General Information:
This application is required for proposals involving minor exterior alterations and/or site improvements.
Proposals to add landscaping do not require DRB approval unless they involve the addition of patios,
water features t gtading, or the addition of retaining walls.

and a basis of bearing must be shown. Show existing pins or monuments found and their

indicated in the title report. List any easement restrictions.. Spot Elevations at the edge of asphalt, along the street frontage of the propefi at twenty-five
foot intervals (25'), and a minimum of one spot elevations on either side of the lot.. Topographic conditions at two foot contour intervals . Existing trees or groups of trees having trunks with diameters of 4" or more, as measured from a
point one foot above grade.. Rock outcroppings and other significant natural features (large boulders, intermittent streams,
etc. ).. All existing improvements (including foundation walls, roof overhangs, building overhangs, etc.).. Environmental Hazards (ie, rocKall, debris flow, avalanche, wetlands, floodplain, soils). Watercourse setbacks, if applicable (show centerline and edge of stream or creek in addition to
the required stream or creak setback). Show all utility meter locations, including any pedestals on site or in the right-of-way adjacent to
the site. Exact location of existing utility sources and proposed service lines from their source to
the structure. Utilities to include:. Cable TV Sewer Gas . Telephone Water Electric . Size and type of drainage culverts, swales, etc,. Adjacent roadways labeled and edge of asphalt for both sides of the roadway shown for a
minimum of 250' in either direction from property.
. Site and Grading Plan:. Scale of L"--20'or larger . Property and setback lines . Existing and proposed easements . Existing and proposed grades . Existing and proposed layout of buildings and other structures including decks, patios, fences and
walls, Indicate the foundation with a dashed line and the roof edge with a solid line.. All proposed roof ridge lines with proposed ridge elevations. Indicate existing and proposed
grades shown underneath all roof lines. This will be used to calculate building height.. Proposed driveways, including percent slope and spot elevations at the property line, garage slab
and as necessary along the centerline of the driveway to accurately reflect grade.. A 4' wide unheated concrete pan at the edge of asphalt for driveways that exit the street in an
uphill direction.. Locations of all utilities including existing sources and proposed service lines from sources to the

PRE-APPLICATION CONFERENCE
A pre-application conference with Town of Vail staff is required. No application can be accepted
unless the mandatory pre-application meeting has been completed. It is the applicant=s
responsibility to schedule this meeting by calling 970-479-2128.
TIME REOUiREMENTS
The Design Review Board meets on the 1st and 3rd Wednesdays of each month. A complete
application form and all accompanying material must be accepted by the Community Development
Department a minimum of three and a half (3 1/2) weeK prior to the date of the DRB public
hearing.
REVIEW CRMRIA
Your proposal will be reviewed for compliance with the Design Guidelines as set forth in Section 12-
1l of the Municipal Code.
NOTE TO ALL APPLICANTS:
A. If a propefi is located in a mapped hazard area (i.e. snow avalanche, rocKall, floodplain,
debris flow, wetland, etc), a hazard study must be submitted and the owner must sign an
affidavit recognizing the hazard report prior to the issuance of a building permit. Applicanb
are encouraged to check with the planning staff prior to submittal of a DRB application to
determine the relationship of the property to all mapped hazards.
B. Easic Plan Sheet Format. For all surveys, site plans, landscape plans and other site
improvements plans, all of the following must be shown.
1. Plan sheet size must be 24"x36". For large projects, larger plan size may be
allowed.2. Scale. The minimum scale is 1"=20'. All plans must be at the same scale.3, Graphic bar scale.4, North arrow.5. litle bloc( project name, project address and legal description.6. Indication of plan preparer, address and phone number.7. Dates of original plan preparation and all revision dates.8. Vicinity map or location map at a scale of 1"=1,000' or larger.
9. Sheet labels and numbers.10. A border with a minimum left side margin of 1.5".11, Names of all adjacent roadways.72. Plan legend.
C. For new construction and additions, the applicant must stake and tape the project site to
indicate property lines, proposed buildings and building corners. All trees to be removed
must be taped. The applicant must ensure that staking done during the winter is not
buried by snow. All site tapings and staking must be completed prior to the day
of the DRB meeting.
D. Applicants who fail to appear before the Design Review Board on their scheduled meeting
date and who have not asked in advance that discussion on their item be postponed, will
have their items removed from the DRB aqenda until such time as the item has been
reoublished.
E. If the DRB approves the application with conditions or modifications, all conditions of
approval must be resolved prior to the issuance of a building permit.
STAFF APPROVAL
The Administrator may review and approve Design Review applications, approve with certain modifications,
deny the application, or may refer the application to the Design Review Board for decision. All staff
approvals are subject to final approval by the DRB. The following types of Design Review applications may
be staff approved:
A. Any application for an addition to an existing building that is consistent with the architectural design,
materials and colors of the building, and approval has been received by an authorized member of
a condominium association, if applicable;
B. Any application to modify an e)(isting building that does not signifrcantly change the existing planes
of the building and is generally consistent with the architectural design, materials and colors of the
building, including, but not limited to exterior building finish materiab (e.9. stoneworK siding, roof
materials, paint or stain,), exterior lighting, canopies or awnings, fences, antennas, satellite dishes,
windows, skylights, siding, minor commercial facade improvemenE, and other similar modfications;
C. Any application for site improvemenE or modifications including, but not limited to, driveway
modifications, site grading, site walls, removal or modifications to existing landscaping, installation
of accessory structures or recreational facilities,
ADDMONAL REVIEW AND FEES
A, ff this application requires a separate rwiew by any local, state or Federal agency other than the
Town of Vail, the application fee shall be increased by $200.00. Examples of such review, may
include, but are not limited to: Colorado DeparEnent of Highway Access Permits, Army Corps of
Engineers 404, etc.
B. The applicant shall be responsible for paying any publbhing fees which are in excess of 50% of the
application fee. If, at the applicant's request, any matter is postponed for hearing, causing the
matter to be re-published, then the entire fee for such re-publication shall be paid by the applicant,
C. Applications deemed by the Community Development Department to have design, land use or other
issues which may have a significant impact on the community may require review by consultants
in addition to Town staff. Should a determination be made by the Town staff that an outside
consultant is needed, the Community Development Department may hire the consuftant. The
Department shall estimate the amount of money necessary to pay the consultant and thb amount
shall be forwarded to the Town by the applicant at the time of filing an application. Expenses
incurred by the Town in excess of the amount forwarded by the application shall be paid to the
Town by the applicant within 30 days of notification by the Town. Any excess funds will be retumed
to the applicant upon review completion.

