. The quantities listed in this
report are approximate and on-site verification of the exact quantity of each material is required. The
following outlines the �Y�inimui�� sample quantities required per homogeneous area:
• Su�facing Matericrls: up to 1,000 ft� of material requires a minimum of three (3) sarnples; l�eeween
1,000 ft�� �nd 5,OQ0 ft� of material requil•es a minirnum of tive (5) sair����les; over 5,000 Ft� of
material requires a miniir�ui�� of seven (7) samples; one (1) sample of each patch
• Thermal System Insulation (TSI): each homogeneous ai�ea ►•equires a minimum of three (3)
samples; at least one (1) sample must be collected fro»� each patch; and collect enough samples
suFficient to adequately assess the material and determine the asbestos content for TS1 fittings
such as pipe elbows or T's.
• Miscellaneous Materinls: collect enough samples sufficient to determine the asbestos content
7.0 Overview of Findings
Asbestos was not reported within any of the bulk-samples collected and analyzed. DSC collected a
total of three (3) asbestos bulk-sa►nples of one (1) homoget�eous area.
Tn�le 1 below describes the materials composing each homogeneous area as well as the locations of each
bulk-sample collected. Also listed is the classification, condition, friaUility anc� estimated quantity of
material to be removed and/or disturbed, as well as the asbestos content within each bulk-sample.

8.0 Conclusion & Recommendations
ACM was not ide��tified within the areas of d�e residence unit that wei-e within the scope of the limited
inspection and bulk-sampling perfori��ed on March 22, "L012; therefore, no professional r�batement
activities are required to ►-emove or disturb the above-i�eferenced s�mpled �iaaterials.
9.0 Asbestos Abatement & Demolition Requirements
If ACM is to be removed or disturbed in a single-family residence, and the total quantity exceeds any of
the regulatory trigger levels of 50 linear ft. on pipes, 32 ft- on other surfaces, or the volume ec�uivalent of
a 55-gallon d►-um, a CDPHE-certified General Abatement Contractor (GAC) is i-equired to perForm the
work. The regulato►y trigger levels within a co►nmercial building are 260 linear ft. on pipes, 160 ft�' on
other surfaces, or the volume ec�uivalent of a 55-gallon drum. In addition, formal ilotification to CDPHE
prior to the abatement of ACM as well as air ti�onitoring, visual inspections, and final air clearances by a
CDPHE-certified Asbestos AMS is required. DSC can pi•ovide the client or building owner with a propos�al
for project design, abatement ovei-sigllt and air►nonitoi•ing upon ren�iest.
CDPHE regulations allow for the de►t�olition of a building that contains non-friable asbestos-containing
materials, such as caulking, rnastic or resilient ffoor tiles. However, dernolition must be completed
without causing the non-friable ACM to be rende��ed friable. Surning a building with any ACM is
prohibited. Opel•ations such as sanding, cutting, crushing, g►•inding, pneurnatic jacking, etc. of ACiVt are
noc permitted. Recycling oF buildin� materials such as concrete, r»etal, or wood that are bonded or
coi�taminated �vith ACM, e.g. glue, caull<ing, or mastic is also pi-ol�ibited. If any of the non-friable asbestos
containing materiais a►•e to be recycled a►Id rendered fr-iable after demolition (i.e. ceushiilg mastic-coated
concrete), these rnaterials must be abated of all ACM prior to shipping offsite for recycling.
OSHA re�ulations rega►-ding occupatio�lal exposu►-e during demolition activities is still m�ndatory. OSHA
29 CFR 1926.1101 rec�uires that workers performing construction-relaCed activities be protected from
asbestos fibers in excess of the permissible exposure limit of 0.1 f/cc of air. Contractors ace must comply
with applicable provisions of OSHA 29 CFR 1926.1101 during demolition and renov�tion activities.
These OSHA provisions include, but are not limited to, PPE and respirators, personnel training, person�l-
exposure air monitoring, employee medical su►-veillance, wet removal tnethods, signage for regulated
areas, etc.
10.0 Major Asbestos Spill Response
If ACM is significantly dam�ged and the total quantity exceeds the regulatory trigger levels, the area is
deemed a "Major Asbestos Spill.° The area is conseguently subject to the requirements in Reg. 8, Section
II1.T.1. - MajorAsbestos Spr!/s, as o�rtlined below. Additional asbestos air or dust sampling should also be
conducted within the remaining areas not directly impacted by the Major Asbestos Spill to deter«�ine iF
asbestos-containing dust/debris has spread to adjacent areas. ]f �sbestos fibers are found within any
othei• areas or oi� buifding contents, they sl�ould be included in the scope of�rofessional abatement and
decontamination. The following response actions must be followed per Colorado Reg. 8 wl�en a Major
Asbestos Spill occ��rs:
• Restrict access to the area and �ost warniiig signs to }�reveilt enti-y to the area by persons other
than those necess�ry to respond to the incident.
• Shut off oi- temporarily modify the air handling systeiY� to pr•event tkle disti�ibution of asbestos
fibers to other areas.
• [mmediately contacC t�he Division by telephone, submit a notification in compliance �Nith
sut�section III.E. (Notifications) and, if in an area of public access, apply for a permit in accordance
with subsection III.G. (Permits).
• Be exe►npted from the requirements to have a certified Supervisor on-site at all times, until such
time as the imrnediate danger has passed. Any cleanup or asbestos abatement that must occ�n-
after the immedi�te danger has passed shall be supervised by a person certified by the Division.
• Using certified Supervisors and certified Workers in accordance with section IL (Certification
Requirernents) of this Regulation, seal all openings between the contaminated and
uncontaminated areas and establish none-detect air pressur•e within the contaminated area in
accordance with paragrapl� ]II.J. (Air Cleaning and None-detect Pressure Requirements). This is to
be accomplished using polyethylene sheeting to cover areas such as doorways, windows, elevator
openings, corridor entrances, grills, drai�ls, g►-ates, diffusers �nd skyliKhts.
• HEPA vacuum or steam clean �Il carpets, drapes, uph��lstery, and other non-clothing fabrics in the
contaminated area, or discard these materials.
• Latu�der oi•discard contaminated clothing in accordance �vith subsection III.R. (Waste Haildling).
• HEPA vacuum or wet clean all su►-faces in the contamin�ted area.
• Discard all materials in accordance with subsec[ion I(I.R. (Waste Handling).
• Following completion of subparagraph Ill.'I'.l.a. through III.T.l.i. above, comply with air
mor�itoring requirements as described in subsection 1[I.P. (Clearing AbaCernent Projects); air
samples shall be collected aggressively as described in 40 C.F.R. Part 763, Appendi� A to Subpart
E (EPA 1995), except th�t the air stream of the leaf blower shall not be directed at any friable ACM
that remains in the area and
• Comply with any other measures deemed necessary by the Division to protect public healdl.

