This letter summarizes the special inspectians GROUND Engineering Consultants, inc. (GROUND)
pe�formed far the proposed additions to the residence at 303 G�re Cr�ek Drive located in Vail, Colorado.
The special inspection and materials testing services were performed at times scheduled by the
Contractor and/or Subcontractors between October 26th, 2011 and May 30'h, 2012.
Representatives of our affice pravided periodic obse►vations far the structural concrete placed for the
proposed additions for the foundation, foundation walis and slab on grade as scheduled by the Owner,
Contractor or Subcontractor. Samples of the structurai concrete were periodicaily tested to determine
physicai properties and compressive strength. tilso, representatives of our office provideci observations
for ths piacement of epoxied dowel reinforcements for#he proposed additions.
Based on the final test results to date, field observations, and other project documentation �eceived from
the design team, it is our opinion that the materials abserved for the structurai concrete and epoxied
dowel reinforcing for the proposed foundation additions were in generai accordance with project plans,
specifications, or applicable RFI's.
The observations as indicated above were per�orm�d periodicaliy tu �rovide data to th�e Con�ractor and
Subcor�tractors to assist tr�em in evaluatiny tfie general corr�pliance �f tf�e fested materials ani/or work �
with the contract docurnents at the times of our observation. We did not supervise, direct or have any
control over any of the Contractor's or Subcontractor's work. We were not responsible for the
Contractor's or Subcontractor's scheduling of our services, means, methods, procedures, techniques, or
sequences of corstruction, nor responsible for Ansuri;�g that any Contractor or Su�acontra�tor performed
or completed the work in accordance with the contract documents or construction industry standards.

This letter summarizes the speciai inspections GROUND Engineering Gonsultants, inc. {GROUND}
performed far the proposed additians to the residence at 303 Gore Creek Drive located in Vail, Colorado.
The special inspection and materials testing services were performed at times scheduled by the
Cantractor andlor Subcontractors between October 26th, 2011 and May 30th, 2012.
Representatives of our office provided periodic observations for the structurai concrete piaced for the
proposed adaitions for the foundation, foundation wafis and slab on grade as scheduled by the Owner,
Contractor or Subcontractor. Sampies of the structural concrete were periodically tested to deterrr�ine
physical properties and compressive strength. Aiso, representatives of our office provids� observations
for the placement of epoxied dowel reinforcements for the proposed additions.
Based on the final test results to date, field observations, and other project documentation received from
the design team, it is our opinian that the materials observed for the structurai concrete and epoxied
dowel reinforcing for the proposed found�tican additians were in general acccr��n�P tivith pr�ject pi�rs,

As requested, a representative of GROUND visited the project to provide periodic construction
materials testing. We observed Vasquez Concrete had drilled holes into the east edge of the
existing north basement foundation wall. Vasquez Concrete utilized a wire brush and air
compressor to clean out the holes. The drilled holes were observed for specified depth,
diameter, and cleanliness. We then observed Vasquez Concrete utilizing Hilti HY-150 Max
epoxy to place reinforcing dowels into the drilled holes until excess epoxy was observed being
displaced from the holes. Based on today's observations, the drilled holes and reinforcing
dowels appeared to have been constructed in general compliance with the project plans and
site specifications.
Gilbert with Vasquez Concrete was notified of our observations.

As requested, a representative of GROUND visited the project to provide periodic construction
materials testing. We observed Vasquez Concrete had drilled holes into the existing concrete
for the reinforcing steel dowels for the footing at approximately gridlines C.0-C.5/1.2. The drilled
holes were observed for depth, diameter and cleanliness of the holes. The drilled holes were
approximately 3"-4" in depth, and did not meet the 6" specified depth per detail #13 on plan
sheet S-5. We immediately informed George with Nedbo Construction. George notified Ground
Engineering they were hitting existing steel and could not drill any deeper and that they would
contact the structural engineer to provide recommendations as how to proceed.
We also observed the reinforcing steel placed by Vasquez Concrete for the pier footings along
gridlines A.0/1.0. The reinforcing steel was observed for specified bar sizing, spacing
tolerances, lap splices and form clearance. Based on today's observations, the reinforcing steel
at the previously described locations appeared to have been placed in general accordance with
the project plans and specifications.
George with Nedbo Construction was informed of today's observations.

materials testing. We observed holes drilled by Vasquez for the footings at gridlines A.0-A.5/1.0
and 2.0. The drilled holes were observed for specified depth, diameter, and overall cleanliness.
We then observed Vasquez Concrete utilizing Hilti HY-150 Max epoxy to place reinforcing
dowels into the drilled holes until excess epoxy was obsenred being displaced from the holes.
Based on our observations, the drilled holes and reinforcing dowels at the previously described
locations appeared to have been placed in general conformance with the project plans and
specifications.

materials testing. We observed holes drilled into the existing concrete by Vasquez Concrete for
the footings in the basement level at approximately gridlines C.8-D.2/1.3 and C.2/1.0-1.2. The
drilled holes were observed for specified depth, diameter, and overall cleanliness.
We then observed Vasquez Concrete utilize Hilti HY-150 Max epoxy to epoxy reinforcing
dowels into the drilled holes until excess epoxy was observed being displaced from the holes.
Based on today's observations, the drilled holes reinforcing dowels at the previously described
locations appeared to have been constructed in general conformance with the project plans and
specifications.
Additionally, we retrieved previously cast concrete test specimens. The specimens were
returned to our laboratory for final curing and future compressive strength testing. Results will
be forwarded upon completion.

the foundation walls at approximately gridlines A.5-B.0/1.0 and 2.0. The drilled holes were
observed for specified depth, diameter, and cleanliness. We observed that the holes drilled into
the existing CMU walls were approximately 5 to 5 1/2" in depth. This was approved by the
structural engineer per a verbal conversation on site.
We then observed Vasquez Concrete utilize Hilti HY-150 Max epoxy to epoxy reinforcing
dowels into the drilled holes until excess epoxy was observed being displaced from the holes.
Based on today's observations, the drilled holes and reinforcing dowels at the previously
