As requested, a representative from our office observed the foundation materials e�cposed in the open excavations for the
proposed entry footings and interior footing pads for the existing residence located at 454 Beaver Dam Road on August 27 and
28,2009. The purpose of our site visits was to verify that the eacposed materials will support the proposed foundations with a low
risk of movement.
The footing excavations observed were located at Gridlines C.0/1.6, C.0/2.1, C.0/3.1, A.3/2.5, A.6/2.6, and E.0/3.0-5.0. The
excavations varied in depth from approximately grade to 3.5 feet T'he material e�osed in the base of the fooNng excavations
consisted of compacted 3-inch minus rock. The subgrade soils below the 3-inch minus rock consisted of a brown to dark brown,
moist to wet, dense, sandy, silty gravel. The 3-inch minus and subgrade had been compacted, and neither organics, nor
deieterious materials were observed in the base of the footing excavations. However, ground water was observed in the
excavation for the front entry along Gridlines E.0/3.0-5.0.
Pmvided that the graund water is removed prior to the placement of concrete,it is our opinion that the subgrade soils will support
a foundation system designed for a ma�cimum allowable soil bearing pressure of 2,000 psf with a low risk of movement. Our
opinion is based on the foundation subgrades being dry,compacted,unsaturated,and frost-free.
The opinions presented in this letter are based on our previous subsurface e�loration,our observations,and our e�erience with
similaz soil conditions in the area. It is possible that subsurFace conditions could change below the exposed excavation depth
encountered at the time of our visi� We appteciate the opportunity to provide this service. If we can be of further assistance,

The framing for the project is 100%complete at this time. The purpose of this inspection was to make a final review
� of the framing and to scrutinize the penetrations through the framing members that have been made by the
subcontractors.
` I found that the framing has been done according to the plans and specificarions certified by this office. I did not
find any issues with the penetrations through the members made by the tradesmen.
The only deviation from the construction documents is the floor framing over the Game Room in the Lower Level.
i The original framing for this floor was not done per the original contract documents. T'he double LVL header and
column were not present. Therefore, the four LVL beam was deleted. Joists,span from the interior bearing line to
the north exterior wall in this area, and three double LVL beams have been installed to pick up point loads from
above. A

Asbestos Testing &Abatement Requirements
Asbestos testing and abatement protects workers, homeowners, neighbors and emergency services responders from ex-
posure to harmful asbestos. The Town of Vail asbestos abatement program is in addition to the State of Colorado's regu-
lations. It is your responsibility to be in compliance with the State. Please contact the State directly for their requirements
at the contact info listed below.
When is asbestos testina reauired7
ANY building projects disturbing more than these threshold levels of building materials require asbestos testing:
One-and Two-Family Dweilings: 30 square feet
Ali Others: 160 square feet
Asbestos testing results must be provided with your application for a building permit.
Tests which identify POSITIVE results at more than 1% require abatement by a State-certified and Vail-registered abate-
ment contractor. An asbestos abatement permit must be approved, and the clearance letter must be submitted to the
Town of Vail before the building permit will be issued

INTRODUCTION:
On March 9th, 2009 an inspection/survey was conducted and 10 bulk
samples were collected from the:
The Residence
454 Beaver Dam Road
Vail, CO. 81657
The purpose of the inspection/survey was to locate and sample
suspected Asbestos containing materials that might be present in the
Residence that is planned for selective interior renovation.
The inspection was made, and the samples were collected by John R.
Peterman, an A.H.E.R.A. and State of Colorado Certified Asbestos
Inspector. Great care was taken during the inspection and sampling to be as
accurate as possible. It should be noted that minimal darnage was done to
the existing building structures during the inspection so there is no
documentation for unseen conditions or stored items.
All samples were analyzed by DCM Science Lab in Wheatridge, CO.
This laboratory is deemed "Proficient" in the E.P.A.Quality Assurance (QA)
program for the determination of asbestos in bulk materials, and is
accredited by the American Hygiene Association (AHA).
SAMPLING PROTOCOL:
A random sampling scheme was used to sample the suspect materials that
were discovered. If during any future demolition or renovation work,
suspect material is discovered that hasn't been sampled and would be
disturbed, work should be halted until the material has been tested.

RENOVATION AREA DESCRIPTION:
The main house, hot tub room, and the guest house are planned for selective
interior renovation. The interior walls are covered in sheetrock with a
troweled-on texture, and wood paneling. The ceilings are covered in
sheetrock like the walls, and exposed wood. The floors are finished wood,
stone and mortar, ceramic tiles, and carpet. The heating is provided by a hot
water system, and fire places. No suspect materials were visible on the
heating system.
CONCLUSIONS & RECOMMENDATIONS:
Laboratory analysis, of the bulk samples, indicates that no Asbestos was
detected in any of the bulk samples.

BULK SAMPLE ANALYSIS PROCEDURES: Page�of�
DCM Science Laboratory, Inc. analyzes bulk asbestos samples following procedures developed by the
McCrone Research Institute and in compliance with guidelines established by the Environmental Protection
Agency(EPA-600/R-93/116, July, 1993).
Bulk samples are prepared for analysis using a 10X-80X stereo microscope in a hepa filter hood which provides
a contamination-free environment. The sample is then analyzed by polarized light microscopy(PLM) at 100X.
When the sample consists of more than one layer, each layer is prepared and analyzed separately. Fiber and
matrix materials are identified �ay the characterization of optical prope�ties including color and pleochroism,
form, cleavage, relief, birefringence, extinction, orientation, twinning, interference figure and other distinguishing
features. Dispersion staining is also used to further aid in mineral identification. All percentages of asbestos,
other fibers and non-fibrous constituents are calculated from the values obtained from analyses using the
stereo and PLM microscopes. In-house and NIST standards as well as a chart prepared by R.D. Terry and
G.V. Chilinger for"The Journal of Sedimentary Petrology", (Volume 24, pp. 229-234, 1955) provide a guide for
estimating percentages. All samples are archived for six months unless other arrangements are made by the
client.

with NVLAP requirements unless otherwise noted.
