This checklist is to be used with any single family/dupiex permit (new construction , addition, or remodel application.)
��' Town of Vail Design Review Board approval must first be obtained (may not apply to interior remodels)
I�� Plan Check Fee must be paid at the time of application for projects over $100,000 valuation (see attached
schedule)
The following information must be shown on all 4 sets of plans:
Architectural Plans
� Site Plans. Provide all site plan information as required for the Design Review Application for your project.
Refer to the DRB application checklist for complete details.
�/ Construction staging plans. Provide construction staging and materials storage site plans.
C�' Floor plans. Complete floor plans provided for each level. Complete dimensions, drawing scale noted, use of
each room shown on the plans. Location of inechanical equipment clearly shown
�I�' Building Elevations N,W,S,E elevations. Show all proposed exterior finish materials, guardrails, windows,
doors, and finish grades.
�►'� Window sizes and operation types. Specified on the floor plans or elevations.
[� �' Stairways, guards, and handrails Show all stairway details with rise/run, handrail and guard details
C��' Roof plan. Show all roof covering materials (Class A covering required) and underlayment, roof pitch

Structural Plans
C�'� Soils Report. Include 2 copies of the soils report for your lot.
C� All sheets of the structural plans stamped and signed by a Colorado State Licensed Engineer.
C�r Design specifications sheet. Roof live load, Deck live load, Floor live load, Wind Speed/Exposure, Soils report
number and soil bearing capacity referenced per the soils report.
�Foundation plan. Provide a complete foundation plan with all footing/foundation section details
C� Framing plans. Provide complete framing plans for floors, decks, roofs. All beams, joists, rafters or trusses
clear�y shown. Include framing construction details and connection schedules.

BUILDING PERMIT APPLICATION CHECKLIST
COMMERCIAL TENANT FINISHIIMPROVEMENT
This checklist is to be used with any tenant finish/improvement permit application.
❑ Town of Vail Design Review Board approval must first be obtained for any exterior modifications proposed
❑ Plan Check Fee must be paid at the time of application for projects over $100,000 valuation (see attached
schedule)
The following info ation must be shown on all 4 sets of plans:
❑ Construction lans stamped and signed by a Colorado State Licensed Architect.
❑ Original buildi g construction type per IBC chapter 6 ex. II-A, V-B
❑ Tenant suite or enant space number.
❑ Tenant space lo tion identification. Show location of the tenant space within the overall building.
❑ Floor plans with isting and proposed walls. Show all wall assembly construction and proposed interior
finishes with flamesp ead index. Proposed construction type must match existing building construction type.
Clearly show propose use of each room or space on the plans.
❑ Reflected ceiling and ish plan. Type of materials must match the fire resistance/type of construction
requirements in the existi building. Show all interior ceiling finishes and flamespread index
❑ Door schedule. All doors s all be clearly identified with fire rating, size, hardware and swing.
❑ Means of egress plan. Sho all occupant loads and number of exits from the tenant space
❑ Areas requiring two (2) exits: xit doors shown to swing in the direction of exit travel, separation of exits
comply with code, illuminated exi igns are shown.
❑ Windows shall be labeled with size, operation, safety glass.
❑ All areas and rooms of the tenant sp e are accessible per ANSI 117.1 requirements.
❑ Accessible maneuvering clearances vided at all doors for accessibility per ANSI 117.1
❑ Service counter-tops and built-in work tions provided with accessible areas per ANSI 117.1
❑ Bathroom facilities. All accessible/adaptabl features, clearances and turning spaces provided per ANSI 117.1
requirements must be shown.
❑ Separate sex bathroom facilities provided if the umber of occupants or employees exceeds (15).
❑ Toilet room floors/walls finish schedule.
❑ Floors have a smooth, hard, nonabsorbent su ce such as concrete, ceramic tile or other approved
material, extending upward onto the walls at least 5".
❑ Walls within 2 feet of the front and sides of urinals d water closets must have a smooth, hard
nonabsorbent surface to a height of 4 feet.
❑ Occupancy separations for walls/ceilings between tenants early shown on the plans with fire resistive
construction details and listings specified.
❑ Fire-stopping. Fire-stopping penetration details for plumbing, m hanical and electrical components in all fire
rated walls and ceilings.
❑ Plumbing, Electrical and Mechanical Plans stamped by a Colora State Licensed Engineer
Mechanical HVAC plans, plumbing plans, electrical plans/panel schedul lload calculations.
❑ Fire/smoke Dampers on mechanical plans at all duct penetrations into fire esistive walls and ceilings.
❑ Fire sprinkler plans. Show all proposed work per Vail Fire and Emergency rvices requirements
❑ Fire alarm plans. Show all proposed work per Vail Fire and Emergency Servic requirements

