If this permit requires a Town of Vail Fire Depaftmenc Approval, Engineert (Public Works)
review and approval, a Planning Department review or Health Department review, and a review
by the Building Department, the estimated time for a total review will take as long as three (3)
weeks.
All commercial (large or small) and all multi-family permits will have to follow the above
mentioned maximum requirements. Residential and small pro;ects should take a lesser amount
of time. However, if residential or smaller projecG impact the various above mentioned
departments with regard to necessary review, these projects may also take three (3) weeks to
review and approve.
E,rery attempt will be made by this department to expedite this permit as soon as possible.

PUBLIC WORKS AND THE PUBTIC WAY PERMIT PROCESS
How it relates to Building Permits:
Fill out the attached check list with the Buildinq Permit Apllication.
If yes was answe!'ed to any of the questions then a "Public Way" permit is required. You
can pick up an application at either Community Development, located at 75 South Frontage
Road or Public Works. located at 1309 Elkhorn Drive.
- Notice sign-offs for utility companies. ALL utilities must fielc verify (locate) respective
utilities prior to signing application. Some utility companies require up to 48 hours notice lo
schedule a locate.
A construction traffic control/staging plan must be prepared on a separate sheet of paper.
An approved site plan may also be used. This plan will show locations of all traffic control
devices (signs, cones, etc.) and the work zone, (area of construction, staging, etc.). This
plan will expire on November 1st and will need to be resubmitted for consideration for
approval through the winter. Be aware that your resubmission for winter may be denied
depending on the location of construction.
Sketch of work being performed must be submitted indicating dimensions (length, width
and depth of work). This may be crawn on the traffic control plan or a site plan for the job.
'. Submit compietec application to the Public Work's office for review. If required, locates will
be scheduled for the Town of Vail electricians and irrigation crew. The locates take place in
the morning, but may require up to 48 hours to perform.
The Public Work's Construction Inspector will review the application and approve or deny
the permit. You will be contacted as to the status and any requirements that may le
needed. Most permits are released within 48 hours of being received, but please allow up
to one (1) week to process.
As soon as the permit is approved, the Building Department will be notified, allowing the
"Building Permit" to be released. Please do nct confuse the "Public Way Permit" with a
"Building Permit".

Building Department has developed the following procedures to
construction sites have adequately established proper drainage
along and adjacent to Town of Vail roads or streets.
/ The Town of Vail Public Works Department will be required to inspect and approve
drainage adjacent to Town of Vail roads or streets and the installation of temporary
or permanent culvefts at access points from the road or street onto the construction
site. Such approval must be obtained prior to any requests for inspection by the
Town of Vail Building Department for footings, temporary electrical or any other
inspection. Please call Leonard Sandoval at479-2798 to request an inspection from
the Public Works Department. Allow a minimum of 24 hour notice.
Also, the Town of Vail Public Works Department will be approving all final drainage
and culveft installation with resulting road patching as necessary. Such approval
must be obtained prior to any Final Certificate of Occupancv issuance.

MATERIAL STORAGE AND CONSTRUCTION PARKING

CODE 5-2-10: DEPOSITS ON PUBLICWAYS PROHIBTTED
Unlawful deposits: Subject to subsection C thereof, it is unlawful for any person to litter, track or
deposit, or cause to )e littered, tracked or deposited, sand, gravel, rocks, mud, dirt, snow, ice, or any
other debris or material upon any street, sidewalk, alley or public place, or any portion thereof.
Notice; Abatement: The Director of ,rublic Works may notify and require any person who violates or
causes another to violate the provision of subsection A hereof, or who has in the Director's employment
a person who violates or causes another to violate the same, top remove such sand, gravel, rocks, mud,
dirt, snow, ice or any other debris or material within twenty four (24) hours after receipt of said notice
by the Director of Public Works. In the event the person so notified does not comply with the notice
within the perioo of time herein spec'fied, the Director of Public Works, or other authorized agent, may
cause any such sand, gravel, rocks, mud, dirt, snow, ice, debris or any other material to be removed
from any street or alley at the expense of the notified.
Summons and P€nalty: As an alternative to the notice for removal provided in subsection B above,
any person who violates or causes another to violate the same, may be issued a summons to appear
before the Municipal Court of the Town l'or said violations, and upon being found guilty of a violation
hereunder be punished as provided in Section 1-4-1 of this code.
D Notice and Penalty: It is unlawful for any person to fail or refuse to comply with the notice of the
Director of Public Works as provided in subsection B hereof, and any such person shall, in addition to
payment of the expense of removal incurred by the Director of Public Works, as provided in subsection B
hereof, upon being found guilty of a violation hereunder, be punishable as provided in Section 1-4-1 of
this Code. (1997 Code: Oroinance 6 (1979).
CODES 7-3A-T AND 7-3A-3: PARKING OBSTRUCTING TRAFFIC & IMPOUNDMENT AUTHORIZED
a No person shall park any vehicle upon a street or at any other place within this Municipality in such a
manner or under such conditions as to inteffere with the free movement of vehicular traffic or proper
streetor highway maintenance. (Ord. 2(1968) 5 1)
d Whenever any police officer finds a vehicle attended or unattended, standing upon any portion of a
street or upon any place within this Municipality in such a manner as to constitute a violation of any
section of this Article, or left unattended for a period of tvventy four (24) hours or more and presumed to
be abandoned under the conditions prescribed by Colorado Revised Statutes section 42-4-1102, as
amended, the officer shall require the vehicle to be removed or cause it to be removed and placed in
storage in the nearest garage or other place of safety designated or maintained by this Municipality, and
the charges for towing and storage of such vehicle shall be charged to the owner of the vehicle in

