State of Colorado Asbestos Testing & Abatement Requirements Asbestos testing and abatement protects workers, homeowners, neighbors and emergency services responders from exposure to
harmful asbestos. It is your responsibility to be in compliance with the State. Please contact the State directly for their requirements at the contact info listed below. When is asbestos
testinq required? ANY building projects disturbing more than these threshold levels of building materials require asbestos testing: One-and Two-Family Dwellings: 32 square feet All Others
(commercial spaces, hotel rooms, etc): 160 square feet Definition of a single-family dwelling: any dwelling unit that is used primarily for a single family, including multi-family/condominium
units, and fractional fee units. Asbestos testing results must be provided with your application for a building permit. Tests which identify POSmVE results at more than 1% require abatement
by a State-certified abatement contractor. The air clearance letter or form must be submitted to the Town of Vail before the building permit will be issued. Project Checklist My project
falls into the category checked below: D Will not disturb more than the threshold limits identified above. D Tested negative, or at 1% or below (1copies of test results included) ~ Tested
positive at more than 1%, requires abatement (1 copies of test results included) Tips & Facts: • Even recent construction projects may include asbestos-containing materials, so buildings
of any age require testing. • The "1989 Ban" on asbestos-containing materials is commonly misunderstood. "In fact, in 1991 the U.S. Fifth Circuit Court of Appeals vacated much of the
so-called "Asbestos Ban and Phaseout" rule and remanded it to the EPA. Thus, much of the original 1989 EPA ban on the U.S. manufacturing, importation, processing, or distribution in
commerce of many asbestos-containing product categories was set aside and did not take effect." -CDPHE Asbestos test results should be submitted to: Town of Vail, Community Development,

Job No. 111 326B
Subject: Observation of Excavation, Proposed Schultz Residence, Lot 10, Block 2,
Bighorn Subdivision Fifth Addition, 5056 Main Gore Drive, Vail,
Colorado
Dear Don:
As requested, a representative of Hepworth-Pawlak Geotechnical, Inc. observed the
excavation at the subject site on August 17, 2012 to evaluate the soils exposed for
foundation support. The fmdings of our observations and recommendations for the
foundation design are presented in this report. The services were performed in
accordance with our agreement for professional engineering services to Golf Resort
Homes, dated July 19, 2012.
We previously conducted a subsoil study for design of foundations at the site and
presented our fmdings in a report dated November 30, 2011, Job No. 111 326A. The
proposed construction is generally similar to that discussed in our report. The residence
will be a two story structure with the north and east side foundation walls retaining the cut
into the southwesterly sloping terrain. Spread footings bearing on the natural granular
soils designed for an allowable soil bearing pressure of 2,000 psf were recommended for
foundation support.
At the time of our site, the foundation excavation, which was essentially complete, had
been cut in multiple levels from about 4 to 71/2 feet below the adjacent ground surface.
The steps in grade between the excavation levels were from about 1 to 2%2 feet. The soils
exposed in the bottom of the excavation consisted of medium dense, silty sand and gravel
with cobbles. Free water was encountered in the excavation in the deeper cut area along
the east side of the excavation. The water was about 2 inches deep in the bottom of the
excavation in area of free water. The soils were moist to wet.
The soil conditions exposed in the excavation are consistent with those previously
encountered on the site and suitable for support of spread footings designed for the
recommended allowable bearing pressure of 2,000 psf. The water seepage into the
excavation should be collected and diverted away from the footing areas and any softened
soils and mud removed before concrete placement. It should be feasible to remove the

water by digging a sump pit outside the footing area and pumping the water. A shallow
depth of screened rock can be placed in sub-excavated areas to re-establish design footing
bearing elevation and help allow construction of the footings in the dry. Other
recommendations presented in our previous report which are applicable, including those
relating to a perimeter foundation drain and drain gravel below the lower level floor slab,
should also be observed.
The recommendations submitted in this letter are based on our observation of the soils
exposed within the foundation excavation and the previous subsurface exploration at the
site. Variations in the subsurface conditions below the excavation could increase the risk
of foundation movement. We should be advised of any variations encountered in the
excavation conditions for possible changes to recommendations contained in this letter.
Our services do not include determining the presence, prevention or possibility of mold or
other biological contaminants (MOBC) developing in the future. If the client is
concerned about MOBC, then a professional in this special field of practice should be
consulted.
If you have any questions or need further assistance, please call our office.
Sincerely,