1. The Developer agrees, at its sole cost and expenses, to furnish all equipment

lmprovements as listed above, in accordance with all plans and specifications filed in the
office of the community Development Department, the Town of Vail, and to do all work
incidental thereto according to and in compliance with lhe following:
a. Such other designs, drawings, maps, specifications, sketches, and olher matter
submitted by the Developer to be approved by any of the above-referenced
governmental entities. All said work shall be done under the inspection of, and
to the satislaction of, the Town Engineer, the Town Building Official, or other
offieial from lhe Town or Vail, as affected by special districts or senrlce distr.icts,
as their respective interest may appear, and shall not be deemed complete until
approved and accepted as completed by the Town of Vail Community
Development Department and public Works Department.
2- To secure and guarantee performance of its obligations as set forth herein, the
Developer agrees to provide security and collateral as follows:

3. The Developer may at any time substitute the collateral originally set forth
above for anolher form ot collateral acceptable to the Town to guarantee the laithful
completion of those improvements referred to herein and the performance of the terms of this
Agreement. Such acceptance by the Town of alternative collateral shall be at the Town's sole
discretion.
4. The Town shall not, nor shall any officer or employee thereof, be liable or
responsible for any accident, loss or damage happening or occuning to the work specified in
this Agreement prior to the completion and acceptance of the same, nor shall the Town, nor
any officer or employee thereof, be liable for any persons or property injured by reason of lhe
nature of said work, but all of said liabilities shall and are hereby assumed by the Developer.
The Developer hereby agrees to indemnify and hold harmless the Town, and any of its
officers, agents and employees against any losses, claims, damages, or liabilities to which the
Town or any of its officers, agents or employees may become subject to, insofar as any such
losses, claims, damages or liabilities (or actions in respect thereof) that arise out of or are
based upon any performance by the Developer hereunder; and the Developer shall reimburse
the Town for any and all legal or other expenses reasonably incurred by the Town in
connection with investigating or defending any such loss, claim, damage, liability or action.
This indemnity provision shall be in addition to any other liability wnicn tne Developer may
have.
5. lt is mutually agreed that the Developer may apply to the Town and the Town
shall authorize for partial release of the collateral deposited with the Town for each category
of improvement at such time as such improvements are constructed in compliance with Al -
plans and specifications as referenced hereunder and accepted by the Town. Under no
condition will the amount of the collateral that is being held be reduced below the amount
necessary to complete such improvements.
6. lf the Town determines, at its sole discretion, that any of the improvements
contemplated hereunder are not constructed in compliance with the plans and specifications
set forth in this Agreement on or before the date set lorth in Paragraph 2, the Town may, but
shall not be required to, redeem the letter of credit as necessary fo complete the unfinished
improvements. The Bank shall release such lunds upon written request from the Town stating
that the improvements have not been completed as required by this agreement. The Bank
shall not require the concurrence of the developer prior to releise of l[e lunds to the Town
nor shall the Bank be required to verify independenlly that such improvements have not been
completed as required by this agreement, bul shall release such lunds solely upon the Town's
written request.
lf the costs of completing the work exceed lhe amount of the deposit, lhe excess,
together with interest attwelve percenl per annum, shall be a lien against the property and
may be collected by civil suit or may be certified to the treasurer of Eagle County'to be collected in the same manner as delinquent ad valorem taxes levied against such property. lf
the permit holder fails or refuses to complete the cleanup and landscaping, as defined in ihis
chapter, such failure or refusal shall be considered a violation of the Zoning code.
7. The Developer warrants all work and material for a period of one year after acceplance of all work referred to in this Agreement by the Town ii such work is located on Town of Vail property or within Town of Vail rlgnt-ot-way pursuant to Section 17.16.250.