Underground services have to be in conduit (PVC) from the transformer to the electric meter,
switch and to the first electrical distribution circuit breaker oanel.
The main disconnect switch shall be located next to the meter on the exterior wall of the
accessible.

All underground conduits are required to be inspected before back-filling the trench.
In multi-family dwelling units, no electrical wiring or feeder cables shall pass from one unit to another. Common
walls and spaces are accepted.
NM Cable (Romex) is not allowed in commercial buildings or structures exceeding three (3) storles.
No use of aluminum wire smaller than size #B will be permitted with the Town of Vail.
TOWN OF VAIL ELECTRICAL PERMIT GUIDELINES
All installations of exterior hot tubs or spa's require a DRB approval from planning. This application will
not be accepted without a copy of the DRB approval form attached (if appticable).
If this permit is for installation of an exterior hot tub or spa on a new elevated platform or deck over
30" above grade, you must also obtain a building permit.
If this permit is for installation of an exterior hot tub or spa on any existing deck or elevated platform,
a structural engineer must review the existing condition and verify that it will support the added
concentrated load. Please provide a copy of the structural engineers wet stamped letter or drawing
with this application.
If this is a remodel in a multi-family building with a homeowners association, a letter of permission
from the association is required.

2.3.1 Base Bid will refled Abatement Conhactor preparing the work areas for
asbestos related demolition/asbestos abatement, performing demolition/abatement
activities, and obtaining requhed clearance inspediors and clearance air sampling per the
General Contractors instructions received during the pr+.bid site walk through and
information herein.

Z.g.1.1Abatement Contractor will initiate work in Unit 4E. Abatement rrrrorkers will
install critical barriers, inslall personnel decontamination unit and separate waste load out
unit, create negative air pressure, and wet and remove all interior case work, trim, stone
work, over gyp-um board walls/ceilings that contain ACM joint compounds. Contrac-torwill
remove all RCM gypsum board and allAGM sheet virrylfloor cotrering. Styrofoam insulation
with ACM adhesives are present behind all perimeter wall finishes and will remain in place'
2.3.1 .1.1Abatement Contractor will leave baseboard hot water heaters intact and
protect in place throughout the demolition/abatement. The Abatement Contractor will cut
the gypsum board wall even wilh the top of the baseboard heaters and leave the material
behind the heaters intact.
2.3.3 Pre,abatement notification and acquisition of permit from the State of
Colorado will be the responsibility of the Abatement Contractor.
2.3.4 Abalement Contractor is advised there is limited electricalservice and hot
and cold water available in the Unit.
2.3.5 Observation windows are required on allabat€ment endosurcs. Observation
window(s) must be installed in such a manner to allow complete view of the entire work
area.
2.3.6 Abatement Contractor should expect tfrat a significant amount of trash/debds
may be left behind in the work areas post move out by ornent Ovners, and these matedals
will require removal/disposal by the Abatement Contractor.
2.3.7 Abatement Contractor may not use elevator to transport any AGM or ACM
contaminated waste. Access to and from the work area can be via the interior stairway.
ACM waste may be Ioaded out through the North bedroom windor and transported
immediately to ACM dumpster or truck located off of paved driveway.
2.3.8 Abatement Contrador will be required to take apprc,ptiate measuras b secure
access to the work areas, secure their property. and secure ACM/ACM wasle throughout
this project.
2.3.9 Limited staging area will be available at this site in the entance conilor. The
entrance corridor windows must be covered with black ploy sheeting. All additional
supplies/materials will need to be mobilized inside the work area immediately.

