How it relates to Building permits:
1) Fill out our check list provicted with a buitdino oermit aoplication.lf yes was answered to any of the above questions. then a "puOlic Way', is
required. You can pick up an application at either community Developmeni,
located at 75 S. Frontage Road or Public Works, tocated at 13dg Vait Valtey Drive.
2l Notice sign offs for utility companies. All utilities must fietd verify (tocate)
respective utilities prior to signing application. Some utility companies iequire up
to a 48 hour notice to schedule a locate.
3) A construction traffic control/staging plan must be prepared on a separate sheet
of paper.An approved site plan may also be used. This plan willshbw locationsof all traffic control devices(signs, cones, etc..) and the work zone, (area of
Construction, Staging, etc..). This plan willexpiie on Oct. t5th. and will'need tobe resubmitted for approval through the winter.
4) Sketch of work being performed must be submitted indicating dimensions (length,
wjdth & depth of work). This may be drawn on the traffic controt plan oi a -site
plan for the job.
5) Submit completed application to the Pubfic Works's office for review. lf required,
locates will be scheduled for the Town of Vail Electricians and lrrigation crew. The
locates take place in the morning but, may require up to 4g hours to perform.
6) The Public Work's Construction Inspector will review the apptication and approve
or disapprove the permit. You will be contacted as to the status and any ttrai mayneeded. Most permits are released within 48 hours of being rec'eived, but ptease, allow up to one week to process.
7) As soon as the permit is processed, a copy will be faxed to community
Development allowing the "Building Permif'to beieleased. Please do not confuse
the "Public Way Permit" with a "Building Permit" to do work on a project itself.

conducted an analysis to determine the appropriate spacing for underpinning piers. The
purpose ofthe piers is to stabilize the foundation movement which is occurring on the
south (downhill) side of the residence.
Based on a load capacity per pier of 40,000 pounds as provided by the manufactwer, a
maximum pier spacing of 8'-0" along both the south side and the east side of the existing

The Chanceo Helical Piero system
includes sha{ts of round and square
configurations to match design

aggressive soils, hot-dip galvaniz-

Helical piers have alleviated
stresses on buildings caused by
foundations shifting in expansive

The load capacity of a multi-helix
anchor is the sum of a]l individual
helix capacities. Individual helix
bearing capacity is the product of its
projected area (Ar,) and the bearing
pressu-re (q'). Reduction in load
capacity should be accounted for in
sensitive soils. Allowances should be
made when the zone of influence for
a helix includes strata of different
strengths.
This analysis depends on a "deep
anchorage" failure mode. This
condition is satisfied when the helix
depth-to-diameter ratio is a mini-
mum of 5.
HOLDING STRENGTH RELATED
TO INSTALLING TORQUE
Monitoring installation torque helps
indicate anchor capacity. The ratio
between holding capacity and
installation torque suggests 10 as a
rule of thumb. This value typically
ranges from ? to 12. Torque moni-
tors are available from Chance.
Their use provides a good method of
quality control during installation.

DESCRIPTION
Square-Shaft (SS) Helical-Piero
anchors are designed for simple
installation and to maximize the
load capacity of each helix. Shafts
and helices are ofsteel selected to
meet specific installing torques and
design loads. Bending moments are
of little concern when the piers are
installed because soil sufticientlv
supports the shafts.
Installing equipment may be a
utility truck, caisson drill, backhoe,
skid-steer loader or hand-held
rotary tool. As proper installation
affects performance, criteria
include high torque, low speed,
down pressure and angle control.
A torque-to-capacity ratio of 1:10 is
excellent to verify capacity. Read
torque on the installing equipment
meter to assure soil consistency
during final depth equal to three
times the anchor helix diameter.

DESCRIPTION
Tension/Compression (T/C) Helical-Piero anchors are
designed for loads that often experience high compres-
sion and uplift as well as lateral forces. The lead section
resembles the HS (see above) type, but T/C extensions
are a heavy pipe shaft (6" or 8" diameter).
Load capacites of 100,000 lb. tension/compression are
common with the T/C type Helical-Pier design.
$pical applications are self-supporting towers, refinery

conducted an analysis to determine the appropriate spacing for underpinning piers. The
purpose of the piers is to stabilize the foundation movement which is occuning on the
south (downhill) side ofthe residence.
Based on a load capacity per pier of 40,000 pounds as provided by the manufacturer, a
maximum pier spacing of 8'-0" along both the south side and the east side of the existing
building is appropriate.

1l Tear off existing roofing and remove all debris from the site. Plath Construction will
keep thejobsite as clean as possible and practical during the construction, and a thorough
cleaning of Plath's work areas will be performed upon completion. Plath Construction will
take all reasonable precautions to protect the interior of the building from water and
weather. However, Plath Construction shall not be held liable for any interior damage
sustained from leaks caused by sudden and/or unforseen storms that occur durilg the tear
off phase of this work.
2l Tear off and remove four solar panels. Panels to be disconnected from any plumbiag or
electrical by others.
3l Inspect the plywood for water damage, and replace as needed.
** This work, if required, will be an additional charge over and above the contract price,
and will be charged at the rate of $ 1.85 per square foot. No additional charges will be
incurred without prior notification of, and authorization from, the owner or owner's
agent.**
4l Plath conshuction will provide all insurance and permits required by the Town of Vail
Building Department.

1l Install 'Jiff-Seal' Ice and Water Guard by'?rotecto 'Wrap' over entire pitched roof area,
according to manufacturer's specifications. Extend membrane up walls a minimum of 12"
at roof-to-wall junctures. Use mastic as needed.
2l Install Heritage II, 30 year asphalt shingles. Use starter and hip-and-ridge shingles as
needed.
3l Install 26 gauge galvanized step flashings at side wall locations.
4l Install 24 gauge prefinished metal endwall flashing and drip edge flashing at eaves and
rakes.
5l Instatl (eight) RV-53 roof vents at locations 18" down from the ridge.

2l Winter work is not figured in this proposal. If roofing work is falls between November
15th and April 15, additionat charges may apply to cover our added expenses for working
under adverse winter conditions. Any such charges shall not exceed lWo of the remaining
contract amount and are in addition to any snow removal and priming charges that may

3] Plath Construction carries property damage liability insurance in the amount of
$1,000,000,00 per occurrence. Ptath Construction carries auto insurance in the amount of
$1,000,000.00. Plath Construction carries appropriate Workmanh Compensation insurance
through Colorado State Compensation. No further insurance coverage is included is our
price, and if required, the additional cost shall be added to the contract price'
4l Upon receipt of the contract price, Plath Construction will issue a waranty certificate
guaranteeing roofing against leaks due to flaws in workmanship for a period of 2 years from
ihe date of ioofing completion. Leaks qualifying under tJre terms of said warranty will be
repaired promptly at no cost to the owner. This warranty does not coYer any subsequent
interior damage that may occur.
