Box 756 Minturn, Colorado 81645 -0756 RE: ARCHITECT'S REVIEW AND ASSESSMENT OF DAMAGE FROM FIRE 1216106 TO TOWNHOMES D- 1 & D -2, AND RECOMMENDED REPAIRS: Dear Jeff, The fire was started and was contained within the common wall between townhomes D -1 and D-2 just inside the corner of the walls at the upper triangular deck. It burned primarily within the floor structure of the bedroom floors on each side of the upper triangular deck. Specifically it burned in and from the solid blocking here in the common wall side of D -1. The common wall between the two townhomes is two 2 "x4" stud walls separated by a Y" air space. Each stud wall has 518" gypsum board on the townhome side. The solid blocking is between the floor joists to act as a fire stop between the units. The blocking that burned initially and is where the fire started from was water damaged from the leaking at the upper triangular deck. Thus it acted as kindling making it easy to start and get going. At the time of discovery, the fire had bumed through the blocking here and was spreading to the joists, sills, studs, and sub floor within this space, but had not yet gotten past the 5 /8m gypsum board into the living spaces. The smoke from the fire was initially discovered by the roofer (Douglass Roofing) who was water proofing the upper triangular deck of D-1. They immediately reported the fire to the contractor project manager, Jeff Armisted, who opened it up and put it completely out. It is my understanding that Douglass Roofing had nothing to do with the start of this fire, only its fortunate discovery. Mr. Armisted did everything correct and expected to do with knocking the fire down and putting it out before more damage could occur. He then immediately contacted the Architect and then ail necessary parties in Vail. He had the area of the fire photographed with infrared film to determine any hotspots not taken care of. He had the damage inspected by the Vail fire chief and the property management company. The next day, Dec. 7, the Architect inspected the damage with Mr. Armisted and went over the following repairs with him on site, As to repairs, it is important to remove all the charred wood that was burned or scorched, and maintain the fire blocking between the two townhomes. No serious structural damage was caused by the fire. Most damage was to fire blocking and charring to sills, joists, and studs which will need to be replaced and/or reinforced if partially cut out. 1) Cut away and remove wall finishes and gypsum beard to expose fire damage inside the stud walls. Cut out areas of gypsum to minimum of 32" and 48" dimension-, preferably to 48 "x48" or larger. Leave open until Architect is allowed to inspect. 2) Peel back carpet on floor levels at fire damage level. Carpet will be re- stretched in place. If easier, cut out carpet and have carpet installer re- stitch carpet piece. Cut out carpet piece large enough to have new seams in logical, non - obvious locations. Leave open until Architect is allowed to inspect. 3) In north bedroom on floor at fire, remove the gypsum board at wall west just inside bedroom door where light switch and duplex outlet are. Examine the wall stud cavity and especially the electrical 4010 FIRE DAMAGE ASSESSMENT: 12/12/2006 _ PAGE 2 OF 2 wiring, switch, duplex and electrical boxes. Replace as required if damage evident. Leave this area open for Architect's inspection before effecting repairs. 4) Remove charred, fire damaged dbl. 2 "x10" blocking. Replace with dbl. 2 "x10 " blocking tight between. joists. 5) Cut away and remove charred, burnt portions of 2 "x4" sills and studs, and replace or reinforce with new. 6) Cut away and remove charred, burnt plywood floor sheathing, and replace with new. Provide new solid blocking under seams with existing plywood sheathing. 7) Cut out floor joist right above entry door to D-1. Replace with two 2 "x10" joists, tying them into balance of joist left above to the east of the entry door. Block in above wail and entry door. 8) Determine whether the low voltage, phone wiring damaged by fire is still alive or abandoned. If abandoned, remove fire damaged portion. If still active repair with approved repairman doing the work. 9) Mark adjacent structural surfaces near fire damage in permanent ink, giving date of fire, and repairs made by O'Halloran Construction to document the fire and repairs made. 10) Accomplish removal of fire damaged material and replacement with new material by Tuesday, Dec. 12, for Architect's on site review, prior to covering up. 11) Replace damaged and /or removed batt insulation. 12) After Architect's inspection, replace all gypsum board and other finish materials /textures. Paint and have Townhome interior content thoroughly cleaned to get rid of smoke damage and construction debris. The purpose of our site visit was to observe and test the soil exposed in the foundation excavation for the proposed decks. There were holes for nine deck: pads at the back of Building A, and two holes in front of buildings A, B, C, D, and E. All holes were about four feet deep. The footing pads for the decks will be constructed with big foot concrete forms, with a 24 to 28-inch wide bell bottom. The soil exposed at the bottom of the holes consisted of sandy gravel with cobbles. Ae sail exposed in the foundation excavation for the deck additions is suitable for a maximum allowable soil bearing pressure of 2000 psf. Attleson- Project Architect RE: Fire @ Sundial Townhomes in Units Dl and D2 on 1216/2006 O�b Qo 6- orV 3 p IEC dED JAN 11 2097 TOWN OF VAIL The purpose of this letter is to report the small fire that was accidentally started at the Sundial Townhomes on the 6" of December, 2006. O'Halloran Construction is the General Contractor for this