i �isited the site referanced above to observe the structural framing. I noted the following items which do nat
comply with the intent of the structura! drawings:
1. The north, south and west exterior walks of "Guest Room 302" were not sheathed with plywood described in
the General Notes, Structural Wood �raming, item 12 and in the shear wall notes on plan. We understand that
this cannot be done until after inspection and +RSUlation, but prior to interior finishes, the plywood sheathing musf
be installed. In addition, the north and south facing shear walls do not contain the blocking specified on plan
(see notes sheet S4).
2. I observed bolts and lags that are not tight, l"his may be a result of the initial shrinkage of timber members,
however, all lag and bo[t connections must be checked to verify that they are properly iightened,
3. The naiEing pattem af new exterior sheathing does not comply with the requirements in many areas. Review
the installation of all new exterior sheathing to verify that the nail spacing does not exceed that described in the
General Notes, Structural Wood Framing, item 12 or s�ecific shear wall notes on plan.
4. I observed multiple built-up posts that were nat composed as specified on plan, or that are missing
altogether. These include but are not limited to the (4}-2x6 post suppor�ing the east end of the southern-mast
6x12 at ihe Master Bedroom and the {3}-2x4 post at the wesi end of the W 8x18 upper floor beam. In addition,
some of the posts thai are instalked correctfy lack the 5olid blocking required in the flaor space directly below.
Verify that all posts match thase specified on plan and that all posxs are packed out solid continuous to t}�e
concrete foundation.
5. The (4)-2x6 built-up posts at the east wall of "Guasi Room 302" are suppased to ha�e {2}-1 '/. x 5'/z LVL
king studs on each side of the trimmer pack; only one was installed each side. Add the missing king studs. The
single LVL king stud in the south wall is adequate.
6. The south upper roof 2x12 deck joists are significantiy crowned and aften do not bear on the supporting 8x14
timber beams. Steel shims appear to have been �nsfafled; however, many of them are not tight Add st�el
shims such that all joists bear on the supporting beams at all beari�g points.
7. The 2x12 ledger at the east side of ihe office roof framing has been cut through for a drain. Add (1)-16d
drive-pin c�,D 12" into the adjacent TS 6x3x3l16 horizontal member.
8. The double 1 3/ x 11 %< LVL joists of the office floor are designed to support heavy file cabinets. They do not
bear within S" of supporting studs at the entry wall below, Add studs to align with the jaists.
9. The new kitchen balcony was constructed with fwo cantilevered MC 6xT2's. We believe that these steel
sections and the connections ai the supparting ends (south) are adequate to support the design load.

10, The Master Bathroom door header has been cut to allow passage af a mechanical duct. It does not span
ihe entire opening. We recommend that the 2x1Q rim with a ripped 2x spacer be removed across the opening
and repiaced with a single 1 3/4 x 11 718 LVL, Lag the LVL to the cut {2)-2x10's with {2)-112" dia. x 4" lags (c� 16"
(6 lags rninimum).
19 _ The eXisting 2x10 joist beneath the Dining Room table and above the sauth end of Bedroom b06 requires
reinforcement with a single piece of 1 1/8" plywood 9" x 2'-0 at twa locations.
12. The Entry �loor joists (between the stair and caat closet} were described on plan as'1'/< x 9'/. LVL @ 16"
CONT. 2 SPAN UND�R STONE', 7his requirement was to stiffen the floor over the 15 foot span, limiting the
deflecEian to prevent damage to the stone fnished surface. The floor was actually framed as simple span joists,
hung from a flush header abave the lower level door. We recommend adding a pair of C 5x6.7 back to back
(one on each side of each of the three existing LVL's}. 3he channels must run up to a maximum distance of 1'-
8 from bearing at each end. Connect with'/" diameter through bolts @ 16".
Oncz these items are corrected, we be(ie�e that the overall structural infegrity will be sound. If you have any
questions regarding these recommendations, pl�ase do not hesitate to call.
This site visit was conducted to obsenre the ganeral progress and quality of the structural aspect of construction
and shafl not be construed as inspection. The items listed in this report are observations only and do not
exclude the possibility that other structural deflciencies existed at th� time af the sife visit but were not r�carded.

was able ta get a copy of �'rinity's Welding Procedure Specification �nd also Mr. �,eihseiYs Welding

INTRODUCTION:
On January 28, 2004, an inspection/survey was conducted and 3 bulk
samples were collected from the:
Residence
303 Gore Creek Drive
Unit # 8
Vail, CO. 81657
The purpose of the inspection/survey was to locate and sample
suspected asbestos containing materials that might be present in the Building
that is planned for renovation. ,
The inspection was made, and the samples were collected by John R.
Peterman, an A.H.E.R.A. and State of Colorado Certified Asbestos
Inspector. Great care was taken during the inspection and sampling to be as
accurate as possible. It should be noted that minimal damage was done to
the existing building structures during the inspection so there is no
documentation for unseen conditions or stored items.
All samples were analyzed by DCM Science Lab in Wheatridge, CO.
This laboratory is deemed "Proficient" in the E.P.A.Quality Assurance (QA)
program for the determination of asbestos in bulk materials, and is
accredited by the American Hygiene Association (AHA).
SAMPLING PROTOCOL:
A random sampling scheme was used to sample the suspect materials that
were discovered. If during any future demolition or renovation work,
suspect material is discovered that hasn't been sampled and would be
disturbed, work should be halted until the material has been tested.

The Residence at 303 Gore Creek Drive, Vail, CO. is planned for
renovation. The renovation will involve the interior walls and ceilings for
the most part. The interior wa11s are sheetrock with a light texture. The
ceilings are sheetrock like the walls. The floors are carpet and finished
wood. .
CONCLUSIONS & RECOMMENDATIONS:

BULK SAMPLE ANALYSIS PROCEDURES:

DCM Science Laboratory, Inc. analyzes bulk asbestos samples following procedures developed by the
McCrone Research Institute and in compliance with guidelines established by the Environmental Protection
Agency (EPA-600/R-93/116, July, 1993).
Bulk samples are prepared for analysis using a 10X-80X stereo microscope in a hepa filter hood which provides
a contamination-free environment. The sample is then analyzed by polarized light microscopy (PLM) at 100X.
When the sample consists of more than one layer, each layer is prepared and analyzed separately. Fiber and
matrix materials are identified by the characterization of optical properties incfuding color and pleochrorism,
form, cleavage, relief, birefringence, extinction, orientation, twinning, interference figure and other disiinguishing
features. Dispersion staining is also used to further aid in mineral identification. All percentages of asbestos,
other fibers and non-fibrous constituents are calculated from the values obtained from the stereo and PLM
microscopes analysis. In-house and NIST standards as well as a chart prepared by R.D. Terry and G.V.
Chilinger for "The Journal of Sedimentary Petrology", (Volume 24, pp. 229-234, 1955) provide a guide for
estimating percentages. All samples are archived for six months unless other arrangements are made by the
client.
