SPECIAL INSPECTION AND TESTING AGREEMENT
(To applicants of projects requiring Special Inspection or Testing per Section 1701 of the IBC)
The owner or his /her representative, on the advice of the design professional in responsible charge, shall
complete, seal, sign and submit a copy of the Special Inspection Agreement and Structural Tests
Scheduled to the Town of Vail for review and approval. Signatures are required on both pages;
photocopied or faxed signatures are acceptable.
The owner and his /her general contractor, where applicable, shall also acknowledge the following
conditions applicable to Special inspection Testing:
1. Contractor is responsible for proper notification to the Inspection or Testing agency for items
listed.(Page 1) (IBC 1704)
2. Only the testing laboratory should take samples and transport them to their laboratory.
3. Copies of all laboratory reports and inspections are to be sent directly to the Town of Vail by the Testing
agency on a weekly basis.
4. Inspection agency to submit names and qualifications of on -site special inspectors to the Town of Vail
for review and approval.( Page 2)
5. The special inspector is responsible to immediately notify the Town of Vail Building Official in writing of
any concerns and /or problems encountered.
6. It is the responsibility of the contractor to review the Town of Vail approved plans for additional
inspection or testing requirements that may be noted. A pre- construction conference at the job site is
recommended to review special inspection procedures.
7. The special inspector shall use only the Town of Vail approved drawings.
8. All special inspection field reports must be left on site for review by the Town of Vail staff prior to
required inspections or re- inspections.
BEFORE OCCUPANCY WILL BE GRANTED: The special inspection agency shall submit a signed and
sealed statement that all items requiring testing and inspection were fulfilled and reported. Those items not
tested and /or inspected shall be noted in this statement. A copy of the statement shall be maintained at the
job site for the Building Inspector's review prior to final inspection.

A. Impact of surcharges by the new building and drainage on the existing retaining walls.

' A section of this existing wall is proposed to be removed for a driveway and be rebuilt to die into
new building. All new proposed footings for the proposed building are located lower in elevation
compared to the bottom of the existing retaining wall therefore there will be no extra loading or
i surcharges against the existing retaining wall. The drainage will remain the same or be less of an
impact to exiting retaining wall. ( as per Jeff Leonardo)
B. How is the existing wall structure integrity to be maintained during excavation?
Where excavation is being done the wall is being removed. The upper cut excavation is far enough
away where it will have no impact on existing retaining wall. ( as per Ted Johnson: excavator)
C. Provide details and design for the proposed boulder walls as required by the Town of Vail
amendments to the building code.
Details and design located on Landscape Plan Sheet. (as per Dale Wibenmeyer: engineer)
D. Provide design for the proposed disturbance fence.
Distuubance fence location is shown on Site Plan and will be Standard orange web plastic with 4'
steel landscape stakes to secure every 20'. ( as per David Irwin: General Contractor)
Sheet A2 — Item # 1. How is drainage being provided adjacent to doors and stairs.
All doors will be min. of 3" above outside surface treatment. All outside stairs will have min. '/4" per
foot drainage and away from stairs.
Sheet A2.1— Item #1. Provide details and plank for required Radon system per Town of Vail,
amendments. Inchtde in electrical plan.
Have shown location of Radon System on Sheets C 1, E1, E2, A2 and A2.1. This System will be wired.

The walls will have 5/8" `X' firerock in garage with R -21 in walls and R- 30 foil faced insulation in
ceiling which will establish a one hour fire rating. All other penetrations will be closed foam sprayed.
6. Sheet A2.1- Item #6. Provide rescheck showing compliance with the 2009 IECC. Plans are required
to reflect energy calculations. Include u values for window fenestration on plans. See Town of Vail
Amendments requiring a maximum of .30 u value.
Energy Calculations have been posted on sheet M1.The u values ( .27 -.30 )for window fenestration
and posted on sheet A2 -1. (Energy cal. Per Mr.Heat @ 110,423 BTU'S)
7. Sheet A2.1 - Item # 7. Plans to include annotation that the NCFR certificate label to be on glazing at
time of rough inspection.

This "Letter of Record" is in regards to the "Structural Comments" and "Public Works
Comments" for the above referenced project. The following comments from the Town of Vail are
followed by their remedies:
Structural Comments
SHEET S1:
1. Sheet S1 - Item # 1. The Town of Vail has amended the IRC to require the
structural design to be in accordance with the IBC. Revise general note.
The above has been adjusted and clouded on sheet S1.
Sheet S1 - Item # 2. Plans to reflect required special inspections per section
1704.1.1IBC.
Sheet S1 in the "General Notes" reflects any special inspections that may be
required per the local building departments requirements. At this time we feel that
there are no special inspections needed except a soil report as stated in the general
notes sheet S1.
3. Sheet S1- Item # 3. Provide details for the propose boulder walls.
Details for boulder walls shall be provided by a geotechnical engineer.
4. Sheet S1 - Item # 4. Clarify the loading proposed against the existing walls and

Detail 5 /S1 is a typical stone veneer support for a maximum height of 6-0. It is
supported on a steel angle with expansion anchors as described in the detail. All
waterproofing, flashing, and drainage requirements are per the architect.
6. Sheet S1- Item # 6. Soils to be included in the special inspection criteria due to the
soils investigation work has not been done per the applicant.
This is stated under the Foundation Design section in the General Notes on Sheet
S1.

Professional Services: Hepworth - Pawlak Geotechnical, Inc. (H -P GEOTECH) agrees to perform the
services outlined below. The CLIENT may subsequently amend the scope of services with the
concurrence of H -P GEOTECH. Services provided by H -P GEOTECH will be consistent with the
engineering standards prevailing at the time and in the area that the services are performed; no
warranty, express or implied, is intended. CLIENT is responsible to provide the information known to
be existing regarding the project as it applies to the execution of H -P GEOTECH's services. Please
note the attached Terms and Conditions are a part of this Agreement and contain a limitation of
liability clause.
Scope of Services: Make site visits as requested to observe the soils exposed in the building
excavation and provide an opinion on their suitability for foundation support. You have assumed
spread footings bearing on the natural soils as the foundation type. Subsurface exploration to evaluate
the soils within the loaded depth of the foundation influence is not requested. Variations in the
subsurface conditions not identified could increase the risk of foundation movement and building
distress. Present the findings of the services performed in letter reports.
Additional scope of services will be based on the unit charges and Terms and Conditions of the
attached Fee Schedule.
Fee: Our fee will be based on the unit costs listed on the attached Fee Schedule and other costs needed
to perform our services.
Termination: This Agreement may be terminated by either party upon reasonable written notice. Our
services will be stopped and fees will be calculated to include all services performed and materials
used or ordered.
Acceptance: We are proceeding with the above scope of services based on the verbal authorization by
Dave Irwin on June 26, 2012. Unless notified otherwise, we assume that you accept this Agreement.
Prior to issuing our report, one copy of this Agreement must be signed and returned to us.
