Please provide direction for remediation, this will require a two step repair:1 ) Pour back of the incorrect blockout 2 ) Removal of concrete at the correct location Possible Solutions 1 ) Form both sides of the incorrect block out and fill with concrete (rebar runs continuous through the block out)2 ) Chip concrete 6 " either side of existing DIP.