I continue to reach out to you to get a response on your requested reimbursements in addition to the
permit. We are ready to provide payment for the permit, subject to you transferring it to THD
Colorado.
I have copied Joe Suther with the Town of Vail with my request for the permit to be transferred to THD
Colorado, Inc, Address: PO Box 8338 Avon, Co. 81620; License Number: 280-A (Town of Vail) and
request you reply with your authorization on the permit transfer as well. Please reply on this request and
your thoughts on the time/costs for assisting with the plan. I request a response by Sunday, March 16th
or we will look to pull a new permit for the job.

As requested on 2/19/08, CEA performed a limited asbestos building inspection, includes
a tactile and visual survey at the above referenced location of accessible materials in the
residence (request by the client).
The purpose of asbestos sampling was to document the specific building materials that
may contain asbestos prior to any renovation, reconstruction or demolition and remediation.
All samples are then taken and delivered to Reservoirs laboratory, a certified and
accredited laboratory. The laboratory analytical results, if any of the samples are attached to this
report. We do not accept any responsibility for these analyses.
Samp��n� �"I�ype/Methc�d �: Bulk sampling — samples were collected using wet methods to
minimize the possibility of a fiber release. Bulk sampling; is done by knife or other similar
coring devices, suitable to cut through the entire thickness of the material being sampled. Bulk
samples are for the purpose of a cross-section of the sample building material to be collected and
sent to the lab for identification.
Site Visit/Work periormed:
To save the building owner additional expenses, we tested randomly throughout the residence as
required by Colorado Reg 8 guidelines for suspect materials that may have contained asbestos.
In some areas; work performed was a non-destructive sampling inspection, in other areas was a
minimal destructive sampling. These azeas will need to be abated by a professional abatement
contractor based on the lab sample results. These areas are listed on the assessment form, 9
samples were taken...

All samples show no asbestos (Non-Detect), except the sample listed below with a trace
asbestos in Reservoirs Laboratory report.

IV.E.3.- a homogeneous area is considered not to contain ACM, only if the results of all
samples required to be collected from the azea show asbestos in a amounts of one percent or less.
IV.E.3.b — a homogeneous area shall be determined to contain ACM based on a finding
that the results of at least one sample collected from the area show the asbestos is present in an
amount greater than one percent.
All samples meet Colorado Regulation 8/ section IV.E.3.a., which does not require
abatement in these area(s). This renort is limited to these 9 samples areas taken or other areas
outline in the report as written "suspected ACM....."
Principal Findings:
Cove base (Hot water heater closet)
Analysis of the Tan cove base has determined that the cove base contains no asbestos (Non
Detect), Tan mastic contains TR Tremolite-Actinolite asbestos (PCT -< 0.25%).
Scope / Area of work for Abatement:
No abatement required, does not meet Colorado Regulation 8/IV.E.3.b or USEPA regulations.
See recommendations below.
Observations (Visible Inspection):
Two floor structures with crawl space, built in early 1980's, stucco/wood exterior. Stone
tile in main hallway, bathroom and kitchen, laid on top of vinyl floor (in Hot Water Heater
closet). Wooden floor in the bedroom on the left as enter the unit and in the living room / dining
room with a foam covering on sub-floor. The Kitchen/Living room/Dining room ceiling(s} and
Hot water heater / crawl space closet are homogenous in surface texture (sprayed on), different in
-• paint color. The Kitchen / Dining room and living room wall(s) are homogenous in smooth
surface texture (trowel on plaster), different paint color.
Electrical is modern wiring, recessed lighting in kitchen has overspray texture inside the
units. Crawl space is concrete foundation and very damp, there is no barrier covering dirt floor
and insulation is between floor joists. There is plastic and copper plumbing, unit is heated by
radiant heat.
Recommendations:
GC ad-here to OSHA 1926.1101 / class II standazds, USEPA & Colorado Regulations.
Per Colorado Regulation 8/ part B, requires additional asbestos testing of the residence, when
remodeling / renovation or demoing and remediation in the remainder of the residence.

technically e�austive and is not to be used as a guarantee or wananty, expressed or implied, regarding
the adequacy, performance or condition of the inspected structure. This report is subject to the limitation
as agreed in writing.
