(GROUND) pe�formed far the proposed additions to the residence at 303 G�re Cr�ek Drive located in Vail, Colorado. The special inspection and materials testing services were performed at times scheduled by the Contractor and/or Subcontractors between October 26th, 2011 and May 30'h, 2012. Representatives of our affice pravided periodic obse►vations far the structural concrete placed for the proposed additions for the foundation, foundation walis and slab on grade as scheduled by the Owner, Contractor or Subcontractor. Samples of the structurai concrete were periodicaily tested to determine physicai properties and compressive strength. tilso, representatives of our office provideci observations for ths piacement of epoxied dowel reinforcements for#he proposed additions. Based on the final test results to date, field observations, and other project documentation �eceived from the design team, it is our opinion that the materials abserved for the structurai concrete and epoxied dowel reinforcing for the proposed foundation additions were in generai accordance with project plans, specifications, or applicable RFI's. Box 1439 Gypsum, Colorado Attn: Mr. Vasquez As requested, a representative of GROUND visited the project to provide periodic construction materials testing. We observed Vasquez Concrete had drilled holes into the east edge of the existing north basement foundation wall. Vasquez Concrete utilized a wire brush and air compressor to clean out the holes. The drilled holes were observed for specified depth, diameter, and cleanliness. We then observed Vasquez Concrete utilizing Hilti HY-150 Max epoxy to place reinforcing dowels into the drilled holes until excess epoxy was observed being displaced from the holes. Based on today's observations, the drilled holes and reinforcing dowels appeared to have been constructed in general compliance with the project plans and site specifications. We observed Vasquez Concrete had drilled holes into the existing concrete for the reinforcing steel dowels for the footing at approximately gridlines C.0-C.5/1.2. The drilled holes were observed for depth, diameter and cleanliness of the holes. The drilled holes were approximately 3"-4" in depth, and did not meet the 6" specified depth per detail #13 on plan sheet S-5. We immediately informed George with Nedbo Construction. George notified Ground Engineering they were hitting existing steel and could not drill any deeper and that they would contact the structural engineer to provide recommendations as how to proceed. We also observed the reinforcing steel placed by Vasquez Concrete for the pier footings along gridlines A.0/1.0. The reinforcing steel was observed for specified bar sizing, spacing tolerances, lap splices and form clearance. Based on today's observations, the reinforcing steel at the previously described locations appeared to have been placed in general accordance with the project plans and specifications.
