ALL utilities must field verify ({ocate) respective utilities prior to signing application. Some utility companies require up to 48 hours notice to schedule a locate. % A construction tranic control/stagirg plan must be prepared on a separate sheet of paper. An approved site plan may also be us2d. This plan wiil show locations of all traffic control devices (signs, cones, etc.) and the work zone, (area of construction, stoging, etc.). This plan wiil expire on November ls� and wili need to be resubmitted for consideration for approva! through the winter. Be aware that your resubmission for winter may be denied depending on the location of construction. > Sketch of work being performed must be submit�ed indicating dimensions (length, width and de�th of work).