Dear Mr. Jobnson:
As requested, Hepworth-Pawlak Geotechnical, Inc. performed a subsoil study for
design of foundations at the subject site. The study was initiated as an observation of
excavation in accordance with our agreement for geotechnical engineering services to
R.J. Builders dated March 30, 1994 and later amended to include an explorarory
boring. The data obtained and our recommendations based on the proposed
construction and subsurface conditions encountered are Dresented in this reoort.
Proposed Construction: The proposed residence will be a 3 story structure with a
basement level. Ground floors will be slab-on-grade and strucural above crawl space.
Cut depths are extensive and range between about 4 to 6 feet in the middle lower part to
about 15 to 20 feet along the back side. Foundation loadings for this rype of
construction are assumed to be relatively light to moderate.
Ifbuilding conditions or foundation loadings are significantly different tiom those
described above, we should be notified to reevaluate the recommendations presented in
this report.
Site Conditions: The building site is located below Lion's Ridge which is a prominent
sandstone outcrop. The area is impacted by potential rockfall and we understand that
mitigation has been designed by others. The ground surface slope is moderate to sreep
down to the south with about 15 feet of elevation difference across the buildins site.
When we observed the excavation on March 31 several cut levels had been made
berween about 4 to 20 feet deep. We understand that the excavation had been made
about 5 weeks prior to our observation. The bottom was still wet and soft in some
areas from recent snowmelt. Most of the excavation material appeared to have been
removed tiom the site except for a stockpile along the front and a pile of large boulders
apparently encountered during excavation.
Subsurface Conditions: The subsurface conditions at the site were evaluated by
observing the soils exposed in the excavation and by drilling 1 boring in the southwest
pan of the excavation. The log of the boring is presented on Fig. 1. The subsoils
exposed in the excavation and to a depth of about 15 feet in the boring consist mainly of

silty to clayey sand with local clay and gravelly zones. The soils appear to be of lower
density in the excavation, and higher density with depth. Results of swell-consolidation
testing perforrned on relatively "ndisturbed samples ofthe sands and clays, presented
on Figs. 2 and 3, indicate low to moderate compressibility under conditiors of loading
and wetting. Results of a gradation analysis perfonned on a sample of the more
gravelly soil (minus 1 1/2-inch fraction) obtained from the excavation are presented on
Fig. 4. No free water was observed in the boring at the time of drilling and the soils
were slighfly moist to moist.
Foundation Recommendations: Considering the subsoil conditiors observed in the
excavation and encountered in the exploratory boring and the nature of the proposed
construction, we recommend spread footings placed on the undisturbed natural soil
designed for an allowable soil bearing pressure of 2,000 psf for support of the
proposed residence. Settlements are expected to be about I inch which could be
differential and occur after construction. Footings should be a minimum width of
16 inches for continuous walls and 2 feet for columns. Loose and disturbed soils and
any backfill encountered at the foundation bearing level within the excavation should be
removed and the footing bearing level extended down to the undisturbed natural soils.
The exposed subgrade should be compacted. Exterior footings should be provided with
adequate cover above their bearing elevations for frost protection. Placement of
footings at least 48 inches below the exterior grade is typically used in this area.
Continuous foundation walls should be reinforced top and bottom to span local
anomalies such as by assuming an unsupported length of at least 12 fert. Additional
reinforcement may be warranted at steps in grade. Foundation walls acting as retaining
structures should be designed to resist a lateral earth pressure based on an equivalent
fluid unit weight of at least 45 pcf for the on-site soil as bacldill. The loading on the
uphill back wall could be about 60 pcf equivalent fluid weight due to surcharge from the
steep backslope.
Floor Slabs: The natural on-site soils are suitable to support lightly loaded
slab-on-grade construction. To reduce the effects of some differentiai movement, non-
structural floor slabs should be separated from all bearing walls and columns with
expansion joints which allow unrestrained vertical movement. Floor slab control joints
should be used to reduce damage due to shrinkage cracking. The requirements for joint
spacing and slab reinforcement should be established by the designer based on
experience and the intended slab use. A minimum 4-inch layer of freedraining gravel
should be placed beneath basement level slabs to facilitate drainage. This material
should consist of minus 2-inch aggregate with less .han 50% passing rhe No. 4 sieve
and less than2% passing the No. 200 sieve.

