is required to request tree removal /replacement in the Town of Vail. This form must be signed by a Town of Vail au-
thorized representative who has inspected the tree(s). To request an inspection, please call Tom Talbot, Wildland Coor-
dinator, at (970) 477 -3509.

Attached are the photos for the tree removal request at the Vail Golf Course Townhomes Phase 1 (1720 Sunburst Dr.,
Vail). The tree in question is marked with a red ribbon as noted in the photos. I wrote on the back of the DRB
application that it was located on the E side behind unit #4, it is actually located on the West side of the building behind
unit #4.
