Architectural Plans! Site Ptans. Provide all site plan information as required for the Design Review Application for your project.
Refer to the DRB application checklist for complete details.! Gonstruction staging plans. Provide construction staging and materials storage site plans.
D Floor plans. Complete floor plans provided for each level. Complete dimensions, drawing scale noted, use of
each room shown on the plans. Location of mechanical equipment clearly shown
! Building Elevations N,W,S,E elevations. Show all proposed exterior finish materials, guardrails, windows,
doors, and finish grades.
n Window sizes and operation types. Specified on the floor plans or elevations.
I Stainrays, guards, and handrails Show all stairway details with rise/run, handrail and guard details
! Roof plan. Show all roof covering materials (Glass A covering required) and underlayment, roof pitch
! Building cross sections. Show roof, wall, floor construction assemblies and insulation R values. Show roof

Provide a complete signed compliance certificate and inspection checklist. Verify all exterior building is detailed
on the building plans as required on the Rescheck compliance report.

Introduction:
On April 18,2008, Boulder Environmental Management, Inc.'s air monitoring specialist,
Mark Symmank, collected final air clearance samples from 641 West Lionshead Circle
#216,Yail, Colorado.
Prior to testing, a visual inspection was performed to confirm the completeness of
asbestos removal and cleanup. The visual inspection was performed in accordance with
the American Standard for Testing and Materials' "Standard Practice for Visual
Inspection of Asbestos Abatement Projects". All abated asbestos materials, including
contaminated dust, debris, or residues were satisfactorily determined to have been
removed.
Air samples were then collected inside the contained area utilizing aggressive air
sampling techniques as described in 40C.F.R. Part763, Appendix A to Subpart E (EPA
1995).
Five air samples and two blanks were analyzed utilizing Phase Contrast Microscopy
(PCM) employing the NIOSH 7400 method. The results of the PCM analyses as follows:

Discussion:
Samples were analyzed by a trained analyst under the direct control ofBoulder
Environmental Management, Inc, Boulder Environmental Management, Inc. successfully
participates in the American Industrial Hygiene Association's Proficiency Analytical
Testing Program (PAT) for determining concentrations of airborne asbestos fibers.

Conclusion:
The sample results complied with the State of Colorado Department of Public Health and
Environment regulations for post-abatement air quality.

30" above grade, you must also obtain a building permit.
If this permit is for installation of an exterior hot fub or spa on any existing dec* or elevated platform,
a structural engineer must review the existing conditj,on and veriff that it will support the added
concenfrated load. Please provide a copy of the struchrral engineers wet stamped letter or drawing
with this application.

This letter is intended to describe the full scope of work for changes to the fire sprinkler system that will
be performed by Western States Fire Proteclion to accommodate the bathroom remodel in unit 216.
Required modifications to the fire sprinkler system are as follows:
: Relocate sidewall head in master bathroom
: Eliminate sidewall head and pendent head not needed per floor plan.
: Relocate sidewall head in second bathroom.
It is our undersianding that this letter will be sufficient to obtain a permit to proceed with the work
described above. No other work will be done without authorization from VFPD. lf further information is
required to issue a permit please contact ouroffice in Glenwood Springs at 970€18-3294.