All fill materials for support of floor slabs should be compacted to at least 95% of
maximum standard Proctor densiry at a moisture content neiu optimum. Required frll
can consist of the on-site soils devoid of vegetation, topsoil and oversized rock.
Underdrain System: Although free water was not encountered during our exploration,
it has been our experience in mountainous areas that local perched groundwater may
develop during times of healry precipitation or seasonal mnoff. Frozen ground during
spring runoif can create a perched condition. We recommend below grade
construction, such as retaining walls, crawl space and basement areas, be protected
fiom wening and hydrostatic pressure buildup by an underdrain system.
The drains should consist of drainpipe piaced in the bottom of the wall backfill
surrounded above the invert level with free-draining granular material. The drain
should be placed at each level of excavation and at least I foot below lowest adjacent
finish grade and sloped at a minimum 1% to a suitable gravity outlet. Free-draining
granular material used in the underdrain system should contain less than 2% passing the
No. 200 sieve, less than 50% passing the No. 4 sieve and have a maximum size of
2 inches. The drain gravel backfill should be at least I 1i2 feet deep.
Surface Drainage: The following drainage precautions should be observed during
construction and maintained at all times atter the residence has been comoleted:
Inundation of the tbundation excavations and underslab areas should be avoided
during construction.
Exterior backfill should be adjusted to near optimum moisture and compacted to
at least 95% of the maximum standard Proctor density in pavement and slab
areas and to at least 90% of the maximum standard Proctor density in landscape
areas. Free-draining wall backfill should be capped with at least 2 feet of the
on-site, finer graded soils to reduce surface water infiltradon.
The ground surface surrounding the exterior ofthe building should be sloped to
drain away from the foundation in all directions. We recommend a minimum
slope of 12 inches in the first l0 feet in unpaved areas and a minimum slope of
3 inches in the first 10 feet in pavement and walkway areas. A swale will be
needed uphill to direct surface runoff around the residence.
Roof dowrspouts and drains should discharge well beyond the limits of all
backtlll.

Limitations: This report has been prepared in accordance with generally accepted
geotechnical engineering principles and practices in this area'at this time. We make no
other warranty either expressed or implied. The conclusions and recommendations
submitted in this report are based upon the data obtained from the excavation
observation and the exploratory boring drilled at the location indicated on Fig. 1, the
proposed type of construction and our experience in the area. Our findings include
interpolation and extrapolation ofthe subsurface conditions identified at the site. If
conditions encountered during construction differ from those described in this report,
we should be notified at once so re+valuation of the recommendations may be made.

The following items were observed:
The framing is essentially finished at this time. We discussed the concrete design for the entry porch and stairs.
The porch slab should be 6" fiber reinforced concrete with a #4 @ 12: olc. mat in the boftom. Drill & grout #4 x
18" dowels into the existing foundation walls around the perimeter, connect these to the reinforcing mat in the slab.
Provide a grade beam at the south side of this porch area. This should be 8" x 12" with 2 #4 in the bottom. This is
also to be doweled into the walls at each end with2 #4 dowels. The stairs should have the same #4 @ 12" olc. mat
in the bottom of the fiber reinforced concrete. Provide a minimum thickness of4" of concrete, and a #4 nosing bar
at each tread. These should also be doweled into the adjacent walls and grade beam with #4 x 18" @24" o/c.
The fiaming has basically been done according to the original plans with the changes that Chuck and I have
discussed. The only item which we had not discussed is the double | 3/4 x l4" micro:lam header at the northeast
comer of the roof fiaming. This is adequate to carry the design loads; add a vertical 2x4 on each side of the west
end up the face of the adjacent truss to stabilize the member against rotation.
Chuck has described the I I 7/8 'l'Jl and micro-lam framing that will be added in the garage to generate a storage
loft. This systern appears to meet all local Code requirements. I will make a final inspection next week to confirm
this.
Over the Living Room, the west valley beam should be connected to the king rafter with Iwo 3/4" diameter threaded
rods. The high end of both of these valleys should be supported off of the glu-lam beam below with three 2x6's.
The high end of both of the valleys over the Master Bedroom should be connected to thc dormer ridge beam with
two 3/4" diameter las bolts.