2.3.10 Non-ACM demolition debris may be loaded out and dumped direcily into a
manned dump truck in the drive located adiacent to Unlt 4E. Truck wilt be required to move
to allow traffic through drive as ne€ded by others.
2.2.11 ACM waste containers must be loaded out in enclosed chute fom North
window of Unit 4E and placed directly into a manned enclosed vannruck in the drive
located adjac€nt to Buitding'B''. Truck will be required to move to allowtraffcthrough drive
as needed by others.
2.3.12 lt is the intent of the enclosed specifications to describe in sufficient detail to
secure bids on complete demolition/abatement of all interior finishes and ACM and ACM
contaminated materials as required by EPA and State of Colorado for this project. All work
not specifically mentioned, which is necessary in order to provide mmplete demolition of
all interior case work, walls, ceilings, and interior finishes/abatement of required ACM (i.e.
ACM dust, debds, ACM contaminated materials) shall be included in the bid. The work
shall conform in quality of materials and workmanship to what is provided as "state of the
art" and "best practices available" in the industry.
2.3.13 Should a bidder find discrepancies in or omissions from the Project
Specifications, or should he/she be in doubt as to their meaning, he/she shall at once notifo
the Building Owner in writing at least 48 hours prior to the time set for opening of bitls. lf
the point in question is not clearly and fully set forth, a wriften addendum, bulletin or
instructions shall be mailed or delivered to each percon obtaining a set of contract
documents. Each person requesting an interpretation will be responsible for delivery of
his/her request to the Building Owner.

PART3. ABATEMENTSTRATEGY
Note: All demolition, and asbestos abatement work will be done in strict accordance with
EpA, OSHA, and State of Colorado regulations and anformation herein which ever prove
to be more stringent.
3.1 Demarcate Work Area:
3.1.1 The Abatement Contractor shall request a variance from Regulation No. 8
for floor, wall. and ceiling poly requirements due to abatement raquirements.
3. 1 .2 Use existing hard, self+losing lockable doors on unib for secufi, and place
"Danger Asbestos Signs" on inside flaps at all entrances to the wolk areas.
3.1.3 Place State of Colorado Air Emissions permit at the entrance to the clean
room inside Unil4E.
3.2 Remove/Dispose of Non.AGM Building Maierials
3.2.1 Abatement Contractor may pre-clean existing carpets with HEPA equipped
vacuums and remove all carpets where doing so does not disturb ACM flooring
underlayments located below carpets.
3.2.2 Abatement Contractor may remove all case worUplumbing fixtures wftere
doing so does not disturb ACM materials/create ACM dust and/orACM debris.
3.3 Not Used
3.4 Initiate Negative Pressure Differcntial:
3.4.1 Negative pressure difierential betnraen the work area (including
decontamination units) and adjacent non-work areas must remain at a minimum of -.03
inches water column pressure differential and a maximum of -.05 inches water column
between the work areas and/or decontamination unils and adjacent areas throughout the
abatement project.
3.4.2 Place 2 HEPA filtered Pressure Differential Units (PDUs), one machine as
a backup, (based on a totalwork area volume of 12,000 cubic feet of air space and 900
cfm actual air flow per PDU under load) in Unit 3E and exhaust the PDU (s) to the outrside
atmosphere through existing window penetrations via flerible or lay flat exhaust ducts
encased in six mil poly and secured to the PDU by hose clamps. Exact locations of the

3.4.3 Contractor will prOvide, install, and maintrain a Strate of Colorado approved
electronic negative pressure manometer with strip graph recorder.
3.4.4 Contractor will perform smoke tests to determine visible negative pressure
daily.
3.5 Install Critical Barriers:
3.5.1 lnstall critical baniers including but not limited to sealing over all windors,
penetrations, equipment, and etc. with $ix mil sheet poly and duct tape except where
OSHA requires two layers of six mil poly critical barriers.
3.6 Not Used.
3.7 Construction and illaintenance of Personnel Decontamination Facllity:
3.7.1 The Abatement Contraclor will build in place contiguous to the work area in
Unit 3E a three stage personnel decontamination enclosure. The three stages will consist
of a clean room, shorer, and equipment room.
3.7.1.1 Personnel decontamination facility walls and ceilings will be consfucled of
two layers of six mil thick PolY.
31 .1 .2 All doorways between individual stages of the decon will include tiple vinyl
flaps or vented rigid doors installed.
3.7.1.3 Any hoses used to supply water to the decon unit, or any part of ihe work
area, must be a minimum rating of heavy duty, and/or high pressure with pipe thrcad or
twist locUqulck coupler fittings. No "garden hose" type hoses or fiftings will be used.
3.7.1.4 The shower water filtering pump assembly will include GFI protected on/ofi
three way switches located on the clean and dirty skles of the shovrler. Shorcr pump hree
way switcfies will be housed in UL approved waterproof boxes and rlrall mounted in the air
locks adjacent to the shower and in the equipment room.
3,7.1.5 All work activilbs end areas outslCe the rrlok araas (including exhaust hoses
in windows), must remain acceptrable and aesthetically appealing to Building Owne/s
Association/City of Vail at all times throughout the projed.