exterior remodel project of the (21) Units at 5040 Main Gore Place. We commenced demolition and construction of this project in mid July of 2006 and have been working on the project since that date. Upon inspection of the decks after the demo was completed, it was determined by the Architect that the structural integrity of the existing decks would not meet current TOV Building Codes. Part of our scope of work was to demolish the (16) upper triangular exterior decks that are on the second floor of the interior units. The building permit was amended per the drawings submitted by the Architect and we were commissioned to correct the water damaged double 2x 12 rim joists, re- construct the triangular decks and waterproof them with a product called HydroTech. The HydroTech product, which is applied at around 400 degrees, requires that the plywood substrate be 100% dry and free of ice, water and dirt. We had made efforts to protect the plywood substrate on these decks from weather and the elements with reinforced plastic. Due to freeze /thaw cycles and snow accumulating on the roofs, water did get under our plastic and subsequently formed small amounts of ice build up. We made attempts at chipping and scraping the ice off, but could not get the decks 100% dry and free of ice and snow, We then decided that we would need to thaw the ice and dry the decks. We also needed to stay ahead of the roofing company (Douglass Roofing) and the inclement weather to maintain our construction schedules. The method that we chose to implement to thaw the decks was with a "weed burner ", which is a type of hand -held propane torch. This was to be used on the horizontal surfaces of the decks in brief passes to melt the ice and then broom or "squeegee" the water off the front edge of the deck. Then the plywood was left to air dry for 30 minutes prior to the waterproofing application. The existing structure was not originally framed very "tightly" and there are numerous places where there small gaps between the sill plates and the plywood sheathing existed. It was in one of these gaps in the existing structure where the fire ignited the old rotten wood. One of O'Halloran Construction's employees was using this method on the deck of Unit D1 at the time that the fire started. He was being supervised by me (Jeff Armistead, Superintendent). I was on -site at the time. The fire started in an inside corner of the common wall between Units D 1 and D2. The torch passed over a small 1" gap in the structure where there was a rotten; water damaged 2x 12 behind the sheathing. This provided the "kindling" for the fire to grow within the 2`1 floor structure. The fire was initially reported to me by one of the Douglass Roofing employees who were installing the HydroTech waterproofing. I am thankful that they reported the fire to me because it could have been a very dangerous situation had they chose to ignore it, fearing blame. With the help of my carpentry subcontractor's men, we removed the exterior plywood sheathing at that location to expose the fire itself. No open flames were observed, only the red cinders burning on the double 2x12s, a small section of plywood and the sill plates of DI and D2. I estimate that the fire burned approximately for 30 -60 minutes inside the floor structure before we were able to extinguish it completely. Extinguishing the fire required removal of a small section of sheetrock in the ceiling of the interior entry of Dl, a small section of sheetrock in the 2 "d floor bedroom wall adjacent to the deck of Dl, a small section of sheetrock in the 2 "d floor bedroom of D2 and a small section of sheetrock in the closet ceiling of D2. We had a water hose connected to a hose bib on Building C next door and we were able to spray water directly onto the burning wood to extinguish the fire. We did not need use any of the fire extinguishers that were on site at the time of the fire, only water. After the fire was extinguished, I reported the fire to the Project Architect, Bob Attleson via phone. I also contacted the Fire station in East Vail to request an inspection by the Fire Department. The fire engine crew and Fire Chief, Mike McGee and Mike Vaughn arrived within 20 minutes of my request. Mike McGee did observe the (1) small fire extinguisher that Douglass Roofing had next to their kettle and did comment that they need to have (2) larger extinguishers on site at all times. The Fire Dept, removed minor amounts of sheetrock from both units to gain a better view of the damaged areas and also observed the areas with another infrared camera. They confirmed that the fire was completely extinguished and were satisfied with the efforts made by myself and my workers to address the situation. Mr. McGee instructed me to draft this report and to submit copies to himself and the T.O.V. Building Dept. I took digital photos of the damage that day and they are available at any time for review. The Architect inspected the damage the next day and also took extensive digital photos to document the damage. His report is also attached for review with regards to the repairs that O'Halloran was instructed to make. Per his report, there was no structural damage to either unit. The damage was more non - structural blocking, plywood sheathing, sill plates and sheetrock. The repairs noted on the Architects report are being implemented at this time and should be completed by the 22 "d of December. O'Halloran also contacted Steammaster on within I hr of the fire and they "fogged" both units that evening to mitigate the smoke damage. They will be performing a secondary "fogging" of Unit D1 on Tues. December 19`n. They will also be steam cleaning the carpets and performing content cleaning in Unit Dl on Thurs. December 21 ".