Gentlemen:
As requested, a representative of Hepworth-Pawiak Geotechnical, Inc. observed the
excavation at the subject site on April 30, 20l 2 to evaluate the soils exposed far �
foundation support. The findings of our observations and recommendations for the
foundation design are presented in this report. The services were performed in
accordance with our agreement for professional engineering services to Ulf&Associates,
dated April 27, 2012.
The addition will be attached to the north side of Unit # 17. Spread footings placed on
the natural granular soiis and sized for an allowable bearing pressure of 3,000 psf were
assumed for the building support.
At the time of our visit to the site,the footing trench excavation had been cut in one level
from 3%z to 4 feet below the adjacent ground surface. The bottom ofthe existing footing
of the unit was exposed on the west side of the excavation. The soils exposed in the
bottom of the excavation consisted ofrelatively dense, silty clayey sandy gravel with
cobbles. T`he results of a gradation analysis performed on a sample of the gravel (minus 5
inch fruction)obtaincd from the site are presented on Figure 1. No free water wa�
encountered in the excavation and the soils were slightty moist to moist. r
Considering the conditions exposed in the excavation and the nature of the proposed
construction, spread footings placed on the undisturbed natural gavel soil designed for an
allowable soil bearing pressure of 3,000 psf should be adequate for support of the
proposed addition. We expect post-construction footing settlements to be relatively
minor. Footings should be a minimum width of 16 inches for continuous walls and 2 feet
for cotumns. Loose disturbed soils should be removed and the bearing level extended
down to the undisturbed natural soils. Exterior footings should be provided with adequate
soil cover above their bearing elevations for frost protection. Continuous foundation
walls should be reinforced top and bottom to span local anomalies such as by assuming
an unsupported length of at least 10 feet. Foundation walls acting as retaining structures

(if any)should also be designed to resist a]ateral earth pressure based on an equivalent
fluid unit weight of al least 45 pcf for on-site soil as backfill. Structural fill placed within
floor slab areas can consist of the on-site soils compacted to at least 95%of standard
Proctor density at a moisture content near optimum. Backfill placed around the structwe
should be compacted and the surface graded to prevent ponding within at least l0 feet of
the building.
The recommendations submitted in this letter are based on our observation of the soils
exposed within the foundation excavation and do not include subsurface exploration to
evaluate the subsurface conditions within the loaded depth of foundation influence. This
study is based on the assumption that soils beneath the footings have eyual or better �
support than those exposed. The risk of foundation movement may be greater than
indicated in this report because ofpossible variations in the subsurface conditions. In
order to reveal the nature and extent of variations in the subsurface conditions below the
excavation, drilling would be required. 1t is possible the data obtained by subsurface
exploration couid change the recommendations contained in this letter. Our services do
not include determining the presence,prevention or possibility of mold or other biological ;
contaminants(MOBC)developing in the future. Ifthe client is concerned about MOBC, �
then a professional in this special field of practice should be consulted.