3.9 Emergency Communications:
3.9.'l The Abatement Contraclor willfurnish and use hand held FM commun'lcation
radios (equivalent to Motorola HT-90), capable of allowing Abatement Gontractods
employees to communicate ftom inside the work area, to a designated person outside the
work area. The FM radios will be used for requesting needed supplies, requesting
information from persons outside the work area to persons inside the work area, and
communicating intemal and extemal emergencies. Tapping or pounding on enclosure
walls, and/or shouting through decon units will not be tolerated as a means to
communicate from inside the enclosure to outside the enclosure.
3.10 OSHArState of Colorado Required Air Sampling
3.10.1 The Abatement Contractor will be responsible for their OS|{A-requhed
personal/excursion and outside contrainment air samples including during pre.cleaning
activities, set up of abatement containments, throughout demolition/asbestos removal
operations, and until clearances have been achieved.
3.10.1.1 Individual(s) acquiring any air samples must have successfully completed
a NIOSH 582 or 582e Course.
3. 10, 1 .2 PCM air sample analysis will be utilized br all OS|'lA+equired air sampling
and for norrclearance air samples. All OSHA+equired air sample results must be posted
at the work site within 24 hours of sample procurement.

3.11 Protection of Work Area Surf,aces/Subsbate Surfaces:
3.11.1 Per Colorado Regulation No. 8, critical banisr and poly requirements-
3.1 1.2 Abatement Contactor must protect all electrical service/equipment, plumbing
lineVpiping, and baseboard heaters/associabd piping throughout the
d emolition/abatement process.
3.11.3 Abatement Contractor will use minimalforce to remove gypsum board so that
the metal studs are not as likely to be damaged. Some damage to studs is likely to occur
but the Abatemenl Contractorwill not be responsible for replacement.
3.11.4 Abatement Contractor will insure that the S$rofoam insulation is not
damaged and is properly cleaned prior to clearance monitioring,
3.12 Removal Activities:
3.12.1 Abatement workem will don full-face porered air puriffing respirators with
HEPA filters and protective clothing for alldemolition and asbestos abatemenUcleanup
procedures. tn the event OSHA-required air monitoring results indicate higher respirator
protection levels are required Abatement Contractor will stop work until Type 'C' pressure
demand air supply system and respirators are available and implemented-

work/wall ,ceiling, and floor finishes that will disturlc ACM materials, and remove allACM
materials including allwood paneling/gypsum board walls & gypsum board ceilings, and
ACM sheet vinylflooring and allACM contaminated dusUdebris within Units 4E.
3.13.1.1 Abatementworkers will remove allACM wet (using amended water) and
remove ACM, and place it directly into double six mil poly leak proof disposal containers.
All containers must be approved by Denver Arapahoe Landfill.
3.1 3.1 .1.1 Hydro sprayers, higMow pressure water sprayers, and similar rneans to
remove ACM, ACM debris, and/or residue from work area surfuces is strictly prohibited.
Excessive water build-up in work area(s) enclosures will not be tolerated.
3.13.1.1.2 Visible emissions during removal or cleaning operations will not be
tolerated.
3.13.1.4 NOTE: In the eventany area(s) adlacent to the work area(s) begoqqe
contaminated with asbestos, asbestos-containing dusUdcbris, and/or visible
omissions from the work area3, the Abatement Contactor wlll clean those arsas at
no additional cost to the Onrner. Additionatly, the Abatement Contractor will
reimburre the Building Owner for anylall damagec and induatrial hygiene sewicea

3.14 Post Removal Clearance Protocol:
3.14,1 After abatement is compleb and alluaste conbiners have been removed
fom the work area, the Contrac-tor will HEPA vacuum and/or wet wipe clean all dust and/or
debris from all work area surfaces including but not limited to all wall, ceilings, and floor
surfaces prior to apptying tock down sncapsulant.

g.14.1.1 When the Abatement Contractor feels asbestos abatement and pre-lock
down cleaning is complete he/she willcontact GHP, GHP willperform a visualclearance
inspection. When GHP is satisfied that the work is completc and the work areas have
passed visualinspection, the Abatement Gontractorwillapply penetrating encapsulantto
"lock down" all work area surfaces post removal.
9.14.1.2Afier the encapsulated surfaces are determined to be dry, GHP will initiate
EPA and State of Colorado required clearance visual inspections and aggrcssive PCM
clearance air sampling.
3.14.1.2.1 All work area surfaces will be agitated by one horse power leaf blowers.
State of Colorado certified Air Monitoring Specialists will direct the leaf blowers at all work
area surfaces, including into all surface cracks and crevices.
3.14.1.2.2A representative of the Abatement Conhastor is required to be present
throughout the clearance sampling period.
3.14.1.2.3 Clearance air samples will be analyzed by PCM protocol.
3.'14.1.2.4 lt is the Abatement Contracto/s responsibility to abate all asbestos
hazards, ACM, and debris from the work area, and pass aggressive clearance air sample
tests.
3.14.1.2.51n the event any cleanance air samples fail (>.01 fibers/cubic centimeter),
the Contractor will re-clean the entire work area. All subsequent clearance air sampling
and sample analysis costs will be the responsibility of the Abatement Contrac'tor.
3.15 Post Clearance Activities:
3.15.1 Abatement Contractor will shut down PDUs.
3.15.2 AbatementContractormustcloselyfollowsetproceduresforshuttingdown
the HEPA equipped PDUs.

or near the PDU, a second clearance air sample phase willbe reguired'
9.15,2.4.1 ln the event a second clearance is required, the Abatement Contractor
will initiate low pressure differential and the entire work area willbe re-c'leaned and subject
to an additional aggressive sampling.
3.15.2.4.1.1Any/all expenses associated with re+leaning, including but not limited
to air sampling costs will be the responsibility of the Abatement ConEactor.
3.16 Disposal:
3.16.1 Vehicle's cargo areas, used to haulACM filled leak proof containers, and/or
wrapped ACM when leak proof containers were not feasible, will be proteded by two laprs
of six mil poly.
3.16.1.1 Vehicles used to haulACM filled drums and/orwrapped ACM must have
enclosed cargo areas, with doors capable of locking. Tarps, poly. vinyl, etc., placed over
open cargo areas do not constitute an enclosed vehicle.
3.16.2 The landfillto be used to dispose of allACM generated from this project is
Denver Arapahoe Disposal Sile, 3500 S. Gun Club Road, Aurora or Highway 36 Landfill.
3.16.2.1 Abatement Contractor must provide waste manifest to GHP in a timely
manner.
t.17 Parking and Stage Area:
3.17.1 Abatement Contractor will have no staging area in or adjacent to the
Condominiums. Abatement Confractor is advised that Owner, General Confactor, andlor
Consultant/Froject Managerwill not be responsible for security, Abatement Conhactor is
responsible ior their orn security of their abatement containment and equipmed/sttppliee

project at the above listed property. During the permit approval process the Town of Vail
and the Fire Department raised concems with the building alarm syst,em. Apparently, the
system cannot be isolated in ttre units identified for remodel. Because tbe alarms are
sensitive to_dust the city has recommended the alarms in each unit be covered (bagged)
during the day and rmcovered at night. Per your recommendation, west Environmental
will cover the alarms in each of the units during thc day while asbestos abatement is
occurring and uncover the alarms at night,

Treelop Condominiums, located in Vail, Colorado. Gobrell Hays Partners, Inc. (GHP)
pcrformed visual clearance Inspections and conducted clearance air test:ng post
abatement using aggressfue Phase Contrast Microsco;ry (PCM) air sampling. All
clearance air samples were analyzed to be below the State ol Colorado ciearance level
of 0.01 f/cc.
Scope of Worlt Complgted:
Unlt 4-E scope of work Included removi,rg 1200 sq- ft. of ACM wall texture and 300 sq ft.
of sheet vinyl floodng. Abaternent Contractor installed critical baniers, installed
personnel decontarninaUon unlt and a separate wssle load out unit, created negative air
pressure within the abatement work area/conlainment, and wet and rernoved all interior
gypsum board walls/caltlngs that contaln€d ACM Jolnt compounds. Contractor also
removed all ACM 6heet vinyl lloor covering. Contraclor removed styrofoam in6ulation
that was present behind th6n gypsum board walls dus to the foam insulation Eticking to
gypsum board walls during demoliton.
